    On August 25, we held a webinar on “Governing through COVID-19 and Beyond” to discuss governance at this time of uncertainty and change. It included guidance and advice to board members on resumption of services; what governors should know about OHTs; and where we can go from here to continue to strengthen organizational governance practices and prioritise board efforts.

    In a time of need: a grassroots initiative in response to PPE shortage in the COVID-19 pandemic

    Article in the July edition of Healthcare Quarterly about the initiative to address a critical shortage of personal protective equipment in the London–Middlesex region.     GoodLife reopening- existing rates extended until November 14
    GoodLife rates for AFHTO members were due to increase at the end of May, but because of closures due to COVID, existing – annual (paid in full) rates of $519 and biweekly rates of $24- have been extended until November 14. Those with existing GoodLife memberships should have already received emails from GoodLife. Phasing updates by region are available on their site here.

    To take advantage of discounted rates, you need a membership ID assigned by AFHTO. Automatically assigned to those on the ‘Bits & Pieces’ mailing list, these are sent to GoodLife monthly near the end of the month. To get your ID or be put on the list for one, please email info@afhto.ca.

    As of November 15, annual (paid in full) rates will be $619 and biweekly rates $25.

    Dear AFHTO members,
     
    Are you interested in serving on the AFHTO Board of Directors? The governance committee of AFHTO’s board invites anyone who works within an AFHTO member organization to apply. We are looking for strategic and emerging leaders to help shape the future of interprofessional team-based care in Ontario!  

    Please share this call for nominations with all who work in your team.

    Six (6) positions are to be elected for a 3-year term on the 14-member board. The AFHTO by-laws call for balanced representation on the board to include the various forms of governance, the regions of the province, and the mix of the professions working within FHTs, NPLCs, and other interprofessional models of primary care.

    Given the composition of the continuing board members, priority for the directors to be elected will be given to candidates who are:

    • Interprofessional health care providers (IHPs), especially those working in mental health and addiction;
    • Board members, especially community members in the position of Chair of the Board; or
    • Experienced in finance, including accreditation as a financial professional (e.g. CA, CPA, CMA).

    To apply:

    The governance committee will review all applications to assist the board in determining the slate of candidates to recommend to the AFHTO membership for ratification at the annual general meeting.

    • Nominees will be informed of their status by September 23, 2020.
    • The slate will be presented to the members at the annual general meeting, which is scheduled for:

    Thursday, October 8, 2020

    On June 17 AFHTO presented its submission to the Standing Committee on the Legislative Assembly on Bill 175: Connecting People to Home and Community Care Act, 2020. Our key recommendations are:

    1. Delay the passage of Bill 175, Connecting People to Home and Community Care Act, 2020
    2. Embed care coordination in primary care
    3. Supporting the patient’s journey through the health and social system with one patient record

    COVID-19 outbreak has likely peaked in Ontario

    Yesterday the Ontario government released updated COVID-19 modelling, which shows that the enhanced public health measures are working to contain the spread of the virus and flatten the curve. However, the Chief Medical Officer of Health says emergency measures must remain in place to continue reducing the number of cases and deaths.

    A reminder that the Ministry has created a new website (https://covid-19.ontario.ca/) that includes the link to check your lab results if you have been tested for COVID-19, allows you to take the self-assessment and includes up to date number of COVID-19 cases, that is updated daily at 10:30 a.m.


    Open letter to Minister PPE Mask and gloves April 2020

    Open letter on PPE in primary care during COVID-19

    On Wednesday, April 15 an open letter regarding the lack of PPE in Primary Care during COVID-19 was released from the Association of Family Health Teams of Ontario (AFHTO), Nurse Practitioners’ Association of Ontario (NPAO) and the Ontario College of Family Physicians (OCFP). On behalf of our members we are seeking urgent assistance from Ontario’s Ministry of Health in ensuring access to appropriate PPE in order to safeguard patient access to primary care.

    We are recommending that the Ontario Ministry of Health immediately streamline the procurement of PPE through one central source for distribution to primary care through Ontario Health/regions and not leave primary care clinics to have to find PPE on their own. Read the full letter here.

    We are happy to hear that discussions related to the lack of PPE are continuing at the newly created Provincial Primary Care Advisory Table and at the Ontario Health regional tables.

    Virtual annual general meetings

    Virtual collaboration is being considered and used more than ever considering the pandemic. On March 31, 2020, the Government of Ontario issued an Emergency Order that permits nonprofits to hold meetings of directors, shareholders, and members electronically or over the phone, even if there is legislation or bylaws (ex: Ontario’s Corporations Act) that say otherwise.

    Boards may consider this option for their Annual General Meetings (AGM). The Ontario Non-profit Network (ONN) has provided some tools to help boards move AGMs online and to facilitate them effectively and efficiently. Get resources here.


    Leadership in a crisis- one-page guide

    The coronavirus has caused LEADS Canada to shift immediately and dramatically to offering support to health leaders across the country in leading through (and following) the pandemic.
     
    In response, they developed a one-page guide called “Leadership in a Crisis”, which is based on recent research and input from a cadre of health and military experts (listed at the bottom of the document).

    There have been some new tools/resources and updates we wanted to share with you that you may find helpful:

    • AFHTO’s Business Continuity Toolkit – we have developed this toolkit to help our teams plan for and adapt to the disruption of COVID-19 and it potentially can be used for future pandemic disruptions. Within the document you will find a number of tools and resources to help develop a plan for your own organization. A reminder to check the AFHTO website for templates your colleagues have developed if you are looking for tools you can adapt. Please also send us any materials you are developing so we can share with your colleagues as well.
    • FHT/NPLC Governance During COVID-19 – Frequently Asked Questions developed by Kathy O’Brien of DDO Health Law. We know many of you are starting to think about year end and how to govern during a pandemic emergency, including how to conduct things like virtual annual general meetings. Kathy has provided some answers to the most pressing concerns and we will continue to provide more information as we receive it.
    • Update #1: Health System COVID-19 Pandemic Response – letter from Matt Anderson, CEO of Ontario Health, which will be sent regularly to provide updates on the work of the pandemic response teams across the province. AFHTO now sits at the weekly COVID-19 Stakeholder Table which now provides an opportunity to articulate the ongoing concerns and challenges being faced by primary care to the Ministry and Ontario Health directly.  
    • The release of two Guidance documents from the Ministry’s Emergency Operations Centre including the Provincial Testing Guidance Update which speaks to increased laboratory testing capacity and Cyber Security Advisory for Zoom conferencing software.
    • Just a reminder that we have a COVID-19 section on the website that hosts tools and resources you may need, including policies and procedures from your colleagues that you may find helpful in adapting for your organization, as well as the daily Ministry Situation Reports. We have also included a new tab Providing Comprehensive Care During COVID-19 which includes resources related to COVID-19 for providing comprehensive primary care – this will be updated as resources become available.

    Every year we say the same thing – what a challenging and busy year it’s been! But for this year in particular truer words have never been spoken! 2019 was an exceptionally busy year,and with ongoing health system transformation it has really tested the resiliency of our teams as many took on more leadership within their communities to support local level partnerships and collaboration.

    As we close out the year, we wanted to provide you with an operational update wrap-up on some of the activities we are working on your behalf.

     


    OHT Development

    Now that the first cohort of 24 Ontario Health Teams has been announced, we understand that the real work in implementation begins if we are to realize the true potential of this initiative. But as fairly lean organizations, we also understand that capacity remains an ongoing challenge – there are only so many hours in a day!

    Capacity and resources (both human resources and infrastructure support), importance in valuing and supporting clinical leadership, and ensuring there is a move towards true population health with primary care as the foundation is part of our ongoing discussions with the Branch. As a reminder, we have ongoing monthly meetings with the OHT Branch to express any questions or concerns that you may have so please let us know if you do have any at any time so we can let them know.

    Expansion of Team-Based Care, More Mental Health and Addictions Support and Care Coordination

    Primary care teams provide value for health dollars by speeding up access to care and offering a wider range of programs and services to promote health and manage chronic disease. They bring together the variety of skills needed to help people stay as healthy as possible. This is what is going to help with reducing hallway healthcare.  

    AFHTO continues to advocate for more supports in primary care, especially as it pertains to ensuring more patients have access to team-based care, in particular in the areas of mental health and care coordination. In the new year, we hope to see a roll out of a comprehensive structured psychotherapy program which will provide much needed mental health supports to more Ontarians and we continue to advocate to ensure that the care coordination resources that currently reside in the LHINs are embedded in primary care.

    We have so many tools and resources available for members, but since they have been announced throughout the years, we thought a refresher would be welcome, especially since we know you’re exposed to so much information every day.   • Mississauga OHT Announced

    On Nov. 25, 2019, Christine Elliott, Deputy Premier and Minister of Health, was at the University of Toronto Mississauga to announce the Mississauga Ontario Health Team, known as Mississauga Health, as one of the first 24 teams in the province. The aim is to implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.

    Mississauga Health reinforces existing partnerships while engaging new partners, focusing on care through the continuum from birth to palliative care, and ensuring seamless transitions.

    AFHTO members in Mississauga Health are Credit Valley FHT, CarePoint Health and Summerville FHT. We look forward to working with the OHT to ensure primary care remains foundational in the transformation.

    Once they created their strategic plan, Chatham-Kent FHT’s board formed an implementation task force. CMCS Consulting, who had facilitated their plan, followed up and profiled the task force in their newsletter. Find out how the task force performed here.

    HQO seeking volunteers to help develop insomnia disorder quality standard
    Health Quality Ontario is seeking volunteers interested in joining an Advisory Committee that will provide advice to support the development of a quality standard focused on people living with insomnia disorder. Apply here. Deadline Nov. 8, 2019.


    CPSO seeking input on stakeholder engagement

    The College of Physicians and Surgeons (CPSO) is currently planning for the implementation of its newly developed Strategic Plan, including the development of a Stakeholder Engagement Framework. The framework’s purpose is to support CPSO in regularly engaging with numerous stakeholders in a way that is meaningful to them and informative to CPSO’s work. This plan will build on the work CPSO is already doing to engage stakeholders, including the public/patients, system partners and physicians.

    Optimus SBR is supporting the framework’s development and is hoping to speak with a few physician leaders who can share their perspectives on the best forums and methods for engagement with their peers.
