  • FHT Contract Recommendations

    Team-based primary care must be the foundation of an integrated care system, and the FHT contract can provide the best direction for strong primary care teams that are the fundamental base of a strong healthcare system.

    In 2021, AFHTO began member and stakeholder consultations to develop recommendations for the next FHT contract. The current one is scheduled to expire on March 31, 2023.

    AFHTO began work with McMurchy Consulting and formed a contract working group of eight team executive directors in 2021. These EDs brought a strong variety of voices to the table. After extensive internal and stakeholder consultations, discussion groups were established in 2022 for what were determined as the three major areas of consultation: governance, accountability and performance, and FHT/physician affiliation. 

    Through this work, we developed our recommendations for modernizing the FHT contract, Schedule A, the governance attestation, and the annual operating plan. This was submitted to the ministry in November 2022. We will keep members apprised of progress on contract reform.

    Thank you to everyone who participated in AFHTO’s FHT contract work – our initial working group, the subsequent discussion groups, the AFHTO Leadership Council, physicians who joined physician consultations, and many stakeholders. Your advice and suggestions were critical for us to develop a thoughtful set of recommendations for the ministry.

  • Join our ED Mentorship Program!

    Are you a leader in your team with 3+ years of experience? Are you interested in supporting new leaders in primary care teams? We are looking for some amazing executive directors and admin leads to join our ED Mentorship Program as mentors! 

    Mentoring is about supporting people to develop more effectively. It is a relationship based on trust that is designed to help a mentee take on increasing initiative, build confidence, and excel as a leader. New EDs can learn from others who have experience and who are willing to help them in developing their skills and expanding their knowledge.

    AFHTO is looking for volunteer mentors for the ED Mentorship Program from different demographics and funding models. We recognize how busy all EDs are as we navigate uncertain times, but we hope you will consider this small investment of your time for a positive and rewarding experience.

  • Physician Orientation Toolkit

    Shared by AFHTO & the Osborne Group

    This toolkit provides easy access to a comprehensive collection of tools, resources and templates that will help orient new physicians to the FHT. Developed in partnership with The Osborne Group and a small working group comprised of AFHTO member EDs and physician volunteers. The Toolkit can be used:

    • As an orientation guide for physicians who are new to FHTs or FHT leadership
    • As a support for current physician leaders
    • As an educational tool to help explain the role of a FHT physician or lead physician
    • As a resource for FHT boards as they orient and support the lead physician role

    1. Family Health Teams: This section describes FHT structure and governance;  the roles of the executive director and lead physician; funding and relationships to the Ministry of Health and Long-Term Care and the Local Health Integration Network (LHIN); and measurement and quality improvement.
    2.  Our Family Health Team: This section can be customized to describe your team’s vision, mission and values; strategic plan; quality improvement plan; organizational structure; patient population; physician/FHT relationship and roles; and community partnerships.

  • Environmental Scan and Strategy Framework for Primary Care

    Dear board chairs, executive directors, and admin leads,
     
    We have developed a succinct 2-page Environmental Scan and Strategy Framework for Primary Care to support teams with generative discussions and strategy development. This has been done using a common strategy framework known as PEST, which groups factors that are affecting primary care into four categories.
     
    PEST stands for Political, Economic, Socio-cultural, and Technological.
     
    Broken into these four categories, the scan lays out current issues in Ontario that boards should keep in mind when doing strategic planning.
     
    This is a standard support tool for boards of primary care organizations, and we encourage teams to use it to help develop or update strategic priorities. Also in this document are 10 questions that we encourage boards to ask during their strategy development.

  • Join AFHTO’s IHP Advisory Council – Applications are now open!

    Are you interested in advising AFHTO of the top issues facing IHPs in team-based primary care? Would you like to be a leader in working with IHPs to advocate for and to speak to the critical role of teams, as well as recruitment and retention of providers in primary care? Would you like to share and spread knowledge and learnings?

    If yes, AFHTO invites you to apply to join the IHP Advisory Council! The council was established in 2018, and several two-year terms are now open. 

    This council provides input on IHP-related matters, gives advice and input to the AFHTO board and staff on AFHTO and primary care priorities, and fosters leadership across the IHP community in teams. Meetings are quarterly, and AFHTO occasionally reaches out for input, advice, and guidance between meetings.

    The council is a mechanism to advance best-practice knowledge transfer and exchange, to enhance collaboration, and to give IHPs the opportunity to communicate operational and other team-related items. 

    AFHTO seeks a diversity of professions and regional representation on the council. However, the primary role of every council member is to represent and be a voice and a leader for all IHPs who work in teams.

  • Help shape final recommendations for the next FHT contract! Sign up for a discussion group

    This email was sent to EDs/Admin Leads, lead clinicians and board chairs of AFHTO member teams.

    Dear triad members,
     
    Since last fall, AFHTO has been working with members and consultant Dale McMurchy to develop recommendations for the next FHT contract. The current contract expires in March 2023, and AFHTO will be providing recommended changes to the Ministry of Health this fall on behalf of our membership. Our input can influence the next stage in the evolution of interprofessional primary care.
     
    As part of the process, we have consulted with our FHT Contract Working Group (seven EDs in various models) since the beginning. We’ve held multiple consultations where we heard from well over 100 members, and we presented draft recommendations to our board in March. We have also received feedback from the ministry and kept the OMA involved in the conversations.
     
    Next, we’d like you to consider joining one of the three working groups we’re forming based on the top areas for advancement that have been identified and where further discussion is needed:
     
    Accountability and Performance Discussion Group

    • The goal is to work towards aligning current reporting and performance measures, standardizing expectations related to performance and accountability, and to have initial discussions towards laying the groundwork for developing a provincial primary care performance framework.
    • Key considerations will be the nature and timing of reporting, applicability, and level of evidence of performance measures, as well as the feasibility, effort, and resources needed.

     
    FHT/Physician Affiliation Discussion Group

    • The goal is to clarify and strengthen FHT/physician partnerships (with affiliated and possibly unaffiliated physicians) by exploring requirements for a shared agreement/MOU.
    • Key considerations for the agreement/MOU template content will include guiding principles and goals; expectations related to roles, responsibilities and outcomes; team collaboration and shared care; and use of resources.

     
    Governance Discussion Group

    • The goal is to revisit the current governance models from a lens of ensuring the boards have a diversity of skills, experience and backgrounds representative of the community served, and are responsive to health equity, community/patient needs, and patient experience.
    • Key considerations will include the minimum standards for the Governance and Compliance attestation; how Boards are held accountable for performing their fiduciary, strategic, and generative duties; the requirement for board education and training; and any additional considerations for blended salary and academic models.

     
    We would like each working group to have 8-10 members, with a mix of EDs, physicians, and board members. We also hope to include 1-2 QIDSS on the Accountability and Performance Discussion Group.
     
    These groups will review the materials developed to date and help us finalise our recommendations to the ministry. We anticipate 3 to 4 meetings for each discussion group between June and September, with the first meeting planned for early June. Timing and frequency will be determined during the first meeting, and we will work to minimize the burden.
     
    We would like to emphasise that the recommendations we develop will be submitted to and discussed with the ministry, who will make the final decisions.

    Reminder- seeking pharmacists’ input on experiences and contributions to mental health care during COVID-19

    If you are a primary care pharmacist, University of Toronto researchers want to hear from you! Their study seeks to examine primary care team pharmacist contributions to patient mental health care, and understand pharmacists’ experiences with collaboration during the COVID-19 pandemic. The 15 to 20 min survey includes a mix of multiple choice and open response questions.
     
    This study from the University of Toronto is co-led by Dr. Rachelle Ashcroft, Assistant Professor at the Factor-Inwentash Faculty of Social Work, and Dr. Lisa Dolovich, Professor & Dean at the Leslie Dan Faculty of Pharmacy.

    • Board members, especially community members in the position of Chair of the Board; or
    • Experienced in finance, including accreditation as a financial professional (e.g. CA, CPA, CMA).

    COVID-19 outbreak has likely peaked in Ontario

    Yesterday the Ontario government released updated COVID-19 modelling, which shows that the enhanced public health measures are working to contain the spread of the virus and flatten the curve. However, the Chief Medical Officer of Health says emergency measures must remain in place to continue reducing the number of cases and deaths.

    A reminder that the Ministry has created a new website (https://covid-19.ontario.ca/) that includes the link to check your lab results if you have been tested for COVID-19, allows you to take the self-assessment and includes up to date number of COVID-19 cases, that is updated daily at 10:30 a.m.


    Open letter to Minister PPE Mask and gloves April 2020

    Open letter on PPE in primary care during COVID-19

    On Wednesday, April 15 an open letter regarding the lack of PPE in Primary Care during COVID-19 was released from the Association of Family Health Teams of Ontario (AFHTO), Nurse Practitioners’ Association of Ontario (NPAO) and the Ontario College of Family Physicians (OCFP). On behalf of our members we are seeking urgent assistance from Ontario’s Ministry of Health in ensuring access to appropriate PPE in order to safeguard patient access to primary care.

    We are recommending that the Ontario Ministry of Health immediately streamline the procurement of PPE through one central source for distribution to primary care through Ontario Health/regions and not leave primary care clinics to have to find PPE on their own. Read the full letter here.

    We are happy to hear that discussions related to the lack of PPE are continuing at the newly created Provincial Primary Care Advisory Table and at the Ontario Health regional tables.

    Virtual annual general meetings

    Virtual collaboration is being considered and used more than ever considering the pandemic. On March 31, 2020, the Government of Ontario issued an Emergency Order that permits nonprofits to hold meetings of directors, shareholders, and members electronically or over the phone, even if there is legislation or bylaws (ex: Ontario’s Corporations Act) that say otherwise.

    Boards may consider this option for their Annual General Meetings (AGM). The Ontario Non-profit Network (ONN) has provided some tools to help boards move AGMs online and to facilitate them effectively and efficiently. Get resources here.


    Leadership in a crisis- one-page guide

    The coronavirus has caused LEADS Canada to shift immediately and dramatically to offering support to health leaders across the country in leading through (and following) the pandemic.
     
    In response, they developed a one-page guide called “Leadership in a Crisis”, which is based on recent research and input from a cadre of health and military experts (listed at the bottom of the document).
