    Welcome to Connor Kemp

    Connor is wearing a light polka dotted shirt with brown tie, belt and grey pants. he has his hands in his pockets as he leans forward a little smiling.

    We’re happy to introduce Connor Kemp, our new Project Manager and KTE Specialist.

    Connor began his professional career in the hospital sector at PRHC in decision support. His desire to work in primary care landed him in a QIDSS position in the Kingston area. He has been engaged in several primary care research initiatives, including a clinic efficiency and burnout pilot project.

    Connor’s interest in primary care research led him to pursue an adjunct position with Queen’s University, where he has spent the past year supervising students in a primary care placement. He is also collaborating with colleagues to design experiments for testing biometric wearables to aid the decision-making process for primary care providers. He wishes to expand this data-driven decision-making research in PC. Connor has a background in mechanical/materials engineering and spent much of his graduate time at McGill developing automated screening processes for biomaterials.

    Compensation market survey walkthrough video

    On Friday, April 14, the Alliance hosted the Ontario Community Health Market Review walkthrough. The video is now available. The survey package itself was sent to EDs, Admin Leads and designated representatives of AFHTO member teams on April 4. The deadline to submit the survey to Eckler has been revised to April 27 to allow closer to two weeks after the walkthrough for survey completion. If you haven’t received the email, please contact us. For inquiries regarding the survey itself, please contact Tiffney Hill at thill@eckler.ca.

    Provincial Webcast: Governing Through Complexity
    As a result of COVID-19, the implementation of Ontario Health Teams, HHR challenges, capacity and access constraints, and the shift to a population based approach, the pace of change has been quick and for many governors of primary health care teams the question remains: “What is the role of our board in this changing environment? How do we strengthen our governance practices? Where should we focus our efforts?”? This provincial webcast will help explore these questions and equip board members with the necessary knowledge to lead effectively and implement high functioning governance practices.

    Many of you are familiar with David Hartley’s particular expertise in primary care governance, so we are excited to share AFHTO’s partnership with him! David will offer his ‘Best Practice FHT Board Governance Training’ to teams on a 50/50 cost shared arrangement with AFHTO. This is $1350 in savings for teams! Up to 20 teams will receive Training in the form of either: 1) Virtual Option: 2 X 90 minute virtual training sessions (the sessions can be spread out 2 months). Cost per team = $1350; 2) In-Person Option: 3 hours at a location of your choosing. Cost per team = $1850

    AFHTO/Charity Village Partnership
    We are pleased to announce a continued partnership with Charity Village to offer our teams a range of free governance courses. Over 100 members across 25 teams took advantage of these sessions last year with overwhelmingly positive feedback. We have added to the course offerings this year to ensure a broad range of topics to choose from.  

    Each course is chock full of solid information and practical resources that you can download and keep on hand for future reference. Courses are offered virtually in a self paced format. You have 3 months to complete the course once started. Below is a list of courses teams may choose from.

    1. Better Board Kit
    This is a three-part series which includes a 1.5 hr online, on-demand Boards that Work course + a Board Self Evaluation Survey + a Facilitation Guide that will walk your board through a planning session based on the survey results. The Boards that Work kit is available for up to 10 board members.

    Setting the Cornerstone: A Blueprint for the Implementation of PCNs in Ontario Discussion document

    And finally, in our efforts to strengthen overall governance practices and models by enabling horizontal integration of the primary care sector, we are pleased to share with you the concept discussion paper developed specifically for the Ministry of Health that outlines preliminary recommendations for the establishment of Primary Care Networks (or PCNs) in Ontario. Developed jointly with AFHTO/OCFP/SGFP, the Blueprint provides an important opportunity to strengthening our province’s primary care system.

    We respectfully acknowledge that further consultation with our members and key stakeholders representative of the sector is required to refine the recommendations being presented. We hope that by sharing the document it will be considered a starting point for discussions between the Ministry of Health, Ontario Health and representative of the primary care sector. With any changes it is critical that we work in co-design and all voices are heard. We will keep all members updated as conversations unfold.

  • Winter Highlights 2023: AFHTO Leadership Council

    Fernando Tavares, Program Manager, Primary Health Care Branch, OHIP, Pharmaceuticals and Devices Division, MOH joined the AFHTO Leadership Council on February 17. Below are highlights from that meeting and updates on work that is underway.

    Q & A with Fernando Tavares
    Fernando Tavares joined the Leadership Council meeting for an hour-long Q&A session to discuss various topics, including provincial factors that are shaping and influencing the direction of team-based care in Ontario. Questions focused on the shift to a population-based approach to care, the future state of OHTs, relations between MOH and OH, HHR challenges, and organizing primary care.     We are also pleased to announce that Fernando will join us for another Q&A session to provide a brief update on the Annual Operating Plan (submission timelines + process), discuss further details on the FHT contract extension including updates to the Governance & Attestation document, and review details of the “Your Health: A Plan for Connected and Convenient Care” investment of $30 million to help bridge the gap in accessing interprofessional primary care.  All EDs are invited to attend the session on March 28th @ 12:00pm.     Preliminary Survey to Capture MHA Funding Impact
    Further to our memo released on January 30th,the IPHCC has developed a preliminary survey to assess the progress and utilization of the MHA funding. Recognizing that teams have until March 31st to spend these funds, the intent of this survey is to capture the work you have underway and identify areas where we can provide further guidance, if necessary. The survey should take 5-7 minutes to complete. Please note this is not a replacement for the fulsome evaluation that the IHPCC will send to you in April. Read the full message here.

    Market Salary Review & Compensation Working Group
    In 2022, multiple associations and organizations came together as the Community Compensation Working Group to jointly support a market survey in order to get a better idea of the wage disparity and compensation challenges across interprofessional primary care, community support services, community mental health and long-term care. The market salary review will be conducted for 78 positions across the primary and community care sectors. The Working Group has since partnered with Eckler Ltd. to begin this work with an estimated completion by end of spring 2023. Once complete, recommendations will be presented to the government and funders on reasonable, fair, and equitable compensation across the healthcare sector. Our goal is to demonstrate the significant pay gap and wage inequities across the community sector and to compel government to pay healthcare providers and staff the compensation they deserve.

    To ensure a successful market study with a substantive sample, Executive Directors are being asked to complete a survey that will accompany the market review. Surveys are targeted to be launched in late February 2023. If you have questions about this market study, please feel free to contact, Abisola Otepola at abisola.otepola@afhto.ca.

    IHP roles and responsibilities
    IHP members requested updated information on roles and responsibilities of IHPs to aid in the coordination and management of their respective teams. AFHTO is currently validating information that had been provided and is planning to release the information over the coming months.  Following the release of the guidance document, plans are in place to host a webinar series with a focus on individual IHP disciplines/roles. The goal is to have generative discussion around individual roles and responsibilities, challenges, and key enablers using the guidance document as the starting point. This work will be done in partnership with associations and key stakeholders. Preliminary timelines for this will be summer 2023.

    A Plan for Connected and Convenient Care – what we know to date
    On Feb 2nd 2023,  Health Minister Sylvia Jones launched Your Health: A Plan for Connected and Convenient Care, a plan that sets the vision and direction for Ontario’s health system strategy for the next few years. We were thrilled to see an investment of $30 million to expand or create new teams and help bridge the gap in accessing interprofessional primary care for vulnerable, marginalized, and unattached patients to ensure they can connect to care where and when they need it.

    AFHTO has been advocating for years around expansion of team-based care and it’s great to see the first large investment to expand teams in the province in a number of years. The details of what the implementation will look like (new teams or expansion of already existing teams) is still being worked through and we hope to provide you with more information as we continue to dialogue with the Ministry and Ontario Health.     PCN Primer
    Within the Your Health: A Plan for Connected and Convenient Care we were also pleased to see the commitment to help support organizing primary care by:

    Creating a connected health care system through Ontario Health Teams by supporting collaboration and engagement with primary care providers across the province through the creation of primary care networks. Every Ontario Health Team will include a group of primary care providers organized in a network to be part of decision-making and to improve access to care for patients.

    To support the notion of primary organization, AFHTO along with our SGFP and OCFP partners have developed a concept discussion paper for the Ministry that outlines preliminary recommendations for the establishment of Primary Care Networks (or PCNs) in Ontario. A two-page primer summarizing the key concepts of PCNs can be seen here. We look forward to further consultations with our members and stakeholders over the coming months to refine the draft recommendations presented.

    $8.1M Funding Announcement for Expansion of Teams
    The provincial government is providing $8.1 million in new annualized base funding to enhance access to primary care in key regions of the province. “This funding will support various interprofessional primary care organizations, including 18 Family Health Teams (FHTs), 2 Community Health Centres (CHCs) and 2 Nurse Practitioner Led-Clinics (NPLCs), to continue the delivery of high-quality care that Ontarians know and deserve.” If your team is receiving new base funding and you’re able to share details, please contact us so we can share.

    On February 6, 2023 AFHTO submitted our recommendations to the government for their 2023 budget. Our recommendations are broken into four sections:

    1. Make team-based primary care available to more Ontarians through an investment of an additional $75 million pre year for 10 years (recommendation provided in partnership with the Primary Care Collaborative)
    2. Address the health human resource (HHR) crisis in primary care through the creation of a primary care HHR table (recommendation provided in partnership with the Primary Care Collaborative)
    3. Provide mental health and additions services and home and community care in coordination with primary care
    4. Provide resources to support the implementation of primary care networks (PCNs) and recognize PCNs as the organizing model for primary care. 

     
    AFHTO has begun to set up meetings with government bodies to present our recommendations and hopes to begin meeting with officials very soon. Members are encouraged to present the pre-budget submission to their own MPPs if given an opportunity to meet. Please let us know if we can help in your local advocacy efforts!

    We are pleased to launch the first AFHTO IHP Advisory Council Communique. The Council meets on a quarterly basis to provide input on primary care team matters related to IHPs, give advice to the AFHTO board and staff, and foster leadership across the IHP community.     FHT and NPLC Contracts
    In late January teams received letters from the Ministry of Health with regards to the NPLC and FHT contracts for the upcoming fiscal year. The Ministry intends to transfer the contract management and oversight of the NPLCs to Ontario Health, effective April 1, 2023. All provisions in the NPLC Agreement shall remain in full force and effect.

    A separate letter went to FHTs noting the FHT contract has been extended for another year. The FHT contract now expires on March 31, 2024. Although there should be no direct impact to IHPs, we understand there may be questions on the NPLC transition process and what this might mean for the potential of a future transition of the FHT contracts to OH. We’re speaking regularly with MOH and OH and will provide regular updates to our members.

    Connect with your peers using AFHTO’s IHP Directory
    IHPs in AFHTO member teams are encouraged to sign up to AFHTO’s IHP Directory! This directory will help improve AFHTO’s communication to IHPs, and it will support IHP networking, information sharing, and collaboration.

    After you sign up, you will be emailed a link to a spreadsheet with the names, professions, team names, and work emails of all IHPs who have signed up for open communication. You may sort by profession to see the names of those in the same discipline as you.

    AFHTO will be checking the directory and emailing links to new registrants every 1-2 weeks. We look forward to improved communication from AFHTO to IHPs, and across IHPs in AFHTO-member teams!

    Collaboration in team-based primary care during the COVID-19 pandemic: study

    Now published in BMJ Open, “A qualitative study of collaboration in interprofessional primary care teams during the COVID-19 pandemic” is our study with Drs. Rachelle Ashcroft, Catherine Donnelly and others. “Results revealed the importance of collaboration for provider well-being, and the challenges of providing collaborative team-based primary care in the pandemic context. Caution against converting primary care collaboration to predominantly virtual modalities postpandemic is recommended.” Read more here.

    Coming soon: Updated High Risk OBSP genetic assessment referral criteria and forms

    Ontario Health wishes to inform everyone of some upcoming changes to the High Risk Ontario Breast Screening Program (OBSP). The High Risk OBSP was launched in 2011 to support organized screening of people at high risk for breast cancer. The program now screens approximately 13,000 participants each year with mammography and screening breast magnetic resonance imaging (MRI) (or screening breast ultrasound if breast MRI is not medically appropriate).     Seeking participants for an online survey about improving osteoarthritis care

    With funding from the Arthritis Society, Dr. Anna Gagliardi and University Health Network (UHN) researchers interviewed 27 women with osteoarthritis (OA) and 31 healthcare professionals to identify strategies needed to improve OA care for diverse women, and possibly other disadvantaged groups. They are now seeking women with OA, family physicians, and healthcare researchers to participate in a two-round online survey. They will ask you to complete the Round One online survey (about 20 minutes). A few weeks later, they will send you anonymous results, and ask you to complete the Round Two survey (about 10 minutes).

    ELIGIBLE WOMEN:
    — Have suspected or confirmed osteoarthritis
    — Are aged 40 or older
    — Were born in or have lived in Canada for 10 years or more
    — Can speak and understand English language
    — Represent the following ethno-cultural group(s): Chinese, Filipino, Indian, Pakistani, African, Caribbean

    Seeking patients for early detection of Mild Cognitive Impairment and Alzheimer’s disease

    The Alzheimer Society of Toronto (AST), Toronto Memory Program, Retispec and Summerhill Optometry are conducting a study, funded by the Davos Alzheimer’s Collaborative. The aim is to enable early detection of Mild Cognitive Impairment and Alzheimer’s disease through increased awareness and assessment in accessible community-based settings. While they have 3 assessment locations in Toronto, they are also open to travel to your location if there is significant interest. Cognitive Assessments are delivered by trained social workers. The completed assessments will be provided to the patient’s primary care provider for interpretation and follow-up. AST will provide services to those diagnosed with mild cognitive impairment or dementia.
    The study is open to individuals 65 years and over who are experiencing memory problems, but do not have a diagnosis of Mild Cognitive Impairment or Dementia.

    Inclusion and Exclusion Criteria:

     Inclusion - Adults (≥ 65 years of age); Able and willing to provide informed consent; Have a memory complaint. Exclusion- Prior diagnosis of dementia or Alzheimer’s disease or MCI; Exclusion Criteria for RetiSpec scan Have contraindications to the use of mydriatic (dilation) drops (for RetiSpec imaging procedure only)

  • Request to Participate in the 2023 Compensation Market Salary Review – Heads Up

    Dear Executive Directors and Admin Leads,

    As you may be aware, in 2022, multiple associations/organizations came together as the Community Compensation Working Group to jointly support a market survey to get a better idea of the wage disparity and compensation challenges. To ensure a successful market study with a substantive sample, we require your collaboration and participation to complete the survey that will accompany this review.

    Why it matters:
    This survey is particularly important considering the impacts of the pandemic on healthcare as well as human resources practices within organizations, and legislated compensation restraints that have impacted many organizations in Ontario. We know that recruitment and retention, as well as salary disparity, is one of the biggest challenges in primary care and getting data on what the levels currently are and what they should be is critical in supporting our HHR advocacy asks to government.

    Who and what will be covered:
    The Community Compensation Working Group includes over 1,200 unique organizations including: 302 primary care organizations comprised of Community Health Centres, Family Health Teams, Nurse Practitioner-Led Clinics, Indigenous Primary Health Care Organizations; 225 home and community care organizations, 90 publicly funded child and youth mental health agencies, 175 community mental health and addiction organizations, 30 local CMHA branches, 221 long-term care homes, 91 housing members and 95 community service agencies. Invitations are being sent to members served by these associations/organizations as well as several other organizations – Ontario and nationally.

    We have partnered with Eckler Ltd. to conduct this survey. They will provide independent evidence-based approaches to reviewing the compensation and provide insights. Eckler Ltd. will receive the surveys collected by AFHTO and others, streamline information (ensuring maintaining confidentiality and privacy for each individual organization), and provide aggregated information in reports. All survey participants will receive a summary report which we hope will assist you in compensation planning.

    The survey is focused on collecting basic salary and information for a series of benchmark positions with a few other general HR practice questions for review and consideration.

    We know you likely receive a number of requests for survey participation and are facing significant resource constraints; however, we hope our request will support any pre-planning as we prepare for a survey launch in February.

    Supporting Caregivers webinar materials available
    On Nov. 24 The Ontario Caregiver Organization and Primary Care Collaborative hosted a webinar ‘Supporting Caregivers Supports Patients- The Critical Role of Primary Care in Preventing Caregiver Burnout’. This included insights from a caregiver with lived experience, as well as examples of primary care initiatives across Ontario that are connecting caregivers to support.     On November 30 we held a webinar on the Ontario COVID-19 Science Advisory Table’s final brief, a 3-part exploration of how primary care responded to and was affected by the pandemic, and implications for ongoing health system planning and policy.   • Year-End Update to Executive Directors/Administrative Leads – December 2022

    Dear Executive Directors/Administrative Leads,

    With December now here, we would like to share updates on a few issues that are front of mind for many. We also met with the Primary Health Care Branch on December 1, and they provided some insight that we would like to share.

    Bill 124
    Bill 124, Protecting a Sustainable Public Sector for Future Generations Act, 2019 was declared “to be void and of no effect” by Justice Koehnen of the Ontario Superior Court on November 29. The court declared Bill 124 was unconstitutional and a violation of the Canadian Charter of Rights and Freedoms.

    AFHTO, along with our health system partners, have been advocating to government to repeal this law since it was proclaimed in 2019, so we welcome this news. We hope it will start the pathway toward fair and equitable compensation for healthcare workers.

    The government plans to appeal the decision, but they have stated that they will not use the notwithstanding clause, so this appeal could take years to settle.

    What does this mean for primary care teams?
    Maria McDonald of McDonald HR Law sent a communique on November 30 that noted:
    As of November 29, 2022, Bill 124 is not an enforceable law. It is unknown what the government will do but an appeal could take years. It is highly unlikely government will make any revised law retroactive. As such, right now, Bill 124 (which does not exist because it is void and unenforceable) does not prevent granting bonuses.

    However, it’s important to note that there are clauses in your agreement that will direct how you can interpret this for your team. For FHTs, this is referenced in Clause 7.3:
    “All Funding shall be applied directly to the payment of Schedule “B” expenditures, and for no other purpose. The Recipient shall expend the Funding in each Funding Year only in accordance with the Annual Budget for each Funding Year. No changes to the approved items in Schedule “B” are permitted without the prior written consent of the Ministry, unless otherwise specified in this Agreement.”

    For NPLCs, refer to Clause 3.03:
    “All funding shall be applied directly to the payment of Schedule “A” and “A-Appendix 1” expenditures, and for no other purpose. No changes to any Schedules are permitted without the prior written consent of the Ministry as set out in section 2.09. Continued Funding under this Agreement is subject to Ministry review and approval of Approved Annual Operating Budgets and Banking and Payment Schedules and Service Requirements.”

    All other models, please reference your agreements for relevant clauses.

    In 2019, the Ministry provided some guidance on how best to interpret increases with staff. This guidance still applies:

    • If you increase salaries, ensure you have sustainable funding to maintain the increase as the Ministry will not be providing additional funding.
    • You cannot eliminate a position to accommodate for increases. During reconciliation, your budget will be ‘right-sized’ if you no longer need that position, and the funding will be taken back.
    • If performance-based merit pay is part of the organization’s compensation plan and fits within the HR budget, this would be allowable.
    • Vacant positions or underfilled FTE positions cannot be used to fund additional compensation for other positions. Funding needs to be allocated for all funded positions within the framework of the board’s compensation plan and policies.

    As we learn more about what is now allowable with the repealing of the Bill, we’ll share this information with you.

    A reminder that we are working on a compensation market refresh with our partners in interprofessional primary care, mental health and addiction, community care, and not-for-profit long-term care: Addiction and Mental Health Ontario, AdvantAge Ontario, Alliance for Healthier Communities, Canadian Mental Health Association (Ontario), Children’s Mental Health Ontario, Family Service Ontario, Indigenous Primary Health Care Council, Nurse Practitioner-Led Clinic Association, and Ontario Community Support Association.

    This work will be the grounding for advocacy for sustainable increases in salaries. We’ll share more once the work is underway with our recently approved vendor.

    Mental Health and Addiction Funding

    Funding packages for the MHA one-time funding are still in approvals. Funding is expected to flow in January, and it will cover expenses from October 1, 2022, to March 31, 2023.
    We know that this isn’t enough time to spend the funds. Planning is very important, so we encourage you consider the following given the tight time frame you will be working in:

    • Work with your fellow primary care teams to discuss pooling the funding to purchase MHA supports for teams, OHT, and/or unaffiliated physicians. We encourage you also work with other mental health organizations and providers, such as CMHA, AMHO organizations, and private psychologists regarding pooling or transferring funding.
    • Increase the FTE of your MHA staff or hire temporary staff to support the backlog of MHA in your team or in your community.

    We appreciate this is difficult without knowing the funding amount you’ll be receiving. However, we hope you can get a general plan in place with or without your partners as you await confirmation of the amount and flow of the funding.

    We’ll be in touch next year to ask about how funding was used as this will help the Ministry build a case for sustainable funding to embed much-needed MHA supports in primary care.

    Approval Process for Extra Costs Incurred During this Respiratory Season
    The Ministry memo from November 21 is still front of mind for many teams. A reminder that the memo, though not well communicated, was about communications to your patients and doing your best to support this respiratory illness surge.

    If your team is planning to increase capacity, we encourage you do your projections to year-end and assess your existing funding. If you still feel unable to fund the increased work you wish to do, please reach out to your program consultant to demonstrate this, and then begin discussion for funding support. If you have any issues, please outreach to us at any time.

    FHT Contract Recommendations
    Thank you to everyone who participated in AFHTO’s FHT contract recommendations. Your advice and suggestions were critical for us to develop a thoughtful set of recommendations for the Ministry.  
    The recommendations for modernizing the FHT contract, Schedule A, the governance attestation document, and the annual operating plan have been submitted. You can read more here. We expect to hear back soon on next steps.

    Budget Planning for FY 2023/2024
    Many of you have heard that CHCs were asked by Ontario Health to plan for a 2% increase in their base funding as part of their planning assumptions for the next year. OH has said that planning assumptions are not a confirmation of funding and should be used for planning purposes only.

    Funding comes from the Ministry, so there is no guarantee of an increase; however, AFHTO has strongly advocated that any increases to CHCs are given to all interprofessional team-based primary care models. We hope to hear soon about the budget approvals.

    TRIN funding
    FHTs and CHCs should have received their temporary retention incentive for nursing (TRIN) funding, and NPLCs were sent their approval letters on November 30 with anticipation of flow-through of funding in January.

    Nurses paid directly by physicians and specialists are still waiting on their incentive funding. These funds will start making their way to the privately funded nurses in the next few months. Any questions should be directed to pcoinquiries@ontario.ca.

    Procurement Activity Reporting
    The Ministry of Public and Business Service Delivery has added large primary care teams to the list of funded organizations responsible for procurement activity reporting. This applies to teams that receive $10M or more in funding. These teams received an email from their senior program consultant notifying them that they now need to submit their procurement activities as a Broader Public Sector (BPS) accountability measure.

  • R&R Funding Letters Rolling Out Soon

    Dear Executive Directors and Admin Leads,

    We received word late Friday afternoon that the FY 2019-20 recruitment and retention letters will be rolling out in the next month or so. Annual Operating Plans (AOPs) will be released shortly as well. As we learn more information, we will share with you, but we are pleased to see that the R&R investment made in the 2018 Ontario Budget is still committed.
