Implementing community based primary healthcare for older adults with complex needs This article featuring nine case studies from Quebec, Ontario and New-Zealand published in the International Journal of Integrated Care is now publicly available. Help us take D2D to the next level! Your input will help guide us as we move, together, into a culture of learning and improvement. We’ve asked EDs to complete this short survey on behalf of their teams (yes – this is the correct link!). So far, we’ve heard from 34% of our members. We would like to double that (or more)! The leading contenders for a collective improvement focus are follow-up after hospitalization and diabetes care. Think it should be something else? There’s still time to have your say!
