This article featuring nine case studies from Quebec, Ontario and New-Zealand published in the International Journal of Integrated Care is now publicly available.

Two of the Ontario case studies feature AFHTO members South East Toronto FHT (integration with  CCAC) and Carefirst FHT (community care agency).

Your input will help guide us as we move, together, into a culture of learning and improvement. We’ve asked EDs to complete this short survey on behalf of their teams (yes – this is the correct link!). So far, we’ve heard from 34% of our members. We would like to double that (or more)!
