| AFHTO Letter to Minister Elliott “As one of your government’s goals is the promise to address the issue of hospital overcrowding and ending hallway medicine, creating a robust and well resourced primary health care system is a major solution to address this problem.” On July 10, 2018 AFHTO’s CEO sent a letter to Ontario’s new Minister of Health and Long-Term Care and Deputy Premier, the Honourable Christine Elliott. Extending our sincerest congratulations on her appointment, she expressed our desire to work with the government in improving healthcare for all Ontarians. | Transitions in Care Team Grants The Canadian Institutes of Health Research (CIHR) will launch team grants under the Transitions in Care (TiC) multi-Institute strategic initiative this summer. The grants “will offer pragmatic solutions to problems associated with transitions in care from a clinical, health care or health system perspective. The funding opportunity will use a partnerships approach where multidisciplinary/interdisciplinary research teams will identify ways to develop, implement, evaluate and/or scale and spread evidence into practice to optimize transitions in care outcomes within two or more of the TiC primary focus areas.” Complete details should be posted on ResearchNet by early August 2018. NEW– The “how to” category winner will also receive an education grant. This means 4 of the 7 categories now have education grants to help you strengthen your team’s development. AFHTO is developing a series of case studies identifying FHTs and NPLCs that have successfully undertaken a range of collaborative governance initiatives with other FHTs, NPLCs or health service providers. The purpose of this work is to support the members of AFHTO by providing a description and analysis of how and why boards are talking to other boards and the benefits achieved. | How effective is your board? Find out for yourself. The GCE’s Board Self-Assessment Tool is an online, questionnaire that takes about 20 minutes to complete. GCE summarizes the results and formats them into an easy-to-read report to support governance improvement, board learning and enhance organizational performance. | Early Psychosis Intervention Ontario Network needs your feedback The Early Psychosis Intervention Ontario Network (EPION) is seeking your feedback. To improve communication pathways between primary care and early psychosis programs throughout Ontario, EPION has developed a simple correspondence template for early psychosis programs to share concise, timely information with primary care about mutual clients. What is included in this template is based on information gathered from primary care providers during a province-wide survey in November 2017. They hope to know if this would be useful for you as primary care providers and seek any ways they can improve it. They aim to have this simple tool available to EPI programs throughout the province by Fall 2018. Please provide your feedback to the forms by August 6 to Brian Cooper (cooperb@stjoes.ca).
