- Ontario Community Hubs resource network: is asking for your help in adding information and resources to the site. This request was brought to our attention by a fellow AFHTO member who noted, “Collaborative hubs are especially useful in our rural and northern communities; however, the concepts are useful in serving multiple urban populations as well, especially those that have higher health equity challenges.” Learn more here.

- Mobile medical app available for patients with hypertension and primary care providers: BProactive prompts patients to take their blood pressure readings, which are then sent directly to a clinician-accessible portal via a secure cloud service.
