    Windsor Family Health Team

    The Windsor Family Health Team expanded their service delivery through a Team Care Centre (TCC) model, in partnership with the City Centre Community Health Centre. This model serves the rostered patients of 100–125 solo primary care practitioners in the Windsor area, or approximately 200,000 people.

    The TCC model provides solo primary care providers with an interdisciplinary team-based, patient–centred approach in mental health diagnosis, and treatment plans for individuals with mental health/addictions and complex care needs.  It also increases access to wrap–around services and programs to meet the health care needs of the community.

    The success of this model is seen through 100 community physicians providing over 1,000 referrals since September 2018. Patient outcomes have improved, patient satisfaction rates have increased and unnecessary visits to health care providers have reduced. Moreover, mental health patients have experienced enhanced quality of life, and benefitted from improved care coordination and navigation.

    Nominations are coming in fast and furious but so are the questions. Given the appetite for recognizing your team, colleagues and partners for the awesome work they do, we’re giving everyone a chance to submit a Bright Lights nomination by extending the deadline. Maybe you can even get to the cottage early.

    While there are six categories, we’d like to encourage members and partners to submit a nomination within the category “4. Patient and family-centred care”.

    Small, rural and Northern teams are encouraged to apply. You can watch this 2016 webinar or view the slides if you’ve never submitted a nomination before. There will also be special recognition for underrepresented teams.There will also be special recognition for underrepresented teams.

  • Northeastern Manitoulin FHT Pharmacist receives Patient Impact Award

    TORONTO – Little Current resident and pharmacist Diane Meehan has received the Ontario Pharmacists Association (OPA)’s Patient Impact of the Year Award. Ms. Meehan will receive her award at Pharmacy Experience Pharmacie (PxP) 2019 – a joint conference hosted by OPA and the Canadian Pharmacists Association in Toronto from June 3-5.

    This award is unique in that it is presented to a pharmacist  nominated by a patient or non-pharmacist colleague. The nomination is based on an outstanding commitment to delivering quality patient care and customer service, and for having a lasting impact on patient outcomes or community health and wellness. 

    “Diane has exemplified what it looks like to care for your profession and your community in tangible ways,” said Bill Wilson, interim CEO of OPA. “Her work in Northern Ontario has helped make quality pharmacy and healthcare council more accessible, which has earned her this recognition and award.”

    After completing her education in southern Ontario and BC, Ms. Meehan returned to Northern Ontario and worked in community pharmacy before buying the Island IDA Pharmacy in Little Current in 1994. She then worked with Wiikwemkoong to open the Mnis IDA Pharmacy. During that time, she was a consultant for the Manitoulin Health Centre, Manitoulin Centennial Manor and the Wikwemikong Nursing Home. In 2004, she sold her stores and worked as a locum pharmacist before working part-time for the Northeastern Manitoulin Family Health Team and the Edgewater Pharmacy in Little Current. 

    In her role with the Northeastern Manitoulin Family Health Team, she works with medical learners from the Northern Ontario School of Medicine and mentors PharmD candidate students on a six-month placement each year. She has also dedicated her career to continuing education and has obtained various certificates offered by OPA and certification in geriatric pharmacy, asthma education, respiratory education, smoking cessation and, most recently, the International Society of Travel Medicine Certificate in Travel Health. 

    Ms. Meehan has also volunteered in the community, including being on the executive of the nursery school, parents’ council of the local school and implementing a greening program for the school to plant trees sponsored by local businesses. 

    OPA’s annual awards ceremony recognizes the significant contributions being made by individuals in the pharmacy profession throughout Ontario.

    Reminder- CCO transition to fecal immunochemical test June 24
    Cancer Care Ontario through its ColonCancerCheck (CCC) screening program is transitioning to the fecal immunochemical test (FIT) as the recommended screening test for people at average risk of developing colorectal cancer. The guaiac fecal occult blood test (gFOBT) will no longer be used in the CCC program. The FIT will be available through the CCC program on June 24, 2019. The community was engaged on how the learning can be applied within Ontario. The links to the summary and video are on our site.

    CAMH opioid de-implementation project update
    On behalf of our partners at the Centre for Addiction and Mental Health (CAMH), we’d like to provide you with an update about the Opioid De-Implementation Project, an initiative which aims to improve opioid stewardship and non-palliative pain management by identifying and replacing low-value clinical practices with high-value, evidence-based approaches (a process known as de-implementation).

    As part of the initiative, CAMH will be offering an electronic pathway that will to guide practitioners to assess a range of patient/client domains, including pain severity and interference, current pain treatments, physical and mental health, and substance use. This information will help to identify the risk of opioid overdose or dependence, and based on these risks, will offer recommendations to support shared care decision-making during treatment planning.

      • In 2018 Algonquin and Cottage Country FHTs won special recognition for effective response to rural community need. This year all new/underrepresented primary care teams will again have the opportunity to win an award for their contributions to Ontario’s healthcare system. The winner(s) will be selected based on the strength of their nomination to one of the six categories above.

TimminsToday Staff NEWS RELEASE NORTH EAST LHIN ************************* The North East Local Health Integration Network (NE LHIN) has named Jennifer McLeod, Executive Director of the Timmins Family Health Team, a Healthy Change Champion. McLeod has worked in many areas of health care for more than three decades, including acute care, long-term care, public health, primary care in remote Northern communities, and most recently, with the Timmins Family Health Team. “Better health care is provided to patients when health agencies work together to deliver quality health services,” says McLeod. “Services are delivered more efficiently, with minimal duplication, and gaps in service are more easily identified and resolved.” McLeod says that more than ever, it’s important for primary care and home and community care to work together closely so that people get the streamlined care they need right across the system, regardless of where they live. “Jennifer has been instrumental in many patient-focused projects in the Timmins area, including the Health Link, Timmins Palliative Resource Team, the North East Specialized Geriatric Centre Regional Systems Steering Committee, and more.  She is renowned for her collaborative approach,” says Louise Paquette, CEO, NE LHIN. “Jennifer’s leadership is exactly what the system needs as we continue to find ways to work together better so Northerners get a positive health care experience as close to where they live as possible.” McLeod’s family moved to Moosonee from Toronto when she was in her teens. She lived there for the next 20 years where she raised her family. Although she moved south to Timmins in 1994, she maintains close ties to family in Moose Factory. McLeod says Timmins has a very collaborative health care team that includes most of the area’s primary care providers and many specialists. She says together they have been able to achieve significant changes in their delivery of health care. “As a Healthy Change Champion, I simply help to guide this process ensuring that we are moving forward. This is an interesting and rewarding role in Timmins as our accomplishments reflect the accumulation of the efforts and commitment of many,” McLeod added. FACTS:

    • Health Links are a model of care across the province that started in 2012 in which several providers in a community – including primary care, hospital, community care – work together to coordinate care plans at the patient level. Health Links are accountable to LHINs. The Timmins Health Link is one of two early adopters in the NE LHIN region and has developed care coordination plans for more than 60 patients with multiple, complex conditions.
    • Health Links locations now include Timmins, Temiskaming, Nipissing-East Parry Sound, North Cochrane, Sault Ste. Marie, and Sudbury.
    • The Timmins Family Health Team provides primary care to approximately 25,000 patients in Timmins across six clinical sites with 31 physicians, five nurse practitioners, and approximately 35 staff.
    • The NE LHIN Healthy Change Champion recognizes people or organizations transforming the health care system so that it becomes more patient/client-focused, integrated, and easier to access.   • Manitoulin Central FHT Physician named Regional Physician of the Year

    MINDEMOYA—Dr. Maurianne Reade, a physician at the Manitoulin Central Family Health Team in Mindemoya, is this year’s Northeast and Northwest Ontario 2016 Regional Physician of the Year as awarded by the Ontario College of Family Physicians (OCFP) Awards.

    The OCFP’s Annual Awards program celebrates the outstanding skill, knowledge and dedication of family doctors across Ontario, a press release from the association states. The awards recognize excellence in community and medical leadership, outstanding contributions in teaching and exceptional teamwork.

    The Regional Family Physician of the Year Award recognizes the outstanding contributions of seven Ontario family doctors who provide exemplary care to their patients and are passionately involved in activities that contribute to excellence in family medicine.

    Dr. Reade has practiced as a rural physician in Mindemoya since 2001. She is an associate professor at the Northern Ontario School of Medicine, a board member of the Physician Clinical Teachers Association and an executive member of the OMA Academic Medicine Forum and Rural Expert Panel.

    Described by peers as someone who views health holistically, her dedication to family medicine shines through in her daily routine where she can be found caring for patients in their homes, in the family health team clinic or First Nations Health Centre, the local emergency room and hospital ward.

    “Good for Dr. Reade,” said Dr. Cooper, MHC chief of staff when contacted by The Expositor. “She certainly deserves some credit for all she does. Dr. Reade is an inspiration to her colleagues, a pleasure to work with, a tireless educator and an excellent clinician.”

    The awards will be handed out at a gala event later this month at the Fairmont Royal York.

  • Couchiching FHT to be named 2016 LEADing Practice

    Congratulations to the Couchiching Family Health Team for being named a 2016 LEADing Practice Initiative. The FHT is being recognized for exemplary use of digital tools to strengthen clinical practice and provider experience. Couchiching FHT’s project integrated a tablet-based system into their EMR to screen patients at risk for chronic obstructive pulmonary disease (COPD). This also allowed patients to edit demographic information stored in their patient chart in the EMR in real time, without any increase in staff time or resources. Presented at the AFHTO 2015 Conference, benefits of the system included approximately 40% (or 3000) of the total patient population completing smoking screening in a little over 4 months compared to the previous, paper-based COPD screening process in which only 200 patients were screened at baseline. There was also a 33% increase in referral to the smoking cessation program. Couchiching is now expanding the tablet program to further identify and support individuals with other chronic diseases e.g. diabetes, and using such screenings for depression. The LEADing Practice Initiative, a partnership between Canada Health Infoway and Accreditation Canada is part of a larger Clinician Education Campaign, identifying LEADing practices across Canada that demonstrate the clinical benefits of digital health. Their award will be presented at the 2016 Peer Leader Symposium: Building Peer Leader Bridges to Advance Clinical Practice event to be held on March 3-4, 2016 at the Intercontinental Toronto Centre in Toronto.     On September 25, 2015 the Nurse Practitioners’ Association of Ontario (NPAO) held their Awards and Celebration Dinner as part of their annual conference. Nurse practitioners (NPs) from different organizations were honoured, with half of the awards presented to NPs from member FHTs and NPLCs.

    Little innovations, big impact – Bright Light Award winners are innovators, making meaningful changes to improve the patient experience, health outcomes and reduce overall costs to the health care system. The awards program recognizes leadership, outstanding work and significant progress being made to improve the value delivered by interprofessional primary care teams across Ontario.

    Awards will be presented at the third annual AFHTO 2014 Conference Awards Dinner, on October 15, 2014. Winners in four award categories will receive an education grant*. Deadline to complete the online form and submit supporting documents was August 8, 2014.

    Make a nomination:

    1. Review the Bright Lights nomination guide to prepare your nomination.
    2. Complete the online nomination form. All nominations must be completed online and in full. To avoid the risk of losing information, it is best to complete the online form in one session.
    3. Submit supporting documents to info@afhto.ca:
