    2022 has been a difficult year for primary care and we want to thank our teams for their resilience and capacity to adapt to the constant challenges that came their way. You have all continued to support not only your patients but non-rostered patients in your communities, collaborated with partners to advance your programs and continued to advocate for the ongoing expansion of team-based care. We aren’t able to highlight all the wonderful work for teams in 2022 in this short newsletter, but we encourage you all to read our Annual Report and explore our Bright Lights Nominees and Award Winners.

    Celebrating Teams: Collaborating to Improve Palliative Care

    Care for Palliative End of Life Patients with Bancroft FHT

    In April 2022 the Bancroft Community Family Health Team formed a partnership and collaboration with North Hastings Hospice in Bancroft Ontario to provide an alternative level of care to their palliative end of life patients and residents of the community.

    Through the System Navigation Program, patients can be referred directly from physicians, nurse practitioners, Home and Community Care Services, Hospitals, and other community support agencies. The Palliative System Navigator RN will complete the initial intake process by meeting with the patient to discuss their end-of-life care and wishes. The nurse will then make the appropriate arrangements for the patient, coordinating in home services or admission to Hospice House in collaboration with one of the BCFHT FHO physicians.

    The goal is to provide effective, quality, compassionate, and coordinated care to palliative end of life patients and community residents in an appropriate setting while at the same time reducing hospital admissions. For any questions, please email mkelly@bancroftfht.com.

    Couchiching FHT Streamlines Access to Palliative Care Resources

    Couchiching FHT has provided leadership and resources to the Couchiching Palliative Care Working Group for several years. With the creation of the Couchiching Ontario Health Team (COHT), this working group was able to leverage the strong collaboration within the COHT to make incredible strides towards accomplishing their goals.

    This group of service providers and system users has created a central referral system which went live in February 2022. There is one referral form to access all palliative care services in the sub-region and a list of primary care practitioners who will accept non-rostered palliative patients, so no palliative patient in Couchiching is going without primary care. In addition, they have streamlined and coordinated resources to reduce duplication and make it easier for patients and their caregivers to find the right resources at the right time.     Celebrating Teams: Filling Gaps in the Community

    Community Newborn Program at the Belleville NPLC

    The purpose of the Community Newborn program is to provide follow up care to newborns who do not have a primary care provider in the community. Infants are seen by the program staff within 72 hours of discharge from the hospital. The Community Newborn Program consists of a Nurse Practitioner-Paediatrics, a Registered Nurse, and an Admin Support who each work 20 hours per week to support patients in this program. The NP and RN provide routine follow up care and non-emergent assessments for the infants up until 6 months of age. They also provide routine vaccinations for siblings of their infant patients who also do not have a primary care provider. For any inquiries about this program, please email admin@bnplc.ca.

    Prenatal and Well Baby Program at Barrie and Community FHT

    The Prenatal and Well Baby (PNWB) Program provides prenatal and well-baby care for women and children (up to the age of 6) without a family physician in the Barrie and surrounding area. The program has nurses, nurse practitioners, physicians and lactation consultants that provide care, education and referrals into community supports. The program also performs tongue tie releases and has a paediatrician available for consultation.

    Throughout the pandemic, the PNWB Program and Breastfeeding Services continued to see patients daily for essential care. During the pandemic, the Barrie area has seen an influx of new residents. Many of these new community members do not have a family doctor close to home, and the PNWB clinic is a place they can receive this essential service in the community they live.     Healthcare Excellence Canada Opportunity: Partnering on Appropriate Virtual Care
    The Partnering on Appropriate Virtual Care collaborative will support primary care practices, organizations, and multidisciplinary teams from across Canada to determine when and how virtual care should be used in their respective healthcare settings. Participants will receive up to $20,000 in seed funding to develop a framework that supports shared decision-making with patients, ensuring choices around virtual care are based on patient needs and capabilities, their care requirements and clinician capacity. This program runs between January and November 2023 and involves a range of online learning and design activities. Learn more about the program and how to apply here.

    Physician Consultations on PHIPA Changes
    The Ministry of Health is seeking physicians interested in participating in a focus group to talk about their experiences with the Personal Health Information Protection Act, 2004 (PHIPA). This work is part of the Ministry’s Dialogue on Data engagement strategy. Your knowledge and experience will inform the development of a data strategy report to the Ontario Health Data Council that will be seen by the Minister of Health, the Information and Privacy Commissioner of Ontario, and many other important stakeholders. Click here to learn more. Focus groups will take place January 17 to 19, from 12-1 pm and we are looking for 20 physician volunteers spread over the three days.     Support Tools for Clinically Appropriate Use of Virtual Care Guidance in Primary Care
    The two patient-facing support tools for the Clinically Appropriate Use of Virtual Care Guidance in Primary Care are now live on the Ontario Health website. The patient-facing tools, ‘Learning about Virtual Care Options’ and ‘Checklist for Use of Virtual Care’, are currently available in English and French. There is work underway to translate the patient-facing tools into additional languages. This will be made available on the Ontario Health website when ready.     Quality Improvement Plans (QIPs) 2023/24
    Ontario Health announced the start of the Quality Improvement (QIP) program cycle for 2023/24. This year’s QIPs will continue to focus on a small number of quality issues that are important to the health care system, such as issues that have been particularly impacted by COVID-19 and need our attention to help support health system recovery.

    Priority indicators have been developed in consultation with the Ministry of Health, Ministry of Long-Term Care, and other teams across Ontario Health; however, organizations may also choose to focus on local issues that are important to their communities. Ontario Health encourages organizations to work on improving equity and supporting cultural safety and responsiveness. Organizations can choose to add custom indicators aimed at improving equitable access to care and/or share goals in the Narrative section.

    QIP Navigator, the online QIP development and submission platform, has been updated for this year’s submission and is now open for 2023/24 QIP submissions. Organizations can download pre-populated templates and current performance will be pre-populated with administrative data in January where available. Ontario Health will be hosting drop-in sessions on Zoom which are optional and will provide you with an opportunity to ask questions about the QIP program, QIP Navigator and learn about available quality improvement resources. QIPs must be submitted by March 31, 2023. Please email QIP@ontariohealth.ca if you have any questions or concerns.

    Integration is the lever to enable population health and well-being. A key component of integration is the role of partnering with the volunteer sector to improve outcomes for our patients, caregivers, and our population. In this session, we will take a deep dive to learn firsthand about the role of engaging the volunteer sector from the perspectives of policymakers, researchers, and a person with lived experience. This session will discuss the available evidence, the volunteer sector’s role in policy, and share approaches and practical examples of local work that engages with the volunteer sector.     ADVANCE Special Topic Series- Balancing Task and Process in Leading Integrated Care Systems
    Monday, January 9, 2023, 12-1 pm
    Developing integrated care systems often requires newly constituted intersectoral teams to form and rapidly engage in mutually reinforcing activities of integration, while attending, in parallel, with the processes by which these new teams engage in collaborative leadership, decision-making and/or system improvement. Professor Robin Miller, based at the University of Birmingham, will discuss the promise and pitfalls of collaborative leadership within integrated care systems and how attending to process can support inter-sectoral teams to achieve the aim of collaborative governance.     Health ministers’ meeting in Vancouver

    “Mr. Duclos … added the federal government is also prepared to make bilateral agreements with provinces and territories on issues of shared priorities, such as increased access to family health teams and mental-health services.” Full article here.

    As the provincial and territorial health ministers gather in Vancouver to meet with the federal Minister of Health, the Hon. Jean-Yves Duclos, AFHTO remains hopeful that all parties share his priority of increasing access to primary care teams and that we will see much needed funding flow to the province through the Canada Health Transfer.

    Reminder – Supporting Caregivers Supports Patients- The Critical Role of Primary Care in Preventing Caregiver Burnout, Nov. 24

    Primary care providers can play a key role in preventing burnout and enhancing caregiver well-being by initiating early interventions and connecting caregivers to support.

    Co-hosted by The Ontario Caregiver Organization and the Primary Care Collaborative, this webinar will include insights from a caregiver with lived experience, as well as examples of primary care initiatives across Ontario that are connecting caregivers to support. Register here.

     

     


    Virtual concussion occupational therapy services for teams

    The Neurology Centre of Toronto (NCT) has developed a concussion rehabilitation program that has been implemented and proven successful with an existing FHT in Ontario. The program provides patients with access to virtual concussion occupational therapy services and has been running successfully since March 2022.

    The patients in this program wouldn’t have access to this care otherwise, and the program has generated very positive feedback and results. Specifically, when surveyed after completing this program:

    • 33% of patients demonstrated an improvement in daily activity tolerance
    • 67% of patients demonstrated an improvement in their symptom scores, and
    • 83% reported satisfaction with the program.

    NCT would like to extend this service to other teams (including NPLCs and CHCs) in need of concussion rehabilitation services to support their patients.     Moving towards an integrated health data ecosystem & QIP update

    Taking place March 27-29, the Ontario Long Term Care Association (OLTCA) in partnership with the Ontario Retirement Communities Association (ORCA) are hosting the Together We Care 2023 Convention in person.     Nominate high performing early career family physicians

    Dr. Monica Aggarwal at the University of Toronto is conducting a study that will identify the key capabilities and competencies of high-performing family medicine graduates. This will help inform how learning, teaching and assessment can be improved in family medicine residency programs in Canada.

    IPHCC & PHAC seeking participants for short videos

    The Indigenous Primary Health Care Council is looking for both health care staff as well as general population participants for the creation of short videos.

    Created in collaboration with the Public Health Agency of Canada, the videos aim to increase knowledge about the importance of vaccination, build vaccine confidence, and address barriers to accessing vaccination services.     Farewell to Sandeep Gill

    It is with genuinely mixed feelings we announce that Sandeep Gill will be leaving AFHTO for a new role in the acute sector.

    Sandeep has been with AFHTO for nearly three years, most recently as Manager, Quality and Knowledge Translation.  So many have you had the pleasure of working with her or attended the webinars she has organized over the last few years. She has also helped AFHTO grow its research portfolio by securing several research opportunities which have also led to a few publications.

    We’re going to miss Sandeep but know she will bring so much to her new team. Please join us in giving her our sincerest best wishes. Her last day with AFHTO will be Friday, April 8th – any questions related to her work can be directed to info@afhto.ca.

    The Role of Primary Care in Vaccine Distribution – Currently recruiting MDs and IHPs!

    AFHTO has partnered with Dr. Rachelle Ashcroft on a study focused on understanding how and in what ways primary care contributes to vaccine distribution, challenges experienced by primary care in the transition of vaccine distribution, as well as generating recommendations about primary care’s ongoing and future roles in vaccine distribution, including COVID-19 boosters.

    This study will be in the format of focus groups with EDs, MDs, NPs, nurses, QIDSS and IHPs.

    Quality Improvement Plans (QIPs) 2022/23

    For the past two years, Quality Improvement Plan (QIP) submissions have been on pause while organizations did the necessary work to manage the COVID-19 pandemic in their communities and regions. It has been a difficult time. While we know focused commitment to quality is vital, we also know that our system is facing unprecedented health human resource challenges and instability related to the COVID-19 pandemic. As such, we want to clearly convey to you our expectations regarding your Quality Improvement Plans (QIPs) for 2022/23: This year submission of your QIP to Ontario Health will be considered voluntary.  Please e-mail QIP@OntarioHealth.ca if you have any questions.  As always, the team is here to support you.

    This year’s QIPs will focus on a small number of quality issues that are important to the health care system—issues that have been particularly impacted by COVID-19 and need our attention to help support health system recovery. The small number of priority indicators in this year’s QIPs have been developed in consultation with the Ministry of Health, the Ministry of Long-Term Care, other teams across Ontario Health, and organizational leaders like yourselves. Organizations may also choose to integrate other local issues that are of importance to their communities.  

    Priority indicators by sector are provided in the Appendix below. Supporting documents, including a QIP guidance document, technical specifications for the indicators, and a snapshot of quality priorities in brief for 2022/23, are also available. QIP Navigator, our online QIP development and submission platform, has been updated in preparation for voluntary submission. As usual, organizations can download pre-populated templates from QIP Navigator to support their QIP development process.  

    Individual organizations who choose to submit their QIPs to Ontario Health are encouraged to do so by April 1, 2022. However, in consideration of the current pressures on the health care system, there will be flexibility with timing of submissions and QIP Navigator will be kept open until June 30, 2022.  

    This is the first year that organizations that are part of an OHT will work to submit their organizational-level QIP while also contributing to their OHT’s cQIP. For that reason, and because organizations are continuing to respond to COVID-19, we have limited the number of focus areas for both the cQIP and the organizational-level QIP. Organizations that wish to reflect their cQIP work in their QIP may also include the cQIP indicators in their organizational QIP.

    In follow up to the enclosed distribution from Ontario Health regarding the submission of this year’s Quality Improvement Plans.  We wanted to confirm that the Ministry is supportive of Ontario Health’s plan to make this year’s submission voluntary and confirm that teams are not contractually required to submit this report for 2022-23.

    We continue to encourage all organizations who choose to complete this work to develop and post your 2022/23 QIP on your website and share it with your administrative staff, clinicians, and patients/residents and their family members.

We strongly advise you to review the list of clinics here to see if your team is on the list, as this solution is not aligned to the service standard.

    For any associated privacy concerns, please reach out to the Information and Privacy Commissioner (IPC). If your team did not agree to be listed on this website and would like to be removed, please directly reach out to Bleen via phone: 1-866-943-9480 and via an online form here. We have brought this issue to Ontario Health’s attention, and they are working with OntarioMD and the Ministry to bring a resolution to the issue.

    We also encourage you to take this opportunity for patient education if you receive any appointment requests through this platform and advise patients on approved appointment booking practices for your team.     New! Quality Standard on Transitions From Youth to Adult Health Care Services
    In February 2022, Ontario Health released the Transitions From Youth to Adult Health Care Services quality standard, developed in partnership with the Provincial Council for Maternal and Child Health.
    When young people transition from youth to adult health care, they are at increased risk for negative health outcomes as they adapt to new services, people, and processes.

    The quality standard is an evidence-based resource that outlines what high-quality care looks like during this critical time. It can be used to help young people, families, and caregivers know what to ask for in their care; to help health care professionals know what care they should be offering; and to help health care organizations measure, assess, and improve their performance in caring for patients.

    Developing A Practice Tool for Prescribing Cascades in Primary Care

    Are you an Ontario physician, nurse practitioner or pharmacist who provides primary care as part of an interprofessional team?

    Are you interested in reducing medication-related harm?

    A University of Toronto research team wants to learn from clinicians working in interprofessional teams about your experiences and thoughts on interventions that may help to address prescribing cascades.

    What is a prescribing cascade?
    Prescribing cascades occur when a new medication is prescribed to treat the side effects of another medication. These often arise when prescribers attribute patients’ new signs or symptoms to a medical condition rather than a medication side effect. Prescribing cascades are important sources of potentially unnecessary medication use.

    What is being asked of me?
    –    Complete a brief questionnaire (< 10 minutes) and then participate in a up to 1.5-hour discussion group or interview to share your perspectives. You will be invited to participate in a second discussion group or interview (also up to 1.5-hours) if interested.
    –    A $100 gift card will be provided for each discussion group or interview that is attended.

    In follow up to the distribution from Ontario Health (EN and FR) regarding the submission of this year’s Quality Improvement Plans, the Ministry wrote to teams yesterday to confirm that they are supportive of Ontario Health’s plan to make this year’s submission voluntary and confirm that teams are not contractually required to submit this report for 2022-23.

    The Ministry continues to encourage all organizations who choose to complete this work to develop and post your 2022/23 QIP on your website and share it with your administrative staff, clinicians, and patients/residents and their family members.

    The provincial election is just over three months away! The election period will start in early May, and election day is June 2. However, campaigns are underway now and health care is a priority for all parties.

    AFHTO is arranging meetings with ministers, opposition health critics, and party leaders or central campaign leads. And now is the time for local candidates to hear from you!

    To help make sure parties understand the importance of team-based primary care and how to address the crisis facing the healthcare sector, candidates need to hear about it – and solutions on how to fix it – from local stakeholders and voters.

    Submission of Quality Improvement Plans (QIPs) for 2022/23 voluntary

    On Feb. 18, Ontario Health sent out a memo regarding annual priorities for the 2022/23 Quality Improvement Plans. For the past two years, QIP submissions have been on pause. This year QIP submission to Ontario Health is voluntary. Please email QIP@OntarioHealth.ca if you have any questions. You can also register for the Feb. 24 webinar QIP 2022/23 Program Launch here.

    Update on the 2021/22 and 2022/23 Quality Improvement Plans (QIPs)

    On March 10, 2021, Ontario Health communicated that an adjusted strategy for the 2021/22 QIP would be forthcoming, designed to provide organizations with flexibility to reflect where they are with their QI efforts given the pandemic, local needs, pressures, and goals for the recovery period.
     
    After careful consideration, they have made the decision to not reopen Navigator for the 2021/22 QIP submission. You are not required to submit your 2021/22 QIP to Ontario Health. They do, however, encourage you to develop and post your 2021/22 QIP on your website and to share it with your staff, clinicians, and patients/clients and their family members. Please reach out to the team at QIP@OntarioHealth.ca if you have any questions about this. You can read the full email in English and French here.

    A new tool for primary care providers: Quality Standard Placemats

    Quality standards outline what high-quality care looks like for conditions or processes where there are large variations in how care is delivered in Ontario. They are based on the best evidence and designed to help you know what to do to reduce gaps and variations in care – there are over 25 quality standards developed by Ontario Health, in collaboration with health care professionals, patients, and caregivers across Ontario.

    Ontario Health is pleased to share a newly developed tool called the Quality Standard (QS) Placemat that is a resource for primary care practitioners. The placemat highlights key elements of care for a condition and:

    • is an evidence-based, quick-reference tool that concisely summarizes key information from the quality standard
    • includes links to helpful resources and tools
    • can be used to facilitate patient-centered discussions

    Three quality standard placemats have been developed to date:

    Low Back Pain QS Placemat – summarizes content from the Low Back Pain Quality Standard, focusing on care for people 16 years of age and older with persistent or recurrent episodes of acute lower back pain. French version.

    Acute low back pain and osteoarthritis represent two of the most common clinical conditions encountered on a daily basis in the primary care setting. The session will review delirium and its significance to COVID-19 infection, introduce key statements from the quality standard, and showcase related resources available to support people who are at risk for delirium or who are experiencing symptoms of delirium.     Quality Improvement Plans – Update from OH(Q)
    Last April, QIP submissions for 2020/21 were paused to allow organizations to focus on supporting their staff, health care providers, and communities during the COVID-19 pandemic. The QIP Navigator remained open so that organizations that had the capacity to continue working on their submissions were able to do so. We will be closing the 2020/21 cycle in Navigator on March 24, 2021.

    As we look ahead to the 2021/22 QIPs, we understand that organizations continue to be under unprecedented pressure related to the COVID-19 pandemic and that this will likely continue for the next several months. As such, we are making adjustments to the 2021/22 QIP to help ensure that organizations are able to focus their efforts on responding to urgent needs related to COVID-19. While quality improvement is of critical importance in our collective response to COVID-19, for now, the pause on the program will continue.

    We will monitor the situation and will share a revised approach in the coming months, ensuring that it is responsive to circumstances in the health system and that you have sufficient time to prepare. The approach to 2021/22 will be designed to provide organizations with flexibility to reflect where they are with their quality improvement efforts given the pandemic, local needs, pressures, and goals for the recovery period. We encourage organizations to continue with their local quality improvement initiatives and we will be in touch soon.

    A Peoples’ Panel on how race and ethnicity data should be used for health data research in Ontario
    ICES is working together with the Tamarack Institute and Digital Justice Lab to lead a Peoples’ Panel on how race and ethnicity data should be used for health data research in Ontario. Through this Peoples’ Panel, ICES seeks to recognize the ways that science has perpetuated racism and understand the complexities in the use of race, ethnicity, and immigration data.

    The Peoples’ Panel will involve 30-60 diverse and representative individuals who will meet weekly for 5 weeks starting on March 17, 2021. ICES is seeking regular people—no experience in health, science or policy needed. If you live in Ontario and have an interest in how race data is used, and especially if you identify as racialized individual, they invite you to express your interest in being part of the Peoples’ Panel. Participants will be compensated.

    Supporting Older Persons At Home – Learning And Collaboration During The COVID-19 Pandemic And Beyond

  • Year-End Ministry of Health Updates

    Dear Executive Directors and Administrative Leads,

     

    We had a year-end call with ministry officials last week and would like to provide an update on multiple operational topics that were discussed.

    AOP submission

    There will be a pause on AOPs for 2021/22. They will resume for an April 2022 submission. While there will be no AOP this year, teams can still submit unsolicited funding requests as needed.

    The ministry is also analyzing quarterly reports and forecasting data to examine increases to overhead, rent, hydro, IT, professional services, etc. Through this process, they hope to get a sense of overall budgetary trends, limitations, and constraints across teams. We will share more on this work as we learn it, but we encourage teams to be timely with quarterly reports and audited financial statements to help inform this work.

    FHT contract

    The FHT contract is due to expire in 2022. However, the ministry has indicated that they will be extending the contract for one year, which they will communicate to teams directly when the decision is made. We have signalled that we would like to modernize the FHT contract to better reflect the current environment, so there will be discussion and consultation on possible contract changes and improvements. We do not yet have a timeline for these consultations, but we will begin internal discussions with members soon.

    If your team has not submitted the 2020/21 QIP, which was due last April, it would be good to close it out. OH(Q) has created a video to help with that process. For any questions, please contact OH(Q) at qip@ontariohealth.ca.

    Mental health and addiction funding

    All teams should have received their one-time federal funding allocation to support the increase in MH&A supports needed as a result of COVID-19. Unfortunately, our request to allow these funds to be carried over due to the timing of the funding receipt and the exceptional circumstances of COVID-19 was denied. Funding must be spent by March 31, 2021. However, there is considerable flexibility in how teams can utilize this funding if it can be tied to providing MH&A relief (e.g., training, equipment, IT).

    Please note there is very specific and very limited criteria for which the government allows funds to be carried over into the next fiscal year and the MH&A funding does not meet the criteria. (For example, OHT funding meets the requirements of “project funding for implementation and start up,” so there is allowance for carry over of OHT funds.)

    Funding for vaccine clinics

    There is no direct funding from the ministry for teams that are working in or running COVID-19 vaccine clinics. Any inquiries about this funding need to be directed to your local public health unit as they are managing the budgets related to roll-out of the vaccine clinics.

    Also, a reminder that physicians on your team can review a Ministry INFOBulletin regarding newly negotiated compensation for vaccinations and vaccine outreach.

    Governance review

    The ministry recently conducted an assessment of the Governance & Attestation documents submitted by teams as part of their annual submission requirements. This assessment identified two areas that need improvement for many boards: 1) board monitoring of organizational performance; and 2) board self-performance monitoring.

    Over the next few months, we will be developing practical tools and templates to support team boards in these two areas. As a result of the review, some teams may also receive a specific action plan from the ministry identifying opportunities for further improvement in governance practices.

    Call-out to EDs to join a short-term working group!

    One of the first tools AFHTO will be developing to assist with governance, as noted in the previous section, is a toolkit for “Governance Oversight of Organizational Performance.” This will be done in collaboration with the Centre for Organizational Effectiveness. AFHTO is looking for guidance and input from EDs to inform this work.

    We’re hoping three or four EDs may be available in April and May to provide insight and feedback! We expect this would be two or three 1-hour virtual meetings over the course of the two months, and a few occasional questions via email regarding the document as it is developed.
