    Focused vaccination efforts in response to Omicron variant

    With the Omicron variant becoming the dominant strain of COVID-19 and cases sharply increasing, the health system has been asked to implement an ‘all hands-on deck’ and immunize as many individuals over 50+ as possible before December 24th.

    To highlight the urgency and the request from the Ministry, several memos have been released to the field:

    From OntarioMD regarding finding those who would be eligible for third shots:

    • OH eReport has included 3rd shots since Nov. 15. Now instead of 1 of 2 or 2 of 2, the record is showing 1, 2, or 3. All toolkits at OntarioMD website have also been updated to work with this new report.
    • Also, there is a New additional report which includes the full vaccination history of all rostered patients whose vaccination status is on record.

    2021 University of Toronto Family Medicine Report

    The 2021 University of Toronto Family Medicine Report is authored by nearly 100 family doctors and health care providers from across Ontario, Canada and beyond. On the theme ‘caring through crisis’, it features personal stories, reflections and anecdotes that represent just a small fraction of the incredible work happening in family medicine and primary care, as well as data from the University of Toronto Practice Based Research Network (UTOPIAN). It includes stories from a number of our teams including St. Michael’s Hospital Academic and Toronto Western FHTs.

    Inequity in online renewal for OHIP

    Inequities in Ontario’s online health card renewal system must be addressed so marginalized people are not left behind.

    The Alliance for Healthier Communities (Alliance), the Association of Family Health Teams of Ontario (AFHTO), the Indigenous Primary Health Care Council (IPHCC), and the Nurse Practitioner-led Clinic Association (NPLCA) call on the government to address the inequities in the online health card renewal system.

    “The ability to renew the Ontario health card online is only available to people who hold a driver’s licence. This needs to change to include those with an Ontario photo card,” says Kavita Mehta, CEO of AFHTO. “There are a number of people who do not or cannot drive, including those with medical conditions. Like those with a licence, they need the same ability to renew online.” Read the full statement here.

    Seeking input on psychological safety factors

    In collaboration with the Mental Health Commission of Canada, researchers at the Centre for Studies in Primary Care at Queen’s University and HEC Montreal are inviting health-care workers and leaders to participate in research to identify barriers and facilitators to accessing support for psychological self-care and protection from moral distress.

    The BETTER Program: building on existing tools to improve chronic disease prevention and screening in primary care

    Teams are invited to learn more about The BETTER Program, which aims to improve chronic disease prevention and screening in primary care settings through a comprehensive, individualized approach provided by a Prevention PractitionerTM, which a regulated healthcare provider on a team can train to be through this program.

    It is an innovative, evidence-based program that focuses on prevention and screening of cancer (breast, cervical, colorectal), heart disease, and diabetes. It also addresses lifestyle risk factors, such as physical activity, smoking, alcohol, and diet. The program was developed by primary care providers for primary care providers.

    OHT supports events calendar launched

    The Ministry of Health, in partnership with the Rapid Improvement Support and Exchange (RISE) team, and partners in the OHT Central Program of Supports have launched the OHT Supports Events Calendar.
     
    The calendar now offers one central online location for teams at all stages of implementation to easily find information on learning and engagement events offered through the OHT Central Program of Supports and other health-system partners (e.g., webinars, information sessions). You can sign up directly for bi-monthly updates on upcoming events and access resources from past events.

    RNAO seeking best practice panelists-deadline extended

    The Registered Nurses’ Association of Ontario (RNAO) is seeking nominations for expert panel members for an upcoming new edition best practice guideline on Person-And Family-Centred Care, Third Edition. They are looking for 1 to 3 family physicians to be a volunteer on this panel which extends over 18 months.

    On Nov. 17 Benjamin Miller from Community Legal Education Ontario (CLEO) held an education session for members on what is new in the Ontario’s Not-for-Profit Corporations Act (ONCA), and steps nonprofits can take to set themselves up for success in transitioning to the new requirements under the Act.     Update on the 2021/22 and 2022/23 Quality Improvement Plans (QIPs)

    On March 10, 2021, Ontario Health communicated that an adjusted strategy for the 2021/22 QIP would be forthcoming, designed to provide organizations with flexibility to reflect where they are with their QI efforts given the pandemic, local needs, pressures, and goals for the recovery period.
     
    After careful consideration, they have made the decision to not reopen Navigator for the 2021/22 QIP submission. You are not required to submit your 2021/22 QIP to Ontario Health. They do, however, encourage you to develop and post your 2021/22 QIP on your website and to share it with your staff, clinicians, and patients/clients and their family members. Please reach out to the team at QIP@OntarioHealth.ca if you have any questions about this. You can read the full email in English and French here.

    2022 Alliance conference- call for learning proposals

    Implementing social interventions in primary care: article

    According to research published in in the Canadian Medical Association Journal (CMAJ), most family physicians now report that they engage in some degree of social intervention in the management of patients.     Workplace violence during care transition toolkit

    PSHSA has released the new Violence, Aggression and Responsive Behaviours (VARB) toolkit: Workplace Violence During Care Transition. This free toolkit provides information on sound practices for planning a course of action, gathering resources, and collaborating with workplace parties and other care organizations to transition care recipients with a risk of violence.     OISE Psychology Clinic welcomes child/youth and family therapy referrals

    Ontario Institute for Studies in Education (OISE) Psychology Clinic, a University of Toronto training site, has expanded services and is currently welcoming child/youth and family therapy referrals.     Ontario Health, in partnership with the Provincial Council for Maternal and Child Health has drafted a quality standard for care for young people who are transitioning from youth to adult health care services.     A reminder to please sign up for and spread the word across your teams and regional networks about AFHTO’s Member Consultation – 2022 Election Messaging on November 16 from 12 PM to 1:30 PM! This is member input on our election strategy and local messaging, and it is critical that we have good representation from a diversity of teams and professions.

    Breaking Free Online – Digital Support Tool for Substance Use Disorder: Ontario Health has launched Breaking Free Online, an online therapy tool for Ontarians 16 and up to support recovery from the use of over 70 substances such as tobacco, alcohol, and drugs, including opioids, and help prevent a relapse.     Health Care Navigation Service – Coming Soon: The Ontario government is awarding a contract to create a one-stop, digitally enabled navigation tool with fully bilingual services that will increase access to care and improve equity. This new tool expected to be ready in early 2022, where Ontarians will have a ‘Digital Front Door’ to Ontario’s health care system. Find out more here.

    Improving care across Ontario一and beyond一for children and youth with depression– The Centre for Addiction and Mental Health (CAMH) has launched a new free online tool to help improve the standard of care for youth living with depression. Cundill Centre Online Tool for the Treatment of Youth Depression summarizes best care practices for supporting youth as they manage their depression and is now available for primary care providers and other frontline clinicians everywhere.

    TUTOR-PHC call for applications
    Transdisciplinary Understanding and Training on Research – Primary Health Care (TUTOR-PHC), is a one-year, pan-Canadian interdisciplinary research capacity building program that has been training primary and integrated health care researchers, clinicians and decision-makers from family medicine, nursing, psychology, epidemiology, social work, occupational therapy, education, policy, and many other disciplines since 2003.  The deadline to apply is December 6, 2021.

    The Registered Nurses’ Association of Ontario (RNAO) is seeking nominations for expert panel members for an upcoming new edition best practice guideline on Person-And Family-Centred Care, Third Edition.     It has been a challenging 20 months, and primary care teams have been leaders.

    Throughout this pandemic, teams adjusted to new circumstances and continued to provide comprehensive care while extending reach to support their communities. They were leaders in organizing assessment centres, conducting tests, and administering COVID-19 vaccines. They collaborated with their partners to help keep communities safe, while keeping their doors open to provide care to their patients and others who needed it most.

    New date- Practical approaches to polypharmacy: the gentle art of deprescribing

    In this webinar on Dec. 1, we will explore approaches to the issues and problems around polypharmacy and deprescribing in primary care patients. We will discuss approaches to the integration of patients’ goals and priorities with evidence. In the second part of the webinar, we will demonstrate the TAPER tool (Team Approach to Polypharmacy Evaluation and Reduction), an integrated pathway designed for use in a primary care team-based approach as routine “prevention” in primary care patients with polypharmacy.     Just launched- Indigenous cultural safety training

    The Indigenous Primary Health Care Council (IPHCC) is starting a training series called Anishinaabe Mino’ayaawin – People in Good Health; the first course in the series, Foundations of Indigenous Cultural Safety, was created in partnership with the IPHCC’s Indigenous Knowledge Keepers’ Circle, bioethicists, and Indigenous patient navigators from across Ontario. It is grounded in a two-eyed-seeing approach to knowledge. It’s an online, multi-media course that takes about three hours to complete. Enrolment is free for individuals and organizations.

    2022 Family Medicine Summit call for abstracts

    The 2022 Family Medicine Summit (FMS) will include two days of livestream events – January 28 and 29, 2022 – with dozens of additional sessions available on demand to participants until July 31, 2022. New this year, they have reserved space in the agenda for speakers selected from a Call for Abstracts.

    The Call for Abstracts is open exclusively to family medicine residents and early-career family physicians. Selected presenters will receive guidance and support in preparing to deliver their session. While all abstracts are welcome, they’d be especially keen to hear from someone who has been working on implementing Choosing Wisely Canada recommendations within their practice and/or someone who has been involved in establishing their local Primary Care Network. Submissions are due by November 8.

    There are two openings on AFHTO’s IHP Advisory Council, and we invite all AFHTO-member IHPs to apply for a two-year term!

    This council was established in May 2018 to provide input on team-based care matters related to IHP issues; to give advice and input to the AFHTO board, staff, and leadership council; and to foster leadership across the IHP community within team-based models of care.     Research project to help you identify and provide appropriate information to COVID-19 vaccine hesitant patients

    A team of family physicians from the University of Ottawa and the Montfort Hospital has partnered with the Eastern Ontario Health Unit multimedia communications team to help clinicians communicate with patients using digital messages.

    The Public Health Agency of Canada has provided funding for a research project using the Canadian Primary Care Information Network (CPIN), an automated patient engagement system for primary care practices, to enable family physicians and nurse practitioners to send messages about practice or COVID-19 related topics to groups of patients by email or text.     Complimentary online virtual care privacy & security training for clinicians

    OntarioMD (OMD) and the Ontario Medical Association (OMA) have developed a new online Virtual Care Privacy & Security Training Module for Ontario physicians, other clinicians and their support staff with the support of Canada Health Infoway. It’s available in English and French.

    New Ontario business registry launched

    The new Ontario Business Registry launched today offers over 90 online services to businesses and not-for-profit corporations. Teams will need to register with ServiceOntario as part of the new requirements under ONCA. Find out more here.

    Help develop educational materials for lipid management
    Researchers at The Knowledge Translation (KT) Program at St. Michael’s Hospital and Women’s College Hospital are collaborating on a study called the “CANHEART Improving Cholesterol Management Study” to improve lipid management and cardiovascular health in Ontario.

    The Community Heart Outcomes Improvement and Cholesterol Education Study (CHOICES) is an Ontario research study being conducted in certain health regions in Ontario with higher-than-average rates of cardiovascular disease. The study is being funded as part of Canada’s Strategy for Patient-Oriented Research (SPOR) and by the Canadian Institutes of Health Research (CIHR). CHOICES will involve implementing a ‘toolbox’ for cholesterol management to inform and empower people when it comes to the appropriate management of their cholesterol and cardiovascular health.

    They are looking to recruit primary care physicians Ontario wide to review and provide feedback on an online decision aid tool aimed to improve cholesterol management among patients. Participants will be compensated with a $100.00 honorarium for their time.  If you are interested in reviewing the tool and participating in a 30 – 45-minute confidential telephone interview, please contact Research Assistant, Suvabna Theivendrampillai at Suvabna.Theivendrampillai@unityhealth.to.

    Influenza-like Illness (ILI) provincial guidance memo for primary care

    The final approved guidance for primary care on navigating patients with ILI symptoms is now available. This guidance is purposely high-level to allow for adaption and implementation at a regional and local level.

    Reminder- Tapering and Stopping Opioids, Oct. 19

    There is evidence that chronic opioid therapy has a negative impact on patients’ quality of life and increasing the risk of serious complications like opioid use disorder, unintentional overdose, and death. Tapering opioids can be challenging to the prescriber and to the patient, and there is not a lot of guidance on how to do it safely, comfortably, and effectively. In this webinar, Dr. Furlan will summarize the 10 years of experience of the pain clinic at Toronto Rehab UHN where she works with an interprofessional team to help patients come off long term opioid therapy for chronic non-cancer pain.     META: PHI provincial RAAM network

    META: PHI supports clinicians across care settings in delivering high-quality care to people who use substances. In 2020, META: PHI was tasked by the Ministry of Health to provide oversight to the province’s network of 74 rapid access addiction medicine (RAAM) clinics as the provincial clinical practice hub for RAAMs. RAAM clinics offer low-barrier, walk-in addiction medicine services, including time-limited pharmacotherapy and brief counselling, without an appointment or formal referral.     AFHTO partnering with 19 to Zero

    AFHTO has partnered with 19 to Zero to help address vaccine hesitancy and to support healthcare workers. 19 to Zero is a dedicated coalition of academics, public health experts, behavioural economists, and creative professionals working to understand, engage with, and ultimately shift public perceptions around COVID-19 behaviours and vaccination. 19 to Zero uses data- and research-informed approaches to support communities and healthcare workers across Canada.

    They have launched public health campaigns, produced, and disseminated tailored resources, translated fact sheets, developed practice change toolkits, and much more! Teams can check out their tools and get more information here!

    Merging Quorum groups: Primary Care and COVID-19 Support CoP and register for next meeting

    Ontario Health is merging the “COVID@Home Monitoring for Primary Care” and the “Primary Care Vaccination Support QI CoP” into one new community of practice.  The new group, Primary Care  and COVID-19 Community of Practice will provide one location to ask questions, find resources and share lessons learned.

    There was substantial overlap in membership between these two groups and OH recognizes that primary care is being pulled in several directions currently. Merging the CoP’s will provide one location for members to share resources, grab information or ask questions and will allow them to be responsive to memberships overall needs.

    Any existing member of either group is automatically a member. Simply log into Quorum as you normally would to access this group. The next CoP meeting for this merged group is on Wednesday, October 13th, 2021, from 8:00 am to 9:00 am where you will learn about monitoring pediatric patients who have tested positive for COVID-19 and discuss other COVID-19 related issues affecting primary care.
