    Free internet-based cognitive behavioural therapy programs

    The Government of Ontario is offering a variety of free mental health resources that they can access online. This includes internet-based cognitive behavioral therapy (iCBT) programs, such as AbilitiCBT, by Morneau Shepell, and Beacon Digital Therapy by MindBeacon.

    Are teams in scope for the PPE Data Collection/ CSE Survey?

    On Tuesday, June 9 EOC Operations sent an email titled “Important Update on the Minister’s Order and Direction on PPE Data Collection/ CSE Survey”. We sent a copy to all EDs and Administrative Leads on Wednesday, June 10.

    As mentioned in April, teams are encouraged to collaborate with the FHO and FHN to submit a survey together. Multiple sites should be included within one survey. If the FHO/FHN teams can’t collaborate with the FHT then when the FHT submits the survey they should clearly identify that the data included is only for the FHT staff.

    Each location is to track individually, but everything is to be submitted in one document. Use the Excel document and make a tab at the bottom for each site (copy and paste the chart into each tab). One tab should be for the FHT and then each FHO/FHN has its own additional tab.   • Patient and Family Engagement in Primary Care: Case Study Report

    On Thursday, June 11, The Change Foundation released Patient and Family Engagement in Primary Care: Building effective patient and family advisory councils in three Ontario communities,a case study report featuring 3 Family Health Teams. The report finds twenty key lessons in 5 categories- launching, recruiting, operating, relationships and sustaining.

    Teams featured:

    • Peterborough Family Health Team– one of the pioneers of advisory councils. Peterborough is a city of just more than 80,000 people about 125 kilometres northeast of Toronto. Because the FHT also serves surrounding Peterborough County, the team has a total of 111,000 patients on its roster and a budget of $11.2 million. There is an estimated 8,000 residents who don’t have a primary care provider locally; the team has opened two nurse-led clinics to serve them and summer residents from nearby cottage country, with one of the clinics providing virtual care. Efforts to create its patient council have been carefully thought out and focused on balanced representation.
    • St. Michael’s Academic Family Health Team– The team serves 46,000 people in a catchment area that encompasses both the city’s poorest neighbourhoods and some of its wealthiest. Forty per cent of its patients were born outside of Canada and 8% have immigrated in the past 10 years. Just over a third of patients own their homes, 5% are “marginally housed” and 30% live below the low-income cut off. Involving members of vulnerable communities and trying to ensure an equal voice to all have been among the challenges it has worked on.
    • Dufferin Area Family Health Team– just northwest of the Greater Toronto Area. It serves some 48,000 patients, who are spread over several communities including former small towns that are transforming into suburbs — some with a high percentage of visible minorities, others not — as well as rural residents who may struggle to make ends meet. As a result, accurate representation of the population has been a concern.

    Primary Care Virtual Community:

    The primary care virtual community is Ontario’s novel collaborative designed to unleash the power of primary care in health system change. As a community, they have co-defined a list of High Impact Actions that are felt to spark meaningful improvements in primary care, building on lessons learned from the UK’s own transformation efforts. The community has also shared insight on the actions they feel are within their control, in addition to where support is needed. As a community they continue to work collaboratively to refine the list of High Impact Action Items for Ontario. Here is the latest list of 10:

    High Impact Action Items:

    1. Navigate & Coordinate

    • Promote patient navigator role that can enable easier patient/caregiver access to resources including those that go beyond health care resources. This could include linking home care coordination to primary care.

    2. Integrate EMRs

    • Either single or intra-operable EMR/EHR systems that can both push and pull required information. Integration of EMRs can enhance quality of care provided and practice efficiency by promoting easier and better communication.

    3. Access to Mental Health and Addictions

    • Provide mental health and addictions supports in the community, embedding these supports in primary care.

    4. Organizing Primary Care

    • Come together at the local level to organize primary care and move towards more team-based care for more Ontarians, opening up the FHO model for primary care delivery.

    5. Practice Facilitation & Change Management

    • Provide dedicated resources to enable peer -to -peer coaching and uptake by providers. A QI-enabled, evidence-driven health care system, in turn, enables best practices.

    6. Build the Extended Team

    • Identify and provide support to the extended team that wraps around the patient/ family/caregivers and provide supports to enable the team to be high performing.

    7. Streamline & Reduce Administrative Burden

    • Reduce bureaucracy and administrative burden.

    8. Recognize and Support Primary Care Continuity

    • Promote the patient/caregiver and provider relationship, ensuring that the care is provided according to the principles of comprehensiveness and continuity.

    9. Build Connections to Address the Social Determinants of Health

    • Recognize the social determinants of health as factors that influence health and well being outside of access to healthcare services, adopting practices like social prescribing.

    10. Patients as Partners

    • Empower patients to take greater control and ownership over their health.

    Local Change Ideas:

    The Primary Care Virtual Community has given communities the opportunity to identify what’s meaningful to them. What are their pain points, headaches or areas that can be improved? And more importantly what changes can be made at the local level to support providers, their clinical practices and the delivery of patient care. Building on the change ideas identified in the primary care high impact actions for Ontario, this document highlights local challenges and solutions to improve clinical practices and care delivery in your own communities. This document is only the beginning of creating a repository of change ideas. Please complete this survey to provide local change ideas within your community!

    The February 13th and the November 21st Virtual Community focused on Organizing Primary Care – the community learned from a number of primary care leaders about how they are mobilizing the primary care community to self-organize and identify what their needs are. We’re pleased to share their lessons and advice with the release of Primary Care Physicians: Working Together for Change primer which highlights strategies that can be employed for organizing primary care at the local level.

    Upcoming transfers to Ontario Health & LHIN transitional regions
    On Nov. 13, Interim CEO of Ontario Health, Susan Fitzpatrick, sent an update on activities underway with the health agency. Transfer orders were issued by the Honourable Christine Elliott, Deputy Premier and Minister of Health, to Cancer Care Ontario, eHealth Ontario, HealthForceOntario Marketing and Recruitment Agency, Health Shared Services Ontario, and Ontario Health Quality Council operating as Health Quality Ontario.

    The transfer orders list December 2, 2019 as the day they will transfer to Ontario Health. Transition of the LHINs will occur at a later date.

    You can find all of the transfer orders on the ministry’s website and Ontario Health’s new website, www.ontariohealth.ca.

    Ontario Health has also aligned the 14 LHINs to five interim and transitional regions. This is not a merger of the LHIN boundaries. Effective November 13, five LHIN CEOs have been appointed as Transitional Regional Leads reporting to the Interim CEO of Ontario Health. Read the full memo on our site.

    A webcast for IHPs: OHTs and health system transformation slides and video
    On Nov. 14 IHPs joined a webcast that was organised to help answer questions and to learn about the work being done at this time of change. It was an overview of OHTs, an update from the ministry, and an opportunity to hear from members who are part of OHTs that are proceeding to full application, especially those OHTs that have included close collaboration with IHPs. Slides and video are now available on our site.


    Care for survivors of blood or marrow transplant therapy
    Stem cell transplant is an important treatment for some types of cancer, but it can also increase future health risks.  Long-term follow-up care involves all members of the patient’s health team. Patients commonly need updated immunizations and regular monitoring for cardiovascular risk factors and screening for secondary cancers.

    Treatment of Adult Major Depressive Disorder (MDD) Tool
    About 7% of Canadians meet the diagnostic criteria for adult major depressive disorder (MDD). The Centre for Effective Practice (CEP) recently developed the Treatment of Adult Major Depressive Disorder (MDD) Tool to help.

    As a highly requested topic by providers, this tool can support primary care providers in treating adult patients (≥ 18 years) who have major depressive disorder. The tool covers treatment options for MDD, which involve psychotherapy and/or pharmacotherapy, as well as complementary and alternative options. The MDD Tool is one of several clinical tools developed as part of the Knowledge Translation in Primary Care Initiative.

    2019 spotlight on caregivers and a new helpline

    The Change Foundation has released the second annual Spotlight on Ontario’s Caregivers, done for the first time in partnership with The Ontario Caregiver Organization. Highlights include:

    • While similar numbers of caregivers are involved in organizing care, in 2019 56% of caregivers find the process difficult, compared to only 39% in 2018.
    • The impact on finances is more pronounced in 2019 – 32% said they had faced financial hardships compared to 22% in 2018.
    • Emotional counselling for both the patient and the caregiver is still one area where more support is needed, with 77% of caregivers wishing for a ‘one-stop-shop’ that they can turn to for help and advice.

    To address this last point The Ontario Caregiver Organization recently launched the Caregiver Helpline – a 24/7 resource that caregivers can call or engage in a live chat between 7 a.m. and 9 p.m., Monday to Friday, to find local supports, get information about caregiving and get other important knowledge that caregivers said they need.

    A webcast for IHPs: OHTs and health system transformation
    All IHPs are invited to join a webcast that has been organised for them to help answer questions and to learn about the work being done at this time of change. Please join us on Thursday, November 14, from 12 PM to 1:30 PM for an overview of OHTs, and to hear from teams proceeding to full application whose OHT development has included close collaboration with IHPs. They will speak to their work to date and highlight how IHPs are engaged.

    We ask this invitation be distributed across teams. While this webcast is organised for IHPs, all members are welcome to join. Please register here.

    New resources for caregivers supporting seniors living with frailty

    Regional Geriatric Programs of Ontario (RGPO) and six project partners from across the province have launched new educational resources for caregivers who are providing care and support for seniors living with frailty.

    Caregiving Strategies: Providing Care and Support for a Senior Living with Frailty was designed by caregivers and health care experts to improve the skills, knowledge, and confidence of family member and friend caregivers.

    Caregiving Strategies consists of:

    • A FREE online course
    • The Caregiving Strategies Handbook
    • Tools, tips and links to great resources

    New ECHO launching in support of adults with intellectual and developmental disabilities

    In January 2020, ECHO Ontario Adult Intellectual and Developmental Disabilities at CAMH, University of Toronto, and Surrey Place will be launching a new ECHO (i.e. a free educational program). The focus will be on helping healthcare or developmental service professionals in their work with adults with intellectual and developmental disabilities.

    They’re looking for individuals in this area of work who might be able to help them to define a curriculum that would matter most. The responses to our short survey will help create the curriculum for the weekly learning sessions. To complete the survey, click here.     The primary care virtual community, brought together as an AFHTO/OCFP partnership with Change Foundation, will next meet on Sep. 25 from 8:00 a.m.- 9:30 a.m. This session builds on the conversation and feedback you shared in prior sessions to further develop the concept of the 10 High Impact Actions implemented within the UK and what our own made-in Ontario High Impact Actions could be.

    As we come together to share these ideas, we are focused on how the high impact actions can shape all 4 elements of the quadruple aim, and how we improve both the provider and patient experience. Register here.

    Patient digital IAA application deadline extended
    On July 16, we shared the Ministry of Health’s call to submit applications for establishing a public-facing digital identity, authentication and authorization (IAA) service for patients in Ontario’s health system.

    The application deadline has been extended to August 16, 2019 at 12pm EST, and there is now an FAQ.

    The Assessment phase ends in mid-October 2019.

    Project ECHO Ontario Mental Health: free professional development opportunity
    Registration is now open for ECHO Ontario Mental Health at CAMH and the University of Toronto. This free educational program uses videoconferencing technology to support front line providers in the delivery of high quality, evidence-based mental health and addictions care. Together with their peers, as well as an inter-disciplinary specialist hub, ECHO Ontario Mental Health allows participants to share and learn best practices, jointly discuss practical recommendations for complex client care, and build sustainable communities of practice.

    The Ministry of Health has posted the full list of Ontario Health Teams online. A number of our teams are actively involved in the application and in development phase including two in development by members ESF NORD-ASKI and Huron Shores FHT.

    Cancer Care Ontario is sharing the new provincial follow-up care clinical guidance summaries for breast, colorectal, and prostate cancers, now available on their website. The summaries are to serve as concise and user-friendly informational tools for primary care providers to support the delivery of evidence-based follow-up care to their patients.     Free workshop to develop support for patient-oriented research
    Do you know any patient partners, policymakers, managers or researchers who would benefit from learning more about how patient-oriented research can improve the patient experience and patient outcomes? If so, McMaster Health Forum is asking you to encourage them to register for one of their two remaining free, online training sessions:

    • Sept. 9 – Nov. 25, 2019 (sessions on Mondays and Thursdays, 2:30 – 3:30pm)
    • Jan. 6 – Mar. 23, 2020 (sessions on Mondays and Thursdays, 12 – 1pm)

    This is a limited opportunity for them to help bring a strong patient voice to both research and decision-making. Over 11 weeks, this virtual course will better prepare participants to support the conduct and use patient-oriented research in Ontario’s health system and become mentors to others looking to do the same.


    Managing benzodiazepine use in older adults- tool available
    The Centre for Effective Practice (CEP) has published a new clinical tool to support  the assessment and discussion of the use of benzodiazepines with older patients.

    Developed as part of the Knowledge Translation in Primary Care initiative, the Managing Benzodiazepine Use in Older Adults tool focuses on the role of benzodiazepines in insomnia, anxiety disorders and panic disorder, with steps to support safely discontinuing, starting and continuing to prescribe benzodiazepines.

    Patient-centered performance measurement
    “Balancing patient priorities for technical and interactional aspects of care in a measure of primary care quality” – article in Primary Health Care Research & Development, Volume 20 focused on results of a co-designed survey where patients were asked to determine the relative priorities of each of a series of primary care measures in the patients’ relationship with their primary care provider. Authors include Dr. Carol Mulder, former Provincial Lead of the QIDS Program at AFHTO and lecturer at Queen’s University and Dr. Nadiya Sunderji Psychiatrist in Chief, Waypoint Centre for Mental Health and Assistant Professor at University of Toronto.


    Project ECHO Ontario Bariatric Network – update your knowledge on post bariatric care and obesity management
    The Ontario Bariatric Network is launching a new Project ECHO in post bariatric care and obesity this coming fall. A series of six sessions will be hosted by seven Bariatric Centres of Excellence across Ontario. Join one of their knowledge sharing networks with other primary care providers and specialist from the Bariatric Centres of Excellence to share best practices and discuss cases using videoconferencing technology.  Sessions are open to all primary care providers and interprofessional health care providers, are offered at no cost and are eligible for Continuing Medical Education credits.
