Resource Guides and Toolkits

The following Resource Guides have been designed to help FHTs meet their goals in providing high-quality, patient-centered care.

The Needs Assessment Resource Guide is designed to provide an overview, with a focus on needs assessment approaches and processes relevant to the FHT and CHC practice reality. The Logic Model Resource Guide is intended for health professionals working in FHTs and CHCs and describes how logic models (diagrams that describe a program and its components) are used in program planning and evaluation. The Community Partnerships Resource Guide will be invaluable in helping FHTs and CHCs establish effective partnerships with other organizations. The Program Evaluation Resource Guide will enable teams to assess their programs to help ensure effectiveness and success.
