Concurrent Session Selection

Concurrent session presentations were chosen by working groups consisting of AFHTO members across Ontario, representing the full breadth of professions within collaborative primary care. Submissions were chosen for reflecting the conference theme, usefulness/applicability to interprofessional primary care teams, innovativeness, evidence of impact, and clear learning objectives.

NEW privacy breach reporting rules effective Oct. 1 and NEW guidelines to support you in meeting them
Effective October 1, 2017, health information custodians (HICs) will be required to report certain privacy breaches to the Information and Privacy Commissioner (IPC). The IPC has published new guidelines to support you in complying with these new rules. Please read them here and distribute them widely to all members of your team. The new guidelines outline the situations in which you must notify the commissioner of a privacy breach, including:
- Use or disclosure of personal health information without authority
- Stolen health information
- Further unauthorized use or disclosure of health information after a breach
- Pattern of similar breaches
- Disciplinary action against an employee or agent of a custodian
- Significant breaches that do not fall into one of the above categories
Effective January 1, 2018, HICs will also be required to start tracking privacy breach statistics, and they will be required to provide the IPC with an annual report of the previous calendar year’s statistics beginning in March 2019. We will update you when the IPC releases detailed guidance on this statistical reporting requirement in the coming months. We are currently updating our privacy resources for AFHTO members, to further support you in complying with the new regulations. In the meantime, you may wish to review these two privacy tools developed for AFHTO members:
- From June 2017: Protecting Personal Health Information, presented by Fida Hindi from the Office of the Information and Privacy Commissioner.
- From February 2016: For Privacy’s Sake, presented by Kate Dewhirst, lawyer specializing in Ontario health law.

FREE Governance Webinar July 11: Strategies for Building and Maintaining an Effective Board in Primary Care
AFHTO has recently partnered with the Governance Centre of Excellence, to offer our members a series of webcast and online learning modules to better serve your governance needs and empower board leadership. Please join us for the first webcast on Tuesday, July 11^th from 12:00-1:30pm: Strategies for Building and Maintaining an Effective Board in Primary Care (see brochure for agenda overview). This interactive discussion will focus on strategies for building a skills-based board, governance insights from the Ministry of Health and Long-Term Care, and examples from the field on strengthening governance practices.

Program Planning Workshop
Beyond Schedule A: Evidence-Based Program Planning for Community Needs

Materials:
How can we use the principles of evidence-based medicine to develop programs that better serve our communities? Join us on Tuesday, October 24^th from 5:00-8:30pm as we explore the program-planning process from situational/needs assessment to determining your program elements. Along the way, we’ll pay special attention to the process of setting appropriate, realistic goals and objectives. We’ll also get practical, looking at how to apply these principles when completing the Schedule A template. You’ll come out of this interactive workshop with the tools and confidence you need to make program planning easier for you, more meaningful for your team, and more effective for your patients. Developed and led by Allison Meserve, Health Promotion Consultant, Planning and Evaluation at Public Health Ontario, it builds on AFHTO’s Program Planning and Evaluation Framework. To ensure attendees get the most out of it, everyone who registers by September 15 will receive a needs assessment to complete about a month before the conference

D2D 4.1 is now live: Check out the interactive display to see how your team is doing, and see a summary of the results here! We’re looking forward to seeing you at our post-launch webinar on March 28^th. In the meantime, if you have any questions about the platform or your results please contact Greg Mitchell. We’re working with patients to make D2D better: In the next few weeks we’ll be sending a survey to patients across Canada to help us understand what’s really important to them in primary care. Watch for an email with more information and consider sharing it with your own patient networks. Help for your QIP from Cancer Care Ontario: This toolkit will help you add cervical and colorectal cancer screening to your QIP and develop programs to improve care. Choosing program-level indicators is getting easier: Check out the refreshed and easier-to-use Program Performance Measures Catalogue to see what indicators your fellow AFHTO members are using and which ones might be useful for you. AFHTO is inviting patients, QIDS Specialists, and IHPs to join us on Tuesday, March 7 for a one-day learning workshop about using physical activity – the most vital of signs – to help patients achieve better health outcomes. You were (always) right about “same/next day appointment”: It is NOT the only thing that matters to patients! Consider this resource to help you talk about access in a more meaningfully way in your QIP. Struggling with getting Diabetes data from your EMR?
