Re-use and Recycle your EMR data: Your conference lanyard said it best! Use queries developed and tested by QIDSS to find patients you can do good things for related to opioid use (which may signal pain or addictions management challenges) and chronic disease (COPD, CHF, diabetes, depression). Providers across the province (i.e., outside of AFHTO) are starting to use these queries too, thanks to the work of the QIDSS and the Algorithm Project team. And they’re all free!

Help for your patients with pain and addictions. The opioid query is a good place to start if you want to find out who exactly these patients are. Once you’ve found them in your EMR, you can do more to help them help themselves, using these tools from the eHealth Centre of Excellence, the Centre for Effective Practice, the OCFP, and CAMH. More coming soon from AFHTO and CAMH! A little help from your (EMR) friends: the EMR Communities of Practice met in person at the AFHTO conference. They’re coming out of it with more energy and focus than ever. Join them to share your questions and answers and make friends on your EMR journey. More meaningful program measurement: Use the Program Performance Measures Catalogue  to find indicators that your peers are using to tell the story of how well their teams are doing. Working with your IHPs and using the catalogue will make program planning and evaluation easier and more meaningful for the whole team. What makes AFHTO’s bright light shine? You do! Thanks to the nearly 90 teams who completed the team profile survey as part of D2D. Now we’ve got lots of data to help us understand how teams work together and how that affects the quality of care they provide. If you want to learn more about how well you collaborate in your team (or even just at one site of your team) it’s not too late ! Connect with our research partners to share your data about team functioning. As with D2D, you’ll get to see your results and how you compare with others. Learn more about medication reconciliation and other team sports: The Managing Medication as a Team all-day almost-for-free workshop is not just for pharmacists or prescribers. We’re putting together a webinar to showcase ways you can use the data you worked so hard on. Watch this space for more information and registration link! Help to convert your information to action: Do your own assessment to see how ready your team is to commit to (and get support for) reaching a few specific quality goals. Who IS that unmasked team?  In D2D 5.0, you will be able to see how individual teams are performing on each indicator– and then call them up to find out how they got there. Help available to move from Information to Action! You’ve gathered a lot of data. You know what you need (and want) to move on. Learn more about some supports that might help you move from your Information to Action and do a self-check to see how ready your team is to sign up for these supports    More help available: seniors with treatment resistant depression Ask your social workers and psychologists (or any other clinicians) to check out Optimum.  It’s a cross between a research study and a service to help patients and providers deal with depression in seniors that just isn’t responding to care.  It’s free and offered through the same people at CAMH who brought you the really useful STOP smoking cessation program. Help Wanted: AFHTO needs clinicians who are into using data and IT to improve care. If that’s you, please apply online by August 15 to join the EMR Data Management (EMR DM) Subcommittee. It’s almost time to “do” D2D again! D2D is about continuous measurement for continuous improvement. Once is not enough. Think about starting conversations about D2D 5.0 now and begin compiling your data. The Data Dictionary, Step-by-Step Guide, and Data Input Toolkit have all been updated for D2D 5.0. Speaking of continuous improvement, find out what’s new with D2D 5.0! Check out these posts on follow-up after hospitalization and individualized HbA1C targets. Find information about how and why you might want to  “unmask” your data to your peers, complete a team profile questionnaire, and submit site or FHO-specific data. Want to know the trick to improvement? Who doesn’t??! Please consider volunteering to be part of AFHTO’s QI enablers study to help us all understand what makes it easier for teams to improve. Save the date: Interprofessional Medication Management workshop. Mark your calendar for November 17^th, 2017 for a full-day session showing how everyone (clinicians, QIDSS, patients) can contribute to managing (and maybe even eliminating the need for) medications. It’s for all team members —not just prescribers and pharmacists. Watch this space for more information. Quarterly Reporting got you down? You may have to live with that for this year but next year, think about using the Program Performance Measures Catalogue. Patients can (and want to) play a role in better follow-up care after hospitalization. Now there’s a tool for that! The Patient-Oriented Discharge Summary (PODS) is tried, tested and FREE. It’s a simple form YOU can use to get hospitals, patients and your team literally on the same page when it comes to follow-up after discharge. Patients also can (and want to) tell us what matters to them. Need help getting the conversation started? Check out  HQO’s new Patient Engagement Tools & Resources. Let us know if you’d like to partner with us to host a focus group for patients about how to do a better job of reflecting quality in primary care. Start compiling your D2D 5.0 data now! Use our updated online Data Dictionary, Data Input Toolkit and Step-by-Step Guide. And if you’ve signed up for your Team-Level Primary Care Practice Report, you can find it here. If you haven’t signed up, you can still get a report in late August if you sign up by June 30^th. Updates to previously-announced changes for 5.0. In response to member feedback, and to take advantage of late-breaking chances to get help from outside experts, some of the changes we told you about earlier are being deferred. Check here for more information. Do you want D2D data at a site or FHO level? You can now add data at the sub-team level (i.e., for individual FHOs, sites, etc.) in your team’s D2D submission. This will help you compare sub-groups within your team. For more details, check out the Step-by-Step Guide. Ready to take off your mask? Lots of you have told us you’re happy to unmask yourselves to your peers in D2D 5.0. Your board may want to weigh in on this. Consider setting up that conversation now, to have their decision in hand by September. Like everything else in D2D, this is completely optional.
