Looking for fresh ideas about interprofessional approaches to physical fitness or diabetes care? Consider joining one of our interprofessional communities of practice. CFPC Advocates for better access to EMR Data for Quality Improvement: AFHTO members know the value of EMR data for supporting practice-level QI initiatives in primary care. The College of Family Physicians of Canada (CFPC) agrees. They’ve issued a statement calling on EMR vendors to make this data more accessible, so practices can measure and improve their performance and understand population health. Start spreading the news: D2D 5.1 is right around the corner! Submission period runs from January 9 February 8, 2018. If you’re planning to participate, you can start getting ready now by getting your board on board, accessing the relevant reports, and running the EMR queries. A physician-level My Practice report from HQO is out now, and the team-level report comes out in late December. You can access them both here. And don’t forget to reach out to a peer at another team and encourage them to be a part of it too. Contact us if you need help getting ready to “do” D2D. Will your team be even better next year?
