  • Data to Decisions eBulletin #10 – April 30, 2015

    Contributing to D2D 2.0 Over half of AFHTO members have signed up for D2D 2.0! One hundred and five AFHTO member organizations (56%) have signed up to submit data for D2D 2.0!  The most common reason teams give for being part of D2D 2.0 is to be able to compare with peers to better focus their local QI efforts. If you are still deciding whether to contribute data, check out this short video for a family doctor’s perspective about why D2D matters to her. Even if you missed the deadline for requesting ICES data you can still sign up to submit D2D data before the May 28, 2015 deadline.  Check out the data dictionary or any of the other resources on the D2D webpage or contact Carol Mulder for more details. Detailed data submission “cheat sheet” coming soon Please check out the D2D webpage next week for a “cheat sheet” with step by step instructions for compiling and submitting data. Included will be directions on how to access EMR queries for the two EMR-based indicators (i.e. childhood immunization and EMR data quality). The QIDSS are finalizing those queries and will be ready to share them with all members within the next week. The deadline for all D2D 2.0 data submission is May 28, 2015. Timelines and distribution for D2D 2.0 The D2D 2.0 report will be available to AFHTO members only mid June 2015. Unlike QIP, Schedule A and other MOHLTC reports, D2D 2.0 is by and for AFHTO members only.  A membership-wide aggregate summary of the report will be shared with external partners to demonstrate the maturity and leadership of AFHTO members in advancing QI and measurement –the team-level performance will only be available to teams themselves. Using data to improve data quality and care Cancer Care Ontario (CCO) moving forward on team-level cancer screening reports CCO is creating a team-level (vs individual doctor-level) screening activity report. The report is being designed in direct response to requests from and in collaboration with the QIDSS. It won’t be ready for D2D 2.0 – but it’s coming! Partnering with patients on measurement works! Early results from the patient-doctor relationship survey highlight the value of partnering with patients about measurement in primary care. At least three-quarters of the 200+ responses came via Patients Canada – a reach we would not have had without their active partnership. So far we’ve learned that over half of the respondents want a 50-50 decision making partnership with their doctors. That might not be true of all patients, as Patients Canada will be sharing at an upcoming conference on “Managing the Canadian Healthcare Strategy” – YET it is certainly an observation well worth considering!  Analysis continues and will be shared in subsequent updates. Congratulations to Hope Latam (QIDSS) and Dr. Kevin Samson who are presenting from the East Wellington FHT at the upcoming TELUS Health Ontario EMR User Conference. Their presentation illustrates useful ways of improving access data to EMR data, including the use of the custom queries that were developed as part of an AFHTO Innovation Project and are now being deployed to FHT’s across the province.     Contributing to D2D 2.0

    REMINDER: The deadline to request ICES data is April 21, 2015. A signed version of the form is required. Please scan and email to improve@afhto.ca or fax to (416) 920-6556 attention Denise Pinto. Requesting this data does not mean you are automatically contributing to D2D 2.0.  It just means you will have it on file, in case you DO decide to contribute later. For more information about ICES data click here. Sign up for D2D 2.0: Regardless of whether you request ICES data, please sign up for D2D 2.0 if you are intending to contribute ANY data. This will ensure that we can support you through the data submission process. On that note, there are now more teams signed up than for D2D 1.0 – nearly 36% of AFHTO members and counting! For more information contact Carol Mulder. NPLC Participation in D2D 2.0: NPLCs will be able to submit data for any indicators that are NOT based on OHIP billing (ICES data). AFHTO is working with the Association of Ontario Health Centres (AOHC) and ICES to make billing data-based indicators more available for NPLCs for future iterations of D2D. For D2D 2.0 please check out the data dictionary for the indicators that you are able to submit.     Using data to improve data quality and care

    AFHTO puts patients first in measuring performance: The nation-wide Patient-Doctor Relationship Survey is now complete with a GREAT response from over 200 patients! The results will be used to calculate the quality “roll-up” indicator in the D2D 2.0 report. Contact Puja Ahluwalia for more details. Build a COPD registry: Interest in the EMR query built by your QIDSS to generate a COPD registry is growing. The query is currently available for Telus PS and Accuro EMRs and an OSCAR query will be released shortly. Steps on how to use the query can be found here. Want to know more? Contact Greg Mitchell.

    Contributing to D2D 2.0

    The deadline to request ICES data is April 21, 2015. A detailed timeline for D2D 2.0 implementation is available on the AFHTO website along with the D2D 2.0 data elements and data dictionary. The most important next steps are:

    1. Sign up to contribute data to D2D 2.0 and create a codename to retrieve data from the report once it’s live.
    2. Schedule meetings with your Board and/or physicians to get the necessary permissions to request ICES data by April 21, 2015.  You will receive your ICES data by May 17th. Note: A signed version of the form is required.  Please scan and email to AFHTO or fax to (416) 920 6556 attention Denise Pinto.
    3. The deadline to submit data from all sources via the D2D 2.0 submission platform is May 28, 2015. Note for NPLCs: we are developing a submission plan so stay tuned!

    The Patient-Doctor Relationship Survey is live – have your say! This patient survey measures what’s important to patients in their relationship with their doctor and results will be a used to inform D2D 2.0. It’s being administered to patients nation-wide. We encourage you to complete the survey yourself, as a patient, and share and/or tweet the following to your patients and peers:

    Using D2D 1.0 to improve data quality and care

    Does you EMR need a spring tune-up? Consider hiring a student to clean up the data in your EMR. The hire a student toolkit will help you recruit a student, write a job description, and conduct an interview.  In addition to the toolkit, AFHTO would like to support members by offering to host orientation sessions for their students. To help us gauge the need, please complete this survey to tell us what type of session would be useful. For more information contact Greg Mitchell.    Strengthening the connection between primary care and the cancer system The regional sessions with the QIDSS hosted by Cancer Care Ontario (CCO) are bearing fruit! In the South East LHIN the CCO primary care cancer lead will be collaborating with local QIDSSs on a regional QI project to collectively reach under-screened patients using the screening activity report (SAR) and other relevant measures. The regional sessions continue in Thunder Bay in May.

    Investment in primary care pays off! In a recent Hamilton Spectator editorial, managing editor Howard Elliott stated that family health teams “are a more holistic approach to primary care” and made the case for continued government support of the family health team model. AFHTO’S response “Investment in primary care lowers costs” quickly became the most popular letter of the day. This is why AFHTO members are working hard to advance measurement and improvement in primary care, with the objective of optimizing quality, access and total health system cost of care for patients.     Deadlines to collect data and submit information have been set: In response to feedback from members burdened with year-end pressures, deadlines have been set for the end of April. Next steps should be started now with your team:

    Additional details and the timeline for D2D 2.0 implementation are available here. D2D 2.0 indicators: Following input from members and the Indicators Working Group the list of indicators and data elements are now available online. The Diabetes and FTE measure indicators are being deferred to later iterations of D2D. See the data dictionary for more details on the indicators included in D2D 2.0. D2D video coming soon to a screen near you: Production is underway to produce a short 2-3 minute video explaining what D2D is and how it can benefit your team.  Teams can use it with staff, physicians, boards and other stakeholders to inform discussions about contributing to D2D 2.0.  Contact Carol Mulder for more information. Patients Canada and AFHTO launch the patient-doctor partnership survey:  Finishing touches are being put on a survey that will go out over Patients Canada’s network of patients later in March.  The survey will find out what’s most important to patients in their relationship with their family doctor. The results will be used in the upcoming D2D 2.0 report to create a “roll-up” indicator of quality that reflects the strength of the patient-doctor partnership.  This is a big step forward in patient-centered performance measurement.  Contact Puja Ahluwalia for more details.

    Using D2D 1.0 to improve data quality and care

    Hire a student to clean data in your EMR: The toolkit to assist members in hiring a student now includes detail on recruiting a student including relevant placement programs, sample job description and sample interview questions. Get started on a COPD registry: Teams interested in generating a list of patients with COPD can get started with a standardized EMR query built by the QIDSS.  The query is currently available for Telus PS and Accuro EMRs.  It isn’t perfect — about 15% of the patients found might not actually have COPD.  However, teams might find it is easier to start with this rather than try to come up with a list from scratch.  Click here for more detailed instructions on how to use the standardized query to get started with building a COPD registry with your team.  Volunteer to be part of the Patient Contact System – Pilot Project:  We are nearly at the 50-team mark for volunteers to pilot this exciting new way to connect with patients! Get easier access to your cancer screening reports: One of the outcomes of the regional sessions Cancer Care Ontario (CCO) has been hosting with QIDSS are tips to make it easier to access cancer screening activity reports (SAR).  As a first step, QIDSS are working to streamline the permissions process (e.g. OneID and delegate status) to help doctors more easily get current cancer screening data.  Contact your QIDSS or Carol Mulder for more information.  The regional sessions continue in Thunder Bay in May.

    Tips from HQO for submitting your QIP in the Navigator: Quality Improvement Plans (QIPs) are due by April 1, 2015. HQO has provided the following tips:

    • At any time, you can test the submission of your QIP in order to see if any information is missing. In order to test the submission of your QIP, click the SUBMIT button – this will generate a detailed list of omissions that you can print. If you get to the sign-off window, it’s a sign that you are able to submit your QIP successfully (if you are not ready to submit, you can simply close the sign-off window).
    • There is no need to send a signed copy of the QIP to HQO. During the submission process you will be asked to include the names of those accountable on the QIP (this is considered sign-off approval). After submission you can export all three components of the QIP, format as desired, print, sign and post.

    Contributing to D2D 2.0 

    Indicator selection: The indicators for D2D 2.0 have been approved by the AFHTO Board. Please review to decide which indicators your team could and would submit to the report. You may want schedule meetings with your clinical lead(s) and/or Board of Directors to review the data and approve it for submission. The deadlines for data submission are projected for early April – dates will be confirmed shortly. Data submission tool: For D2D 2.0 teams will enter their data directly into a tool that is now being developed (instead of sending in an excel file). Teams participating in D2D 2.0 will be asked to designate one individual from their team with authority to submit data. Several QIDSS will be testing the tool for usability starting on March 19th.     Using D2D 1.0 to improve data quality and care

    Hire a student to improve data quality in your EMR: The toolkit to assist members in hiring a student has been posted on the members-only website. The toolkit was developed with input from members that have successfully engaged students to improve data quality in their EMRs. If you are considering getting a student, particularly if it is for this summer, it is important to start the process now. See the toolkit for next steps. Build a COPD registry in Telus PSS and Accuro EMRs: A query will soon be available from the QIDSS to generate a list of COPD patients. The QIDSS have developed the query in collaboration with CPCSSN, EMRALD and the ALIVE project. It is not perfect – about 15% of the patients found might not actually have COPD – but this may be an easier way to start finding these patients than starting from scratch. The query and instructions will be released in the next eBulletin. In the meantime, contact Greg Mitchell for more information. Pilot project for a patient contact system (for patient experience surveys etc): Proposals are under review for a vendor to develop and implement a service to automate the process of contacting patients.We will be looking for 10 teams to pilot the service by March 31, 2015.     HQO practice-level indicator prioritization: HQO is about to launch its consultation process to prioritize the practice-level indicators that were identified as part of the Primary Care Performance Measurement Framework. AFHTO staff are involved in the process, which includes consideration of the D2D process. AFHTO members may also be approached directly for their feedback. The sessions are intended for board members, executive directors and quality improvement leaders in AFHTO’s member organizations.     Using D2D 1.0 to improve data quality and care

    Improving quality of clinical data in EMRs: Cancer Care Ontario (CCO) hosted the first of four or five regional sessions this week with QIDSS and the CCO Regional Primary Care Leads, supporting improvements in the quality of cancer screening data in EMRs and the progress in cancer screening efforts. Next week, QIDSS and leaders from CPCSSN and EMRALD are joining forces to extend the logic from CPCSSN/EMRALD case definitions to queries that AFHTO members can use in their own EMRs, even if they are not part of CPCSSN or EMRALD. Contact Carol Mulder for more information about either initiative. Automating patient experience surveys: AFHTO is inviting quotes for a service for members that will take much of the work out of administering patient experience surveys, while at the same time will increase the consistency of both the content and process for the surveys.  Please see the Request For Quotes on the web site for more details. Tool kit to help you recruit summer students: A tool kit that will assist AFHTO members on recruiting summer students for EMR data cleaning projects will be available in the next few weeks on the AFHTO members-only website. It is based on input from QIDSS and AFHTO members who have had success with this in the past.

    Success!! Accessing clinical data in PSS: Almost overnight, nearly 50 AFHTO members have taken up a new tool to extract data from any custom forms in PS, something that was not possible for teams to do previously. The tool emerged from the Innovation project developed by the East Wellington FHT and has spread like wildfire via the QIDSS.  As a result, Dr. Kevin Samson and Hope Latam (QIDSS) have been invited to present their work on this to the national Practice Solutions (PS) forum this spring. Physician Leadership Council: AFHTO is pleased to announce the inaugural meeting of the Physician Leadership Council (PLC) that took place on January 28th. The newly created PLC plays a significant role in enabling physician leaders from across the province to provide a sounding board on FHT-related matters and give advice to the AFHTO board and staff as needed. For the first time physician leaders now have a platform to create a collective impact that can optimize FHT value, function and operations.     Contributing to D2D 2.0

    LAST DAY REMINDER!! Complete the indicator selection survey by Jan 23, 2015: Please review the instructions for working with your team to provide input about which indicators should be included in D2D 2.0. QIDS steering committee meeting Feb 10, 2015: The committee will decide on the indicators to be included in D2D 2.0 from the results of the survey, among other things. Construction underway for D2D 2.0 data submission and presentation tool: A small team of QIDSS and AFHTO staff are working with a technical service provider to develop a user-friendly sustainable tool for data submission and presentation.  Contact Puja Ahluwalia or Greg Mitchell for more information.

    Using D2D 1.0 to improve data quality and care

    Improving quality of clinical data in EMRs: Over the next 2 months, QIDSS are participating in two initiatives to advance their collective efforts to increase consistency of clinical data capture and/or extraction.

    1. The first involves a small team of QIDSS and leaders from CPCSSN and EMRALD collaborating to expand and implement the work of the algorithm team which has been attempting to extend the logic from the CPCSSN/EMRALD case definitions to EMR queries.
    2. The second initiative is a series of regional sessions sponsored by CCO.  The goal is to facilitate collaboration between QIDSS and their respective CCO primary care leads in improving the quality of cancer screening data in EMRs and support improvements in cancer screening efforts.

    Leadership opportunities: Thanks for volunteering!!  AFHTO is in the process of connecting with those who will be filling the vacancies in the Physician Leadership council, the QIDS steering committee, Indicators Working Group, the D2D advisory panel and the EMR Data Management subcommittee.  Thanks in advance for your time and commitment. Patient-doctor partnership composite indicator development: Work continues with Patients Canada, QIDSS and others to develop a tool to measure the patient-doctor partnership.  The tool is intended to be available to interested parties to try possibly by March 2015. See project outline or contact Puja Ahluwalia for more details 7 Innovation Projects to inspire you: Check out the findings from the innovation projects funded by the QIDS program for ideas that are already spreading across the membership to improve capacity for measurement and improvement.

Preliminary data dictionary for D2D 2.0: Please check out the preliminary data dictionary describing potential indicators for D2D 2.0.  It is now posted on the D2D web page.  Indicators Working Group meeting Jan 13, 2015: The group will discuss D2D report design and communication strategies among other things.
