    Inequity in online renewal for OHIP

    Inequities in Ontario’s online health card renewal system must be addressed so marginalized people are not left behind.

    The Alliance for Healthier Communities (Alliance), the Association of Family Health Teams of Ontario (AFHTO), the Indigenous Primary Health Care Council (IPHCC), and the Nurse Practitioner-led Clinic Association (NPLCA) call on the government to address the inequities in the online health card renewal system.

    “The ability to renew the Ontario health card online is only available to people who hold a driver’s licence. This needs to change to include those with an Ontario photo card,” says Kavita Mehta, CEO of AFHTO. “There are a number of people who do not or cannot drive, including those with medical conditions. Like those with a licence, they need the same ability to renew online.” Read the full statement here.

    Seeking input on psychological safety factors

    In collaboration with the Mental Health Commission of Canada, researchers at the Centre for Studies in Primary Care at Queen’s University and HEC Montreal are inviting health-care workers and leaders to participate in research to identify barriers and facilitators to accessing support for psychological self-care and protection from moral distress.

    Together We Care 2022 Virtual Conference: call for content

    The Ontario Long Term Care Association (OLTCA) in partnership with the Ontario Retirement Communities Association (ORCA) will host an online experience for Together We Care (TWC), Monday, April 4 – Wednesday, April 6, 2022. The call for content is open until Dec. 20.

    The BETTER Program: building on existing tools to improve chronic disease prevention and screening in primary care

    Teams are invited to learn more about The BETTER Program, which aims to improve chronic disease prevention and screening in primary care settings through a comprehensive, individualized approach provided by a Prevention PractitionerTM, which a regulated healthcare provider on a team can train to be through this program.

    It is an innovative, evidence-based program that focuses on prevention and screening of cancer (breast, cervical, colorectal), heart disease, and diabetes. It also addresses lifestyle risk factors, such as physical activity, smoking, alcohol, and diet. The program was developed by primary care providers for primary care providers.

    OHT supports events calendar launched

    The Ministry of Health, in partnership with the Rapid Improvement Support and Exchange (RISE) team, and partners in the OHT Central Program of Supports have launched the OHT Supports Events Calendar.
     
    The calendar now offers one central online location for teams at all stages of implementation to easily find information on learning and engagement events offered through the OHT Central Program of Supports and other health-system partners (e.g., webinars, information sessions). You can sign up directly for bi-monthly updates on upcoming events and access resources from past events.


    RNAO seeking best practice panelists-deadline extended

    The Registered Nurses’ Association of Ontario (RNAO) is seeking nominations for expert panel members for an upcoming new edition best practice guideline on Person-And Family-Centred Care, Third Edition. They are looking for 1 to 3 family physicians to be a volunteer on this panel which extends over 18 months.

    Resources and Tools for the Rollout of Vaccines for 5–11-Year-Olds

    Dear Members,

    As you know, Health Canada approved the Comirnaty (Pfizer-BioNTech) COVID-19 vaccine for children aged 5 to 11 years old last Friday. The Government of Ontario opened booking beginning today, November 23. Ontario is expected to receive 1,076,000 doses of the vaccine from the federal government for the approximately one million eligible children, which will be distributed to public health units, pharmacies, and primary care settings across the province. If you are not aware of your PHU’s plans or would like to participate in the vaccine efforts, please contact your PHU directly.

    Below are some resources you may find helpful in either planning for vaccination or helping to address questions parents/caregivers may have about vaccinating their kids.

    University of Toronto DFCM and OCFP Free Learning Modules

    Free learning module: Vaccination in children age 5-11
    DFCM and OCFP have developed a free learning module on vaccination in children aged 5-11 designed to support primary care clinicians as they begin discussing vaccination of children with patients and their families.

    The vaccination in children module includes:

    Update on the 2021/22 and 2022/23 Quality Improvement Plans (QIPs)

    On March 10, 2021, Ontario Health communicated that an adjusted strategy for the 2021/22 QIP would be forthcoming, designed to provide organizations with flexibility to reflect where they are with their QI efforts given the pandemic, local needs, pressures, and goals for the recovery period.
     
    After careful consideration, they have made the decision to not reopen Navigator for the 2021/22 QIP submission. You are not required to submit your 2021/22 QIP to Ontario Health. They do, however, encourage you to develop and post your 2021/22 QIP on your website and to share it with your staff, clinicians, and patients/clients and their family members. Please reach out to the team at QIP@OntarioHealth.ca if you have any questions about this. You can read the full email in English and French here.

    AFHTO 2021 Conference Poster Highlights

    At the AFHTO 2021 Conference, we hosted an exhibit of numerous posters highlighting evidence-based, impactful innovations by your colleagues. We encourage you to take a moment to check out these posters and reach out to teams to see how you can implement these innovations too!

    Given November is Diabetes Awareness Month we would like to highlight a few innovations you can implement in your teams to improve diabetes care, all poster innovations presented at the AFHTO 2021 conference.

    1) Proactive Diabetes Care

    Proactive Diabetes Care poster

    COVID-19 disrupted the continuity of care for patients and propagated deterioration of chronic conditions such as diabetes. Credit Valley Family Health Team (CVFHT) proactively assessed the impact of the pandemic on their diabetes patient care using a triangulated approach comprised of a clinic survey, EMR query, and a patient survey. CVFHT prioritized the needs, creating a Just in Time intervention which was a proactive virtual educational strategy designed to reduce deterioration of diabetes outcomes, foster patient engagement and improve patient self-management. The 2-part education intervention utilized a team-based approach and was grounded in principles of learning and behavioural psychology with the goal of promoting behaviour change.     2) Implement Self Management Tools for Patients

    Implement Self Management Tools for Patients poster

    St. Joseph’s Urban Family Health Team’s initiative aims to improve the glycemic control of those with type 2 diabetes, reducing the risk of long-term complications. Their aim is to ensure all patients eligible, are offered flash glucose monitoring to monitor their glucose and provide them with more insight over lifestyle factors impacting their glycemic control. Their aim is that if this monitoring system does improve indices, that all patients requiring insulin therapy are offered this as an option, to improve their quality of life and reduce the risk of long-term complications.     3) Dynamic Digital Engagement

    Mount Forest Family Health Team, in partnership with IMA Medical Group, demonstrated how Engagement and Success can be tracked differently within a virtual care model. They also included how engagement over time can be leveraged to build more personalized success experiences that enhance their ability to guide themselves from pre-contemplation to action.     4) Updating EMR tools for Accessible and Trackable Data

    Updating EMR tools for Accessible and Trackable Data poster

    Wound documentation using a validated tracking tool in combination with proper wound care product selection is a best practice. When Elliot Lake Family Health Team heard from their patients, “Patients really want to know how long it will take to heal their wound” they implemented a tool within their electronic medical record (EMR) to give patients accessible and trackable data. Elliot Lake Family Health Team was able to track wounds and able to predict which wounds will heal and which wounds will be hard to heal.     Your Role In Improving The Population Health Of People At Risk Or Living With Diabetic Foot Complications

    Earlier this year, AFHTO, in collaboration with Wounds Canada hosted a webinar to provide an overview of your role to improve the population health of people at risk or living with diabetic foot complications across the full continuum of care settings to improve patient experience, outcomes and costs.

    Query Supports: Identify your Target Population
    MyPractice: Primary Care Report provides an update on the HbA1c Indicator: % of patients with diabetes who had 2 or more HbA1c tests within the past 12 months, approximately every 6 months. In order to assist you with retrieving data in real time from your EMR, the QI in PC Council has created a number of queries which can be found here. Please note, these queries will have to be adjusted per team, but they provide a reference point. In addition, eCE has created a Diabetes Manager.

    On November 18th the QIPC council shared a process that worked well for London FHT that collaborated with ‘Partnering for Quality’ to develop and create comprehensive searches for guiding data supporting quality improvement initiatives on Diabetes, Congestive Heart Failure, and Chronic Obstructive Pulmonary Disease. Using the foundational approach of identifying suspects for coding patients, to then selecting targets to improve the patient level of care, and displaying the data in PowerBI analytical software, made quarterly reporting easier and initiated further discussion around what targets to select next.

In Ontario, about one in ten people have some form of diabetes. See below for some upcoming events and resources that can support you in your practice related to care for diabetes:

    MyPractice Primary Care A tailored report for quality care

    Webinar | MyPractice: Primary Care Report: Using Data for Improvement | Monday November 22, 2021 • 12–1 p.m. Ontario Health is hosting a webinar for primary care providers on using the diabetes management indicators in the MyPractice: Primary Care report to drive improvements in patient care. Please register here to join the session. This is the first of an ongoing series that will focus on using data from the MyPractice: Primary Care report for quality improvement.
    This program is part of a series that has been certified for up to 4 Mainpro+ credits.
     
    Quality Standards
    To address gaps in the health care that people with diabetes receive across Ontario, and improve their health outcomes, Ontario Health developed three quality standards:

    • The Type 1 Diabetes quality standard addresses care for people of any age with a diagnosis of type 1 diabetes.
    • The Prediabetes and Type 2 Diabetes quality standard addresses care for children and adults who are at risk of developing prediabetes or type 2 diabetes or who already have a diagnosis of either. It includes the assessment, diagnosis, and management of prediabetes and type 2 diabetes.
    • The Diabetes in Pregnancy quality standard addresses care for people with type 1 and type 2 diabetes who become pregnant and people diagnosed with gestational diabetes. It includes preconception care as well as management of diabetes during pregnancy.

    Included with each quality standard is a patient guide you can use to support your conversations when discussing diabetes care with your patients. Stay tuned for information on upcoming webinars related to diabetes.

    Choosing Wisely Talks on December 1 at 12 p.m. ET will explore efforts to work with EMR vendors to embed antibiotic stewardship tools into their digital workflows. Dr. Darren Larsen, family physician and expert in digital health, will share how leveraging EMRs can not only help improve patient care, but also help change clinician behaviour.

    According to research published in in the Canadian Medical Association Journal (CMAJ), most family physicians now report that they engage in some degree of social intervention in the management of patients.     Workplace violence during care transition toolkit

    PSHSA has released the new Violence, Aggression and Responsive Behaviours (VARB) toolkit: Workplace Violence During Care Transition. This free toolkit provides information on sound practices for planning a course of action, gathering resources, and collaborating with workplace parties and other care organizations to transition care recipients with a risk of violence.     Breaking Free Online – Digital Support Tool for Substance Use Disorder: Ontario Health has launched Breaking Free Online, an online therapy tool for Ontarians 16 and up to support recovery from the use of over 70 substances such as tobacco, alcohol, and drugs, including opioids, and help prevent a relapse. To implement, please email Nicholette Smith at nsmith@breakingfreegroup.com to receive your organization’s service code.

    Health Care Navigation Service – Coming Soon: The Ontario government is awarding a contract to create a one-stop, digitally enabled navigation tool with fully bilingual services that will increase access to care and improve equity. This new tool expected to be ready in early 2022, where Ontarians will have a ‘Digital Front Door’ to Ontario’s health care system. Find out more here.

    Improving care across Ontario一and beyond一for children and youth with depression– The Centre for Addiction and Mental Health (CAMH) has launched a new free online tool to help improve the standard of care for youth living with depression. Cundill Centre Online Tool for the Treatment of Youth Depression summarizes best care practices for supporting youth as they manage their depression and is now available for primary care providers and other frontline clinicians everywhere.


    TUTOR-PHC call for applications
    Transdisciplinary Understanding and Training on Research – Primary Health Care (TUTOR-PHC), is a one-year, pan-Canadian interdisciplinary research capacity building program that has been training primary and integrated health care researchers, clinicians and decision-makers from family medicine, nursing, psychology, epidemiology, social work, occupational therapy, education, policy, and many other disciplines since 2003.  The deadline to apply is December 6, 2021.

    It has been a challenging 20 months, and primary care teams have been leaders.

    Throughout this pandemic, teams adjusted to new circumstances and continued to provide comprehensive care while extending reach to support their communities. They were leaders in organizing assessment centres, conducting tests, and administering COVID-19 vaccines. They collaborated with their partners to help keep communities safe, while keeping their doors open to provide care to their patients and others who needed it most.

    Just launched- Indigenous cultural safety training

    The Indigenous Primary Health Care Council (IPHCC) is starting a training series called Anishinaabe Mino’ayaawin – People in Good Health; the first course in the series, Foundations of Indigenous Cultural Safety, was created in partnership with the IPHCC’s Indigenous Knowledge Keepers’ Circle, bioethicists, and Indigenous patient navigators from across Ontario. It is grounded in a two-eyed-seeing approach to knowledge. It’s an online, multi-media course that takes about three hours to complete. Enrolment is free for individuals and organizations.


    2022 Family Medicine Summit call for abstracts

    The 2022 Family Medicine Summit (FMS) will include two days of livestream events – January 28 and 29, 2022 – with dozens of additional sessions available on demand to participants until July 31, 2022. New this year, they have reserved space in the agenda for speakers selected from a Call for Abstracts.

    The Call for Abstracts is open exclusively to family medicine residents and early-career family physicians. Selected presenters will receive guidance and support in preparing to deliver their session. While all abstracts are welcome, they’d be especially keen to hear from someone who has been working on implementing Choosing Wisely Canada recommendations within their practice and/or someone who has been involved in establishing their local Primary Care Network. Submissions are due by November 8.

    Ontario Structured Psychotherapy for Alcohol Use Disorder Survey

    The Ontario Structured Psychotherapy program (OSP) has been designed and implemented for people with mild to moderate anxiety and depression. Screening for substance use including Alcohol Use Disorder (AUD) does occur, however people are not disclosing concerns around their alcohol consumption in initial screenings. Clients often disclose concerns related to alcohol consumption after a few sessions which leaves some clinicians unable to adequately meet their clients needs.

    Addictions and Mental Health Ontario (AMHO), along with the Ministry of Health (MOH) and the Mental Health and Addictions Centre of Excellence (MHA CoE), has convened an advisory committee to recommend next steps for an OSP program for clients with mild to moderate AUD. This survey is intended to gather information that would support the committee in making recommendations to support the development of the program.
     
    The survey will take approximately 10-15 minutes to complete and will be open until November 12, 2021. Please note, this survey was initially designed for people working in the mental health and addiction sector. Not all question will be relevant to all providers, so feel free to only complete sections that are relevant to your care setting.

    Researchers at The Knowledge Translation (KT) Program at St. Michael’s Hospital and Women’s College Hospital are collaborating on a study called the “CANHEART Improving Cholesterol Management Study” to improve lipid management and cardiovascular health in Ontario.

    The Community Heart Outcomes Improvement and Cholesterol Education Study (CHOICES) is an Ontario research study being conducted in certain health regions in Ontario with higher than average rates of cardiovascular disease. The study is being funded as part of Canada’s Strategy for Patient-Oriented Research (SPOR) and by the Canadian Institutes of Health Research (CIHR). CHOICES will involve implementing a ‘toolbox’ for cholesterol management to inform and empower people when it comes to the appropriate management of their cholesterol and cardiovascular health. The tools within this ‘toolbox’, such as cholesterol tip sheets, educational videos, and online decision aids, will be widely distributed in the high-risk health regions, to allow people to work with their primary care physician and share in the decision-making process together.

    Currently, we are looking to recruit primary care physicians Ontario wide to review and provide feedback on an online decision aid tool aimed to improve cholesterol management among patients.     Are you a Family Physician delivering virtual care?

    A study on digital health and compassionate care led by Dr. Laura Desveaux from Women’s College Hospital is exploring how technology influences interactions in primary care. Family physicians who have experience delivering virtual care (e.g., video visits, asynchronous messaging, and remote monitoring) with their patients are being invited to participate in a 30–45-minute interview over Zoom. The interview will explore your experiences with technology, how you have adapted your practice, and what is needed moving forward. A $75 e-gift card will be provided in recognition of your time.   • Letter to the Premier on Mandatory Hospital Vaccinations

    The Premier’s Office recently requested input from multiple stakeholders on making vaccinations mandatory in hospitals with a province-wide strategy.

    AFHTO agrees this is critical in lowering the risk to employees’ and patients’ health from outbreaks, and it helps to address risks to staffing shortages due to illnesses that can be significant and unpredictable. While there will be one-time challenges with mandatory vaccines, the benefits of doing this outweigh the concerns. The risks posed by a vaccine mandate are more predicable, easier to prepare for, and less likely to impact patient care.

    It is also critical that this mandate be extended to primary care and all other community healthcare providers.

    In collaboration with the Alliance for Healthier Communities, the Nurse Practitioner-Led Clinic Association, and the Indigenous Primary Health Care Council, we submitted our response to the premier’s questions on October 19, 2021.
