    Thank you to everyone who submitted an abstract for concurrent session and poster presentations at the AFHTO 2022 Conference. The working groups have now reviewed all submissions and a notification email has been sent to each contact person.

    If you are part of a group that has prepared an abstract, please ensure your group contact has received an e-mail including the words “AFHTO 2022 Conference” in the subject on June 22, 2022. (Please have them check their junk mail as well.)

    If your contact person has NOT received this notification, please contact info@afhto.ca before Monday, June 27, 2022. The notification email is your assurance that your abstract has been reviewed by a working group for presentation at the conference and a decision made.

    The program with all session descriptions will be announced when registration opens in early July 2022.

    Other important updates

    • Hotel: Book by September 19th, 2022, to take advantage of the $285 group rate (plus tax, single/double occupancy); after this date, availability and prices cannot be guaranteed. Click here to book online.

  • AFHTO Bright Lights Awards – nominations now open! Plus, join the review committee

    Shine a light on your team’s achievements. Nominate an outstanding team or individual for a “Bright Lights” Award by July 15, 2022.

    AFHTO’s “Bright Lights” Awards recognize AFHTO members’ leadership, outstanding work and the significant progress being made to improve the value interprofessional primary care teams across Ontario deliver.

    There’s no better time to show what you can do.

    “Bright Lights” Award recipients are:

    • Innovators – making small changes for maximum impact to improve patient experience and outcomes
    • Team Players – interprofessional teams, collaborators, and relationship builders
    • Demonstrating Impact – can show how their work is benefitting the health care system

    Awards will be presented at the AFHTO 2022 Conference Awards ceremony, which will be held at lunchtime on October 12, 2022.

    Not sure if your team’s achievements merit recognition? Never submitted a nomination before?  You can watch this 2016 webinar or view the slides for an overview of what reviewers are looking for and tips for writing a nomination.

    Award Categories

    Awards will be presented in five categories this year:

    • It takes a team: collaboration inside and out
    • Health equity at the centre
    • Sustainable solutions to primary care problems
    • Mental health and addictions
    • Special recognition for underrepresented teams
      • In 2019 Trent Hills FHT won special recognition for providing better care for rural children . This year all new/underrepresented primary care teams again can win an award for their contributions to Ontario’s healthcare system. The winner(s) will be selected based on the strength of their nomination to one of the four categories above.

     

    Shine a light on your team – Make a nomination today:

    • Review the nomination categories in advance (see nomination guide pg. 3 for descriptions).
    • Review the nomination evaluation criteria (see nomination guide pg. 2 for details).
    • Complete the online nomination form and send all supporting materials before July 15, 2022.
    • Submit supporting documents to info@afhto.ca:
      • 1-4 high-quality photos of nominees in png. or jpg. format to be featured at the Awards Ceremony on October 12.
      • Take a video (30-90 seconds long) with your smartphone and share with info@afhto.ca.
      • Signed statement  to release photos for AFHTO use.
      • Any supporting evidence and materials as appropriate.
      • Deadline to submit photos and video is July 27, 2022

    On May 19 Dr. Samir Sinha, Director of Geriatrics at Sinai Health, and University Health Network (UHN) and Director of Health Policy Research at the National Institute on Ageing (NIA) presented on Practical Technologies that can Enable Ageing in Place. Materials are now available here.

How will you ensure there is a sufficient healthcare provider workforce to sustain our healthcare system now and in the future?

    Examining cannabis use and mental health concerns in youth

    Dr. Toula Kourgiantakis from the Factor-Inwentash Faculty of Social Work at the University of Toronto is conducting a research study titled Examining Cannabis Use and Mental Health Concerns in Youth from the Perspectives of Youth, Parents, and Service Providers: A Community-Based Participatory Research Approach. To participate, you must be a service provider (physician, social worker, psychologist, nurse, pharmacist, or psychotherapist) in a health and mental health care setting, who works with or occasionally treat youth and or young adults (aged 16-24).     Reminder- seeking input on the structure of social work practice in primary care

    Dr. Rachelle Ashcroft, an Assistant Professor at the Factor-Inwentash Faculty of Social Work, University of Toronto, is the lead investigator for a research study titled The Structure of Social Work Practice in Ontario Family Health Teams: A Mixed-Methods Study. It involves a provincial-wide survey and semi-structured focus groups with discussions focused on examining the daily practice of social workers in FHTs during the pandemic. Read more here.

Come behind the scenes of our first hybrid conference and be the first to learn about new developments in the field, influence conference programming and discover the latest innovators in areas directly relevant to your work. Working group members also receive a discount off their registration fee.

    Seeking input on the structure of social work practice in primary care

    Dr. Rachelle Ashcroft, an Assistant Professor at the Factor-Inwentash Faculty of Social Work, University of Toronto, is the lead investigator for this research study titled The Structure of Social Work Practice in Ontario Family Health Teams: A Mixed-Methods Study. It involves a provincial-wide survey and semi-structured focus groups with discussions focused on examining the daily practice of social workers in FHTs during the pandemic. Participants will be social workers working in primary care teams located across Ontario.

    Participants will be asked to complete a survey asking about the role of social work, the current and optimal state of practice, and social work leadership. At the end, participants can express their interest in taking part of a focus group. Each focus group will last 60 minutes and consist of social workers working in various team-based, primary care settings.     Interview on the diagnosis and management of concussion in adolescents

    Graduate students are researching the perspectives of community-based primary care providers in Toronto on the diagnosis and management of concussion in adolescents. They’re interested in connecting with a few family physicians, pediatricians, and nurse practitioners working in Toronto for a half-hour interview to participate in their capstone project.

    Survey for a national framework for diabetes in Canada

    The Public Health Agency of Canada (PHAC) is undertaking a virtual engagement process to support Bill C-237 – An Act to Establish a National Framework for Diabetes, which received Royal Assent in June 2021.

    As part of this process, they invite you to provide your input via an online survey platform- EN and FR. Deadline May 9.

    The Ministry of Health has initiated an analysis of the current state and drivers of fax use to identify potential options for phasing out fax use in Ontario’s health system. As part of their first phase, there is an assessment survey with a purpose to obtain input from a variety of stakeholders about their use of traditional paper-based fax and electronic fax.     Reminder- online appointment booking funding webinar tomorrow

    The Ministry of Health has approved funding for Ontario Health Teams (OHT) in development teams and other health care organizations to enable more patients to schedule an online appointment with their primary care provider or other community care provider.

    This funding is intended to support transforming how care is delivered and assisting OHTs and the broader health sector to build their digital health maturity. This funding is also available to primary care and teams that are interested in implementing OAB in their clinics.

Additional discounts apply for patients presenting at the conference. (Discount doesn’t apply for poster displays.)
