  • Central Lambton FHT and North Lambton CHC increase access to testing facility in Plympton-Wyoming

    Beginning Dec. 14, the Rural Lambton COVID Testing Centre will be moving from Petrolia to Plympton-Wyoming.

    Thanks to a partnership between the North Lambton Community Health Centre, the Central Lambton Family Health Team and the municipality of Plympton-Wyoming, the testing facility will now be located at the municipality’s new Health and Wellness Centre, located at 596 Broadway St. in Wyoming.

    Following provincial guidelines, COVID-19 testing will continue to be by appointment only, which can be booked at testing.getcorigan.ca.

    Family health team executive Sarah Milner said that the move will broaden access to COVID-19 testing in Lambton County.

    “Our teams have been working collaboratively for several months now out of the Petrolia testing site,” Milner said. “Shifting testing to Wyoming will further broaden access. We are thrilled to partner with the municipality to provide this important service to the entire rural community. Mayor Lonny Napper and CAO Carolyn Tripp have displayed outstanding leadership through their involvement in this important, provincial health care initiative.”
    “It is important to ensure access to COVID-19 testing in rural locations in Sarnia Lambton to benefit the residents by providing testing closer to home,” said Community Health Centre executive director Kathy Bresett. “Our team looks forward to continuing working together with the Central Lambton Family Health Team and the municipality of Plympton-Wyoming.”

    Mayor Lonny Napper said the new Wellness Centre, located in the former TD Bank building, is perfectly suited for this new function.

    “Plympton-Wyoming along with its community partners being the Wyoming Lion’s Club, Plympton-Wyoming Agricultural Society and TD Canada Trust Bank, are thrilled to be a part of this collaboration,” he said. “We cannot think of a better opportunity to introduce our new Health and Wellness Centre to Lambton County. The town is looking forward to working with the Central Lambton Family Health Team and the North Lambton Community Health Centre and supporting their endeavours in the future.”

    Petrolia & North Enniskillen Fire Department chief Jay Arns said that the department’s partnership with the Rural Lambton COVID Testing Centre will continue at the new site.

    “The CLFHT has been instrumental in the fight against COVID-19 since the pandemic hit. A continued collaborative effort between PNEFD and CLFHT for a COVID-19 testing center since the pandemic started has shown how strong our community is and the great things that can be accomplished with these types of collaborative efforts,” he said. “The new partnership between CLFHT /NLCHC and Plympton-Wyoming including the relocation of the COVID-19 assessment center to the Wyoming Wellness Center is yet another example of how rural communities come together on a united front to provide increased wellness for all residents.”

    Hours of operations, testing processes and holiday hours will be made available on both primary care team and Lambton Public Health websites at www.centrallambtonfamilyhealthteam.com, www.nlchc.com and www.lambtonpublichealth.ca.

    Senior friendly care learning series for clinicians now available in French

    A critical factor in improving outcomes for older adults living with complex health concerns (e.g. frailty) during the second wave of the pandemic and beyond is ensuring that all healthcare providers are knowledgeable and skilled in addressing the unique needs of older adults.

    To support the knowledge needs in Francophone communities, the RGP and the Provincial Geriatrics Leadership Office have launched the senior friendly care (sfCare) Learning Series for clinicians in French.

    The series comprises nine educational modules for clinicians, along with supporting posters and patient handouts on the following clinical topics: delirium, mobility, polypharmacy, pain, loneliness, nutrition, and urinary incontinence. Access the sfCare Learning Series – French

    Ontario Society of Occupational Therapists’ Find an OT search tool can aid Ontarians attempting to access private OT services. It allows the public, heath care professionals, referral agents and others to access a directory of licensed occupational therapists who provide private OT services. Searches can be made based on location, practice area, services, and age group.

  • Team-based Primary Care Pharmacists – Position Statement

    The Canadian Society of Hospital Pharmacists (CSHP)  has released a position statement on team-based primary care pharmacists.

     “Greater involvement of pharmacists in primary care will improve the health of populations, enhance the patient experience of care, reduce the per capita cost of healthcare, and improve the experience and satisfaction of those working in the healthcare system. Team-based primary care pharmacists working collaboratively with other healthcare professionals in this setting also contribute to optimal prevention and management of disease, rational medication use, and cost-effective care.”

    The statement speaks to roles, resolving drug therapy problems, target populations, cost effectiveness, satisfaction or providers/patients, and guidelines for integration into teams.

    As part of our ongoing commitment to keep you informed of organizational transformation efforts, provided here is an update on the discussions underway with the Ministry of Health regarding the intent to transfer Local Health Integration Network (LHIN) non-patient care functions and Trillium Gift of Life Network (TGLN) into Ontario Health.

    As you may remember, the Ministry of Health previously announced the intention to transfer LHIN non-patient care functions into Ontario Health for April 1, 2020. However, due to the COVID-19 pandemic, the transfer was paused to maintain health system stability and to ensure our resources were focused on addressing the pandemic and on protecting the health and well-being of Ontarians. We are now resuming this work and while we do not yet have firm dates, we will be doing some preliminary planning for this to potentially move forward April 1, 2021.

    A survey of interprofessional health care providers in Family Health Teams during the COVID-19 pandemic

    We are now nine months since the initial restrictions of COVID-19 and we are interested in hearing about your experiences working in Family Health Teams during this time. IHPs are invited to participate in a brief web-based survey to understand the evolving experiences of interprofessional health care providers in Family Health Teams in Ontario during the COVID-19 pandemic.

  • What you need to know now that Health Canada has approved the first COVID vaccine

    Dear Members,

    This week, the Public Health Agency of Canada released Canada’s COVID-19 immunization plan, and Health Canada approved our first COVID-19 vaccine. Both represent significant progress in our COVID-19 efforts and are a testament to our scientific community worldwide and the power of unity.

    As provinces look to deploy vaccines, your voice and the role of primary care will be critical. We have shared challenges ahead before hopefully moving to a post-pandemic era, among them logistics, vaccine hesitancy, and more. We know there is no greater antidote to vaccine hesitancy than advice from someone we trust; for most Ontarians, that means a community’s primary care professional.
     
    Although there is no primary care (or public health) representation on the Ontario Ministers’ COVID-19 Vaccine Distribution Task Force, we are working hard to influence and connect with them for the benefit of Ontarians. Partnering with our primary care associations and representatives, we will ensure your community’s needs are heard and that they inform the Task Force’s decisions.
     
    The Task Force is yet to release details regarding its COVID-19 vaccination plan. What we understand is that healthcare workers will be in the first priority group, along with the highest risk members of the population. It is likely that the long-term care sector will be prioritized ahead of community-based primary care settings, but we know that availability of vaccines will increase substantially over the coming months.
     
    We believe it is critical that all of us get vaccinated for at least three reasons:

    1. to protect ourselves and our loved ones
    2. to protect our patients, as well as their families and caregivers
    3. to act as role models for our communities

    The details on how you will access the vaccine for yourselves and your practices is still to be determined. As you have likely heard, there are challenges related to availability and logistics. We understand that the details will be available soon. We are connected with relevant stakeholders and members of the Task Force, and we will share details as soon as we have them.
     
    In the meantime, working collaboratively, we are creating educational materials to understand the vaccine, and to help you have these discussions in your communities. We are also working with stakeholders across Ontario and Canada to ensure you have the resources and support you need to care for your patients.

    Our ask: To help us prepare, we ask you to please complete this important survey from the Public Health Agency of Canada (PHAC) so that we can fully understand your needs, unique challenges, and opportunities as they relate to undertaking the most important vaccine program in our recent history. Please also reach out to us if you have ideas beyond the survey.

    PHAC is also planning a series of national webinars for health professionals. To register for the first webinar, “COVID-19 Vaccine Foundations for Health Care Providers,” please click on one of the following options:

    COVID-19 vaccine update

    As you are aware, yesterday the government released a news release identifying the key groups for distribution of the COVID-19 vaccines. Aligned with the recommendations of the National Advisory Committee on Immunization, early vaccine doses will go to:

    • Residents, staff, essential caregivers, and other employees of congregate living settings (e.g., long-term care homes and retirement homes) that provide care for seniors as they are at higher risk of infection and serious illness from COVID-19;
    • Health care workers, including hospital employees, other staff who work or study in hospitals, and other health care personnel;
    • Adults in Indigenous communities, including remote communities where risk of transmission is high; and
    • Adult recipients of chronic home health care.

    Our understanding is that the definition of health care workers is very broad and will include primary and community care providers though we continue to seek clarity on this.

    To ensure there is a strong primary care voice to the COVID-19 Vaccine Distribution Task Force led by General Rick Hillier, AFHTO has been working collaboratively with our primary care partners to advocate and provide solution-focused ideas so that primary care is at the forefront of the community vaccine deployment. The work is rapidly moving and ever changing but in the next few days we will be sharing some of the activities under way. Please stay tuned.

The webinar series presents helpful and practical tools to use when patients are experiencing physical, emotional, and spiritual suffering as they cope with complicated grief. Special attention is directed to ways for health care providers to support themselves and coworkers during this pandemic.

    Access to clean drinking water to all First Nation and Inuit communities

    The Alliance for Healthier Communities’ Board of Directors has issued a statement and written a letter to the Prime Minister calling for the government to hold itself accountable to its promise for clean drinking water to all First Nation and Inuit communities.

    If you wish to participate in this call to action, you can use this letter template (in English and French). You can tailor it and use it to send letters to your MPs to urge the government to accelerate the work on this issue and provide clear timelines and funding commitments.

    Systems navigator graduate certificate student placement request

    To be eligible for admission, students must possess a bachelor’s degree, advanced diploma or diploma in health, or social/community or other health-related or human services field.  

    Humber College students are prepared to work across various health/community service systems to guide and assist patients, clients and families through complex health and social care systems. Placement runs from May 3rd – June 11th, 2021 and students are to complete 240 hours (full-time, 8hrs/day for 6 weeks) of unpaid internship.     OTF resilient communities fund- update on eligibility

    We regret to inform our members that we’ve recently been notified FHTs are in fact not eligible to apply for the OTF Resilient Communities Fund. Although we had originally been informed that our teams could qualify, upon further investigation Section 3.3 of the document states that “subsidization of government mandated positions or activities” is considered ineligible, and it appears that is where they see FHTs fitting.

    There is nothing in the online eligibility criteria list that directly excludes FHTs like it does call out hospitals, municipalities, etc., and we have engaged in lengthy conversations to determine if there is any way to make FHTs eligible but unfortunately, we have not been successful in these discussions. This is obviously very disappointing and frustrating for all of us. We sincerely apologize for acting on bad information and for creating any unnecessary workload for our members. The electronic newsletter provides regular prevention and cancer screening updates on topics such as the fecal immunochemical test, upcoming educational opportunities, and regional activities which may be relevant to you or your staff.

    We recognize that the pandemic has impacted our teams and continues to impact you in many ways. PPE has been critical during the pandemic to help protect teams’ physical wellbeing; however, the “psychological PPE” is crucial to protect teams’ mental well being.

    Coping with the stress in a healthy manner will allow for you to take care of yourself and in return take care of patients. To support your mental well being, AFHTO has partnered with CMHA’s Mental Health Works program to provide 4 workshops to help build your “psychological PPE”.

    Cochrane FHTmaking Christmas merry for LTC residents

    Parry Sound FHT– since the usual Christmas parties are cancelled, Parry Sound FHT is trying something else – 12 Days of Christmas

    Starting on Dec 9th, Staff/MD participating in the “theme of the day”, are eligible to win prizes. Details will be on posters around the clinic. This staff/MD appreciation initiative is funded by the PSFHT, and two FHOs, and will be supporting 24 local Parry Sound businesses.

    Just a thought if you’d like to celebrate with your teams and foster community spirit as we go into an unusual holiday season.

    Coordinating Care During the COVID-19 Pandemic: An Evaluation of Providers’ Perspectives of their Practices

    You are invited to participate in an exciting research endeavour evaluating existing care coordination practices in Ontario and how

    they have been impacted by the COVID-19 pandemic. The CONCEPT study will evaluate providers’ perspectives on current care coordination practices for patients with multiple comorbidities, and modifications made during the COVID-19 pandemic.  
     
    If you agree to participate in this study, you will be asked to complete a 10-minute survey. You are asked to answer each question based on your opinion of the care coordination provided at your FHT or community-based primary care health centre before and during COVID-19.
