  • Superior FHT and Partners’ Neighbourhood Resource Centre has Great Impact in Sault Ste. Marie

    May 13 – According to Police Service Chief Robert Keetch, the Neighbourhood Resource Centre has been instrumental in decreasing crime in downtown Sault Ste. Marie. The centre, launched by Sault Ste. Marie police last May, serves as an access point for area residents to medical assistance provided by Superior FHT team members and community agencies, such as Ontario Works, Children’s Aid Society and Canadian Mental Health Association. Superior FHT’s lead physician, Dr. Alan McLean, along with a nurse and administrative assistant, run the weekly clinic, with a nurse practitioner switching with Dr. McLean every other week. Services have even been expanded with a weekly smoking cessation clinic run by a nurse as well. The centre’s success may lead to similar programs in the area.   • Northern FHTs partner with CAMH in new Telepsychiatry Program

    May 13 – Based on successful pilots at the East End FHT, Great Northern FHT and Superior FHT, a new telepsychiatry program aims to create a collaborative care model for both patients and providers and increase access to mental health care across the North. Family Health Teams in the North East Local Health Integration Network (LHIN) will be linked by the Ontario Telemedicine Network (OTN) to dedicated psychiatrists at the Centre for Addiction and Mental Health (CAMH) in Toronto who will provide consults with patients and support team members with their clinical questions. According to Dr. Alan McLean with the Superior FHT, the program “has cut down the wait list for very seriously ill patients by more than six months.” Five more FHTs will be linked to psychiatric care during the expansion phase.   • Data to Decisions (D2D) 2.0: Step-by-Step Guide for contributing data now available

    You can get started preparing your data for Data to Decisions (D2D) 2.0 today. The new Step-by-Step Guide for D2D 2.0 provides detailed instructions for compiling and contributing data.

    The online form to submit data will be available on the AFHTO website on Friday, May 15, 2015. All data must be submitted by May 28, 2015.

    Well over half of AFHTO members are working towards contributing data to D2D 2.0. All teams are invited to submit data to the indicators where data is currently available for your team. D2D 2.0 is a membership-wide summary of performance on a small number of indicators that are both possible for members to measure and meaningful to them.
