    • Quality Improvement in Dietetics. This presentation introduces several QI tools that are effective in interprofessional primary care settings. It then explores in-depth several real-life examples of QI initiatives. Participants are then invited to practice using the tools to develop QI plans for challenges they are experiencing in their own teams.
    • Learning How to Get Lucky: Enablers of High-Performing TeamsThis presentation describes what D2D is telling us about enablers of high performance in primary care teams. Some of these factors may be out of teams’ control (e.g. panel characteristics), others, such as single- vs. multi-site design, EMR management, and governance may be amenable to change.
    • The Care and Feeding of QI Specialists in Ontario: The First Five Years. This poster describes the experience of the first five years of the QIDS program, from the perspective of those coordinating, delivering, and receiving QIDS Specialist support.
    • Using Mobile Technology to Collect Patient-Reported Outcomes for Quality Improvement in Primary Care: There’s an App for That (with C. Steele Gray, J. Nie, J. Gravesande, P. Hans, and C. Cott). This presentation demonstrates how mobile technology can be used to improve care for patients with multiple chronic conditions and complex needs and to gather data which support research and system-level planning. Abstract.
    • Readiness for Patient Engagement in Ontario’s Primary Care Teams (with N. Sunderji, V. Tang, A. Ion, and J. Rayner): This poster/presentation describes research into the question of how ready and willing primary care teams are to practice meaningful patient engagement in quality improvement related to mental health care. Abstract.
    • Engaging Ontario’s Primary Care Teams: Learning the secrets of success from those who have achieved it. D2D data reveals that patients of single-site teams had statistically lower per capita health system costs, even when patient complexity is considered, regardless of whether the team is rural or urban. This paper explores how single-site design might be a useful place to start exploring what drives better outcomes in primary care teams.
    • Evidence of the positive impact of co-location on quality and healthcare system costs in Ontario’s primary care teams. This poster describes the finding, observed over 6 iterations of D2D, that single-site teams are associated with lower system costs, and it explores the implications for quality improvement in all team, including multi-site teams.
    • Ontario’s primary care teams are moving beyond measurement to improvement This poster described patters of improvement observed over the first five iterations of D2D. “Increasers” began below average in the first iteration but were among the highest overall performers by the most recent iterations. “Decreasers” began among the highest performers but became the worst performance in the most recent iterations. Differences in characteristics between these groups are being examined and will be shared in the poster.

  • Opioid tapering and tools for use in clinical practice

    Dr. Andrea Furlan, co-chair of ECHO Ontario Chronic Pain and Opioid Stewardship, spoke on November 19, 2018, about opioid tapering and tools for use in clinical practice. The following were the learning objectives: • Describe barriers and facilitators for a successful opioid taper • Develop an approach for managing opioid withdrawal • Remember some tools for use in clinical practice to assist in opioid tapering The slide deck can be seen here.   • AFHTO members recognised in 2018 OCFP Awards

  • Peterborough FHT adds third ultrasound machine

    To be used for Ontario Breast Screening Program general imaging in Peterborough

    The Medical Centre now has a third ultrasound machine in operation to increase access and reduce wait times. The centre received a $16,000 donation from GPHSF, Your Family Health Team Foundation to refurbish an ultrasound machine for the facility.

    It’ll be used for general imaging along with the Medical Centre’s Ontario Breast Screening Program.

    The machine was previously used by a doctor at the centre for breast screening. When he retired, he donated the machine to the centre, but it wasn’t equipped to do abdomens and obstetricals. The probe to do the additional screening was $16,000. GPHSF, Your Family Health Team Foundation supports patient care delivered by the Peterborough Family Health Team. It raises money through fundraisers, such as an annual golf tournament. “If you have a family doctor in Peterborough, then you’re a member of the Peterborough Family Health Team and you have access to all these resources we’ve been funding as well,” foundation executive director Laura Kennedy said at the Medical Centre Tuesday. Cheryl Collins, the centre’s diagnostic services manager, said the new equipment and designated room will speed up the entire process for someone who needs an ultrasound, from wait time to results. The centre has three obstetricians in the building who refer patients for ultrasounds, and other patients from outside the building get referred there as well. The Medical Centre sees between 8,000 to 10,000 patients a year for ultrasounds. With the two machines, Collins said patients could wait a week or more to get an appointment.

    Now, with the additional equipment, patients will be booked in within the same week.

    “Having an additional ultrasound room and equipment will mean we can provide even more service to our community when they need it most,” said Dr. Bob Neville, Medical Centre physician lead. The Medical Centre was established in 1948, with three physicians. Today, it houses more than 35 medical practices, 19 family medicine specialists, five nurse practitioners and several allied health professionals.     Between July 1, 2018, and March 31, 2019, health and community care workplaces may be visited as part of the MOL’s Safe At Work Ontario internal responsibility system (IRS) initiative to promote compliance with the Occupational Health and Safety Act (OHSA) and its regulations..

    The following IRS focus priorities will be integrated into MOL workplace inspections:

    • Training / information and instruction/supervisory competency
    • Personal protective equipment (PPE) / protective devices:
    • Reporting incidents of workplace violence and other reporting requirements
    • Notification requirements
    • Duties of directors and officers of a corporation

  • City of Kawartha Lakes FHT helps keep Woodville Medical open

    The Lindsay Advocate article published on November 12, 2018 By Roderick Benns,The Lindsay Advocate Health care services continue to be available at the Woodville Medical Centre, thanks to a collaborative effort between the Community Care Health & Care Network and the City of Kawartha Lakes Family Health Team. Following the departure of Dr. Muhammed Khan from his practice at the Woodville Medical Centre at the end of October, the two organizations have stepped up to meet the health care needs of Woodville residents through an interim arrangement until March 31, 2019. Primary care professionals from the Community Care Community Health Centre (CHC), including family physician Dr. Jill Caines, nurses and nurse practitioners, as well as medical administration staff, are stepping up. The City of Kawartha Lakes Family Health Team will provide additional support through a nurse practitioner one day a week. The clinic will operate five days a week, Monday to Friday, 9 am to 5 pm. When efforts to find a replacement for Dr. Khan were not successful, the Woodville Medical Trust asked Community Care for assistance. Community Care Director of Clinical Services Margot Fitzpatrick says the organization moved quickly to expand service to Woodville. “At the heart of the community health centre model of care is the ability of residents to receive health care where they live. It has been rewarding to work with our dedicated CHC health care team to keep the Medical Centre open to serve the residents of Woodville,” Fitzpatrick explained. Mike Perry, executive director of the City of Kawartha Lakes Family Health Team said, “I am delighted to join Community Care in finding a local solution for ongoing primary care in Woodville. I really want to commend Community Care for its leadership to help ensure care for patients in the Woodville area.” “Health care takes local cooperation to provide care where people live, especially in our rural area,” adds Perry.   • The most effective outreach for patients overdue for cancer screening, according to St. Michael’s Hospital Academic FHT

    Background: There is good evidence that cancer-specific patient outreach improves rates of cervical, breast, and colorectal cancer screening. However, it is unclear how primary care practices should implement integrated outreach for all 3 types of cancer screening. They aimed to understand whether integrated outreach using mailed letters or phone calls were more effective at increasing screening uptake in a primary care organization.

    Walk or Run to Quit 2.0 feasibility survey

    In December Canadian Cancer Society’s Walk or Run to Quit (WRTQ) will close out the current 3-year project. The results have inspired them to consider a new program design that could reach more smokers with the opportunity to use a physical-activity based cessation program to help them along in their quit attempts.

    They’re seeking your input on the feasibility of having a modified version of WRTQ available for community-based champions to deliver in their own settings. Deadline Nov. 15.

    Queen’s University, Sinai Health System and partners are developing an online education platform to educate primary care physicians and NPs about dementia diagnosis and management. They’re seeking physicians and NPs to participate in focus groups to help them design a system that is valuable and useful to providers. Dates available:

    •  Thurs. Nov 22, 5pm – 14th floor classroom (600 University) – Mount Sinai)
