Summary/Abstract
Using existing data, we identified physician offices in those neighbourhoods of highest need in our OHT geography.  Through our partners, we identified existing resources that could be leveraged to create a team similar to the services provided under the family health team model. This included IPC teams, SCOPE, mental health services and others.  Embarking on an engagement process with the physician offices, we identified those willing partners who would be interested in working with us, and who saw benefit of having access to team-based resources for their patients.  By connecting these physicians to these team resources, we started demonstrating how those complex patients that physicians are typically concerned with, could access resources to better care for these patients.
