Notice of 2018 Annual General Meeting: Reports attached / Confirm your voting Delegate/ If not attending send in your proxy by October 22nd

Dear AFHTO Members:

This Notice of the Annual General Meeting (AGM) is being e-mailed to the Board Chair, Executive Director, Lead Physician or Clinic Director Nurse Practitioner, of each organization who, according to the AFHTO by-law, is eligible to vote. The AGM will be held just before the Leadership Triad Session and the official opening of the AFHTO 2018 Conference with the Bright Lights Awards Ceremony. The Meeting takes place:

Wednesday October 24, 2018
8:30 a.m. to 9:30 a.m.
Harbour Ballroom A&B, Westin Harbour Castle
One Harbour Square, Toronto, Ontario

Each AFHTO member FHT, NPLC, MICC and CHC is entitled to designate one voting representative for the AGM. The member Board Chair has the right to appoint any person associated with their organization as the voting delegate.

Please e-mail sombo.saviye@afhto.ca to indicate who will be voting as representative of your organization before the Annual General Meeting as soon as possible. Voting delegates will be required to register before the meeting to receive their package and voting keypad.

For individuals not able to attend the AGM, attached you will also find a proxy ballot which you can submit 48 hours before the meeting with your vote. Please send to afhto.board@afhto.ca no later than 8:30 a.m. on Monday October 22nd.

The agenda and accompanying reports for this AGM are attached. The AFHTO Board will present:

  • The Annual Report to the members;
  • AFHTO’s financial report including the audited statements for the fiscal year 2017/18; and
  • The proposed acclamation of seven Board of Directors.

There will be an opportunity for you to ask questions and share ideas. You are also most welcome to send me your comments, questions or ideas in advance.

A Message from the President and Chair of the Board, Dr. Rob Annis

As you will read in our Annual Report, AFHTO, with its members, has accomplished a lot in the last year and should be proud of our many collective accomplishments.  Through D2D and our QIDSS in the field, the momentum of moving from measurement to improvement is palpable, especially as we know that team-based primary care has a significant role in delivering on better patient outcomes while reducing costs to the health care system.  We have also seen a significant improvement in our member Boards with the adoption of governance best practices, including the move towards skills-based Boards, while also seeing a cadre of exceptional leaders step up to enhance their own leadership skills through our various leadership programs. Our advocacy efforts in supporting our members who deliver comprehensive primary health care is respected and heard – expanding this care to every patient in every community is the right long-term vision and will be the ongoing strategy for our work.

Our two flagship programs on Governance and Leadership (G&L) and Quality Improvement Decision Support (QIDS) have been key in helping us improve. AFHTO has gratefully received annual one-time project funding from the Ministry of Health and Long-Term Care to help support the programs. However, we are managing new challenges as the new government has frozen discretionary spending of all non-essential funding in all ministries while they do a line-by-line audit and review the financial health of the province. While base funding for the QIDS and G&L staff is still committed, the project funding will not be available at least for this year. This will impact the work we do for our members.

Now, more than ever, we need to hear from our members in a meaningful dialogue on where the direction of the organization should be heading and how to work collectively to get there. During the conference, we will have two opportunities to exchange ideas, and I highly encourage you to attend both:

  • At the Annual General Meeting on October 24th starting at 8:30 a.m. in Harbour Ballroom A&B, Westin Harbour Castle
  • During the New Faces, New Approaches: How to Navigate the New Government fireside chat on October 24th starting at 6:30 p.m. in Harbour Ballroom A&B, Westin Harbour Castle

**For those not able to attend the conference or any of these sessions, we will be doing a synopsis of the discussions, which we will be sending out post-conference.

While it is a challenging time, it is also a time of opportunity, and AFHTO will continue to represent an organized primary care sector that is defined by a culture of skills-based governance, transparent measurement, and the ability to coordinate improvement for provincial problems. This is how system issues can be addressed systemically, and hallway medicine, as one issue among many, will disappear for every patient in every community.
