Raising the Bar on Governance

D2D: Demonstrating Value & Identifying Enablers of Quality
- One of the most exciting stories coming out of D2D 5.0 is the good news that health system costs for patients of primary-care teams are continuing to decline, and high quality primary care is still related to lower system cost.
- The team profile data in D2D is allowing us to identify, for the first time, what team characteristics are associated with high performance. If your team didn’t participate in this aspect of D2D this time around, talk to them about doing so in the next iteration (January-February 2018).
- 36 teams agreed to be unmasked to their peers, which will allow AFHTO members to reach out to teams that performed well on indicators where they’re looking to improve, to get support.
- If you’re connected with your LHIN leaders, consider sharing LHIN-level D2D data with them to engage them in what primary care teams are doing and saying about measurement.
Additional Training, Tools & Resources
- As you start planning your programs and services for 2018-2019, don’t forget the Program Performance Measures Catalogue and other tools developed with AFHTO members, to help make program planning & evaluation easier. Use these tools with your IHPs to make program planning an interprofessional exercise and ensure that you are measuring your programs in a way that’s meaningful to the team.
- Partnered supports for managing pain & addictions are now available, including queries for opioid use created by QIDS Specialists for use with the three most commonly-used EMRs among our membership.
- Partnership with Public Services Health & Safety Association (PSHSA)to equip members with the tools, resources and training requirements needed to meet the Safe at Work Ontario enforcement initiative.

Recruitment & Retention
- AFHTO is currently waiting for the Ministry’s response to the Q & A document.
FHT-MOHLTC Contract Implementation
- Support for teams to meet the new FHT-Ministry contract requirements. AFHTO hopes to receive the final draft contract in the next few weeks!
Regional Workshops
- Co-development of regional leadership workshops for EDs, lead physicians and board chairs to meet with key stakeholders in their region to address relevant priority areas, advance local collaborations, and generate practical, actionable plans to move ahead. (winter/spring)
Advancing LHIN relations & education
- AFHTO continues to stay abreast of the varying approaches and engagement to date across each of the LHINs re. the expansion of interprofessional care teams. Supporting our members in business case development and LHIN relations will remain a key priority moving forward.
- For LHINs to fund FHTs & NPLCs they will have to establish a formal relationship through the signing of provincial Service Accountability Agreement (SAA). AFHTO is participating on the provincial SAA Advisory Committee with the LHINs to help inform the development of these agreements.
