To better serve and represent member needs and interests, AFHTO would like to learn more about the current services your FHT/NPLC provides, and the barriers and facilitators to providing palliative care in the community. Cancer Care Ontario is currently developing education tools and resources to support primary care providers in delivering palliative care. To ensure that these are appropriately tailored to meet your needs, CCO has developed a short survey to collect this important information.
