AFHTO FHT-LHIN Leadership Session: Priorities, Goals and Actions

On October 17 the Association of Family Health Teams of Ontario (AFHTO) hosted a meeting of about 300 FHT and LHIN leaders to delve into the question –

“How do we continue to build a stronger primary care foundation and work in a more integrated manner within the LHIN to ensure coordinated and seamless care for our patients?”

The Priorities, Goals and Actions report:

  • Lists the priorities and cross-cutting themes that emerged across all LHIN groups.
  • Documents the priorities, goals and short-term actions identified by the leaders from your LHIN.

This e-mail was sent to all FHT leaders (board chair, ED, lead MD), LHIN CEOs and LHIN Primary Care Physician Leads. Click here for a list of the people pre-registered for the session.  (This list may differ a bit from the actual attendees.)

FHTs are at various stages of developing their collaborative relationships with other FHTs, other health service providers and the LHIN in their regions.  We trust this report will provide insight and guidance for all leaders to facilitate and support your work to improve care for patients. It will also inform AFHTO’s work as the advocate, network and resource centre for FHTs, as integration of primary care with LHINs develops across the province.
