OR When you see [or visit] your doctor or nurse practitioner, do they provide you with an opportunity to ask questions about recommended treatment? OR Did the person (you saw during your visit today) give you an opportunity to ask your questions/share your concerns? FUTURE: HQO PES standardized question When you see your doctor or nurse practitioner, how often do they or someone else in the office give you an opportunity to ask questions about recommended treatment? Definition Source: HQO PES (modified by AFHTO to allow variations noted) Data Source: Existing patient experience surveys Data Access Notes: For this indicator please use your patient survey responses between April 1 2014 and March 31 2015 only. Numerator: Compile the top two positive survey responses (e.g agree and strongly agree) for each relevant question. Denominator: Compile the total number of survey responses for each relevant question Teams whose surveys did not include the relevant questions will not be able to contribute data for these indicators. They may consider including these questions in subsequent surveys.
