Expanding and Enhancing Interprofessional Primary Care Teams – Expression of Interest Now Available

Dear AFHTO Member EDs, Lead Physicians and Board Chairs,

Over the past year, AFHTO has kept primary care team expansion and enhancement at the center of our advocacy efforts. Bringing the success stories of our members to the forefront in our conversations with government has been instrumental in demonstrating the positive impact your teams have across the province. As many of you know, the government recently announced new funding to expand existing interprofessional primary care teams and/or create up to 18 new teams in communities with the greatest need.  

The Ministry of Health and Ontario Health have officially released a call for Expressions of Interest (EOIs). While the $30 million set aside for this investment is a positive step, we know it will not be enough to meet current demand and needs. However, we believe there is an opportunity for further funding to become available pending the Federal Health Transfer to Ontario. A strong response to the EOI process could help convey the sector’s urgent need for further support and funding.

EOIs must be submitted by June 16, 2023 at 5:00 pm. The application process will require time and effort, but there are resources available. More information on the application process and associated supports can be found below.

EOI Information from Ontario’s Ministry of Health:

Ontario is supporting primary care by building collaborative networks and expanding access to team models of care. Thousands of patients in communities across the province are connecting to health care quickly and closer to home. Additional funding is available to increase access to team models of care in communities with the greatest need.

The Ministry of Health is moving forward with an Expression of Interest (EOI) process to collect and evaluate proposals for new and expanded interprofessional primary care teams.

Apply for Funding
Completing an Expression of Interest form is a requirement for funding consideration; it will be evaluated against specific criteria. The ministry and Ontario Health are working together to manage the application process. Ontario Health will work with local primary care and community partners to support proponents’ EOI applications.

Information and Support
The EOI form as well as related information and tools are available at https://www.ontariohealth.ca/system-planning/funding-opportunities-primary-care. Completed applications are due to Ontario Health by 5 pm Eastern Daylight Time on June 16, 2023.  

On Thursday, May 18, Ontario Health and the ministry will host two technical webinar sessions to guide applicants through the EOI form. There will also be an opportunity to ask questions about the EOI form and the process. The webinars will take place at 8 am to 9 am and at 4 pm to 5 pm. L’Ontario fortifie les soins primaires en créant des réseaux de collaboration et en élargissant l’accès aux modèles de soins assurés par des équipes. Des milliers de patients dans les communautés de toute la province sont connectés aux soins de santé rapidement et plus près de chez eux. Des fonds supplémentaires sont disponibles pour accroître l’accès aux modèles de soins assurés par des équipes dans les communautés qui en ont le plus besoin.

Le ministère de la Santé va de l’avant avec un processus de déclaration d’intérêt (DDI) pour recueillir et évaluer des propositions pour équipes de soins primaires interprofessionnelles nouvelles et élargies.

Demander un financement
Remplir un formulaire de déclaration d’intérêt (DDI) est une exigence pour l’examen du financement et sera évalué en fonction de critères spécifiques. Le ministère et Santé Ontario collaborent pour gérer le processus de candidature. Santé Ontario collaborera avec les partenaires locaux de soins primaires et communautaires pour soutenir les déclarations d’intérêt des promoteurs.

Démarches pour présenter une demande
Le formulaire de déclaration d’intérêt ainsi que les informations et outils connexes sont disponibles à Possibilités de financement de soins primaires | Santé Ontario (ontariohealth.ca). Les demandes dûment remplies doivent être remises à Santé Ontario au plus tard le vendredi 16 juin 2023, à 17 h (HAE).

Le jeudi 18 mai 2023, Santé Ontario et le ministère organiseront deux webinaires techniques pour guider les candidats à travers le formulaire de déclaration d’intérêt. Il sera également possible de poser des questions sur le formulaire DDI et le processus. Les webinaires auront lieu de 8 h à 9 h et de 16 h à 17 h.
