
Re-use and Recycle your EMR data: Your conference lanyard said it best! Use queries developed and tested by QIDSS to find patients you can do good things for related to opioid use (which may signal pain or addictions management challenges) and chronic disease (COPD, CHF, diabetes, depression). Providers across the province (i.e., outside of AFHTO) are starting to use these queries too, thanks to the work of the QIDSS and the Algorithm Project team. And they’re all free!
Help for your patients with pain and addictions. The opioid query is a good place to start if you want to find out who exactly these patients are. Once you’ve found them in your EMR, you can do more to help them help themselves, using these tools from the eHealth Centre of Excellence, the Centre for Effective Practice, the OCFP, and CAMH. They’re coming out of it with more energy and focus than ever. Join them to share your questions and answers and make friends on your EMR journey. More meaningful program measurement: Use the Program Performance Measures Catalogue to find indicators that your peers are using to tell the story of how well their teams are doing. Working with your IHPs and using the catalogue will make program planning and evaluation easier and more meaningful for the whole team. What makes AFHTO’s bright light shine? You do! Thanks to the nearly 90 teams who completed the team profile survey as part of D2D. Now we’ve got lots of data to help us understand how teams work together and how that affects the quality of care they provide.
