• OHIP+ Webinar: AFHTO will host a webinar on OHIP+: Children and Youth Pharmacare facilitated by the Ministry of Health and Long-Term Care on Nov. 30^th, 2017 from 1:00 p.m. to 2:00 p.m. Learn what changes this program will bring to your patients.

• Partnered Supports for Managing Pain & Addiction: Partnered supports are available for managing pain and addictions from AFHTO, OCFP, Centre for Effective Practice, the eHealth Centre of Excellence, and HQO. These QIDSS-developed EMR queries can help identify patients who would benefit.

• Ontario Community Hubs resource network: is asking for your help in adding information and resources to the site. This request was brought to our attention by a fellow AFHTO member who noted, “Collaborative hubs are especially useful in our rural and northern communities; however, the concepts are useful in serving multiple urban populations as well, especially those that have higher health equity challenges.” Learn more here.

• Ontario’s Primary Care Diabetes Prevention Program – Implementation Manual for Your Primary Care Organization: released by Nutrition Resource Centre (NRC) and Physical Activity Resource Centre (PARC).

• Governance Centre of Excellence Board Self-Assessment Tool: free tool available on their website until December 1, 2017. Each member can anonymously complete an online questionnaire, and the GCE creates an aggregate board-level report from the individual responses. Contact cmcnamee@thegce.ca for more information.
  

• Mobile medical app available for patients with hypertension and primary care providers: BProactive prompts patients to take their blood pressure readings, which are then sent directly to a clinician-accessible portal via a secure cloud service. It alerts you when a patient is not meeting their targets and that a follow-up assessment is required. Visit the site to learn more or sign up for a free account.
