Patients can (and want to) play a role in better follow-up care after hospitalization. Now there’s a tool for that! The Patient-Oriented Discharge Summary (PODS) is tried, tested and FREE. It’s a simple form YOU can use to get hospitals, patients and your team literally on the same page when it comes to follow-up after discharge.
