Watch for your next team-level Primary Care Practice Report (PCPR). HQO has indicated that it’s coming your way in the first week of June, if you signed up by March 31, 2017. Check it out: PCPR was in D2D; now D2D is in the PCPR! The summary pages now include links to D2D. All D2D elements, including the new demographic indicators, are highlighted to help you with data submission. If you haven’t done so already, now is your last chance to get a PCPR in time for D2D 5.0. Sign up by June 30 to receive the August report – just in time for D2D 5.0 data submission (open mid-August through mid-September). You want to know what it takes to get better – so we’re asking you. New contextual indicators in D2D 5.0 (everything from board structure to EMR data access) will paint a picture of what makes some teams so successful in their improvement efforts. Like everything else in D2D, participation is optional.
