D2D is the first leg of a journey, not the destination. The following improvement initiatives and activities will help you move from measurement to improvement:
- Identify your patients with Congestive Heart Failure (CHF) to plan appropriate care using the new standardized, tested EMR query. Want to learn more about improving outcomes for CHF patients? Free webinar on Thursday, October 6.
- IHP Networking Sessions on October 17 at the AFHTO Conference: Encourage your IHPs to join these free sessions dedicated to building interprofessional collaboration. This year all groups will be contributing to a research study to understand what it means to provide good interprofessional primary care.
- Moving the Needle on Diabetes Care: Learn how your peers are making progress with diabetes care through better measurement in a presentation at the free HQO conference on October 20.
- Making Schedule A make more sense: A refreshed catalogue of indicators is in the works to make program planning easier and Schedule A more meaningful. We’re presenting about this process at the HQO conference.
