Login information for the Health Quality Ontario (HQO) QIP Navigator has been sent to all primary care organizations. All QIPs will need to be submitted through the QIP Navigator by April 1, 2014. If you have any difficulties logging in or have not received your password please e-mail QIP@HQOntario.ca for assistance.
