Chronic obstructive pulmonary disease quality standard implementation toolkit
“In partnership with the Lung Health Foundation, Ontario Health’s Clinical and Quality Standard Program has released a Chronic Obstructive Pulmonary Disease (COPD) Quality Standard Implementation Toolkit to support the implementation of the COPD: Care in the Community for Adults quality standard in Ontario. This implementation toolkit features a clinical care pathway that includes a suite of evidence-based resources and digital tools that can be used by clinicians, health system planners, and health organizations to improve care for people with COPD in primary and community care settings. Education and self-management resources and tools for patients and their care partners are also featured.”
Sickle Cell Disease Community of Practice April webinar
On May 15, 2024, Ontario Health launched a Sickle Cell Disease (SCD) Community of Practice. The goal of the community of practice is to create a community that will champion improving the quality of care that people with SCD in Ontario receive across all settings, through the implementation of the SCD quality standard.
The SCD Community of Practice is hosting its next webinar on April 23, from 12:00 to 1:00 p.m. The focus of the session will be Community Health Centres Coordinating Care to Support Implementation of the SCD Quality Standard.
Participants will learn about the collaborative strategies between community health centres and specialized SCD centres, in both pediatric and adult care settings across Ontario. A panel of experts will explore tools and approaches to improve communication and care coordination to support people with SCD. This will include guidance for implementing approaches to improve care coordination from Ms. Serena Thompson (Lived Experience Advisor, Patient Advocate), Ms. Cheryl Prescod (Nurse Practitioner, Executive Director, Black Creek Community Health Centre) and Mr. Liben Gebremikael (Chief Executive Officer and Executive Director, TAIBU Community Health Centre).
Primary care and specialist physicians can receive Continuing Medical Education (CME) credits for attending the webinar.

Osteoporosis toolkit
“Osteoporosis Canada and the Ontario Osteoporosis Strategy are pleased to announce the release of the updated Osteoporosis Toolkit for Telus Health PS Suite Electronic Medical Record (EMR) systems. The new version of the Osteoporosis Main Form has been updated in partnership with the eHealth Centre of Excellence (eCE) to align with the Clinical Practice Guideline for Management of Osteoporosis and Fracture Prevention in Canada: 2023 Update.
The updated form integrates with the Telus Health PS Suite EMR, providing an efficient way to assess fracture risk and deliver targeted care, ultimately improving patient outcomes. The development of Accuro and Oscar EMR custom forms are currently in progress. In the interim, the Osteoporosis Main Form PDF is available for immediate use.
