Ontario Disability Support Program Health Care Professionals’ Virtual Guide
“The Ministry is enhancing its efforts to help reduce administrative burden for health care professionals (HCPs) who complete the disability applications for their patients applying for the Ontario Disability Support Program (ODSP). To achieve this, we have redesigned our HCP virtual guide to better support clinicians in completing the ODSP application on behalf of their patients.
The HCP guide will reduce the number of forms that are incorrectly filled out and highlight the range of HCPs that can complete the forms.
Additionally, the virtual guide will introduce HCPs to the digital pathway of the DDP that is available through the Ministry of Health’s Special Authorization Digital Information Exchange (SADIE):
- SADIE is available to all Ontario Physicians and Nurse Practitioners
- the digital format is a quicker alternative to the paper ODSP application.”

Reminder- nominations to the AFHTO Board of Directors open until August 1, 2024
Are you interested in serving on the AFHTO board of directors? The Governance Committee of AFHTO’s board invites anyone who works within an AFHTO member organization to apply. Please share this call for nominations with all who work in your team and with members on your Board.
No prior experience as a Board Director is required. The AFHTO Board of Directors provides orientation, training and ongoing support to board directors. The Board counts on you using the skills you bring to the Board and will support your learning about governance and the role of the Board.
Three to six (3-6) positions are to be elected for a 3-year term on the AFHTO board. The AFHTO by-law calls for balanced representation on the board to include the various forms of governance, the regions of the province, and the mix of the professions/individuals working within FHTs, NPLCs and other interprofessional models of primary care. The Board award recognizes an individual (or a group) who is not an AFHTO member but has significantly contributed to the development and growth of team-based primary care within their local community or at the provincial level.
The winner(s) will receive their award(s) during the Bright Lights Awards Ceremony at the AFHTO 2024 Conference on October 24, 2024.
Submit a nomination online for the Board Award category by August 2, 2024.

New update- Ontario Health’s Major Depression Quality Standard
“Ontario Health has released an updated quality standard on major depression. Major depression is one of the most common mental illnesses, imposing a substantial human and economic burden on people and society. Originally released in 2016, the Major Depression quality standard includes 12 quality statements aligned with the most recent clinical evidence and current practice in Ontario. Please review and share the updated quality standard, placemat, and patient guide with your networks. For more information, please contact QualityStandards@OntarioHealth.ca.”
