Summary/Abstract

Nurses, when working to their full scope of practice, support patients in family health teams on the breadth of interdisciplinary programs and services. This includes: wellness and prevention, chronic disease management, diabetes care, mental health, system and care navigation and coordination, etc. A handguide was developed to enable nurses to develop and refine competencies needed for safe and effective practice, act more autonomously and increase confidence. It outlines common duties and responsibilities which are evidence based and reflect clinical best practice guidelines. By compiling this information in a single document, it results in increased efficiency, time management, and time spent on delivering patient care.
