AFHTO 2023 Conference: vote for your streaming sessions

It’s time to vote – which concurrent session should be livestreamed at the conference?

This year, in addition to our plenaries and awards ceremony, we’ll livestream one concurrent session per timeslot (descriptions here) for our virtual attendees, for a total of three sessions. But we need you to help us decide!

Pick your favourite sessions by Aug. 31. The winners will be streamed from the main ballroom.

The Ontario Breast Screening Program (OBSP) has shared changes to:

  1. Enhance breast density notification to OBSP participants,

Interested in adopting the MINT Memory Clinic model?

“We are currently looking for primary care organizations that would be interested in adopting the MINT Memory Clinic model (https://mintmemory.ca/) as part of their primary healthcare team services. Our hope is to get funding to support all or part of the training for your staff.

This model has been running successfully in over 110 sites in Ontario and 14 sites in other provinces across Canada, enabling primary care to diagnose and manage persons living with dementia and other memory conditions and supporting their family care partners.

Please email: michael.lee@mintmemory.ca to indicate your interest or for any other questions.”

New prediabetes and type 2 diabetes digital tools & supports

With the emergence of new medications with weight loss and cardiovascular benefit as well as biosimiliar insulins, clinicians need easier point-of-care access to evidence-based tools to support more personalized management and improve patient health outcomes. These new tools and supports for prediabetes and type 2 diabetes integrate the most up-to-date evidence and quality standards into front-line primary care clinical systems to support clinicians and enhance patient care.

These tools and supports are brought to you by the Centre for Effective Practice, eHealth Centre of Excellence and North York General Hospital with funding and strategic guidance provided by Ontario Health and the Ministry of Health.
